STATEMENT OF WORK

CNC Horizontal Boring and Milling Machine April 24, 2017

1. DESCRIPTION OF EQUIPMENT TO BE PROVIDED CNC

HORIZONTAL BORING AND MILLING MACHINE:

1.1 CNC Horizontal Boring and Milling Machine shall meet the following minimum characteristics:

Control CNC Fanuc type control or equal

Spindle Diameter 4.33”

Spindle Taper ISO 50 (7:24)

Spindle motor 15 kW (20hp)

Spindle speeds 5-2000 rpm

Facing head speed 5.4 -140 rpm

Rotary Table size: Minimum 5’ x 6’

2. DESCRIPTION OF WORK AND SERVICES: Turnkey package is required to include shipping, delivery, placement, assembly, and set up in building 8A at USCG Coast

Guard Yard. Coast Guard Yard will connect the horizontal boring and milling machine to an existing power disconnect and existing airline. It is the contractor’s full responsibility to provide any material required to perform the stated requirements under the terms of this statement of work. This product will be installed with the manufacturer’s recommended methods, and the final setup and pre-production test routine must be accomplished by a factory trained technician who is knowledgeable of this specific product to prevent exclusion of manufacturer’s warranty.

2.1 In addition the contractor shall:

1. Provide all operator and maintenance manuals, electrical, mechanical and foundation drawings.

2. Provide the manufacturer’s standard full parts and labor warranty, no less than one year full warranty required.

3. Provide Technical support at the time of delivery and set-up.

4. Shall insure the machine meets all of the design requirements, specifications, and

OSHA safety requirements.

5. Operate the machine after set-up to insure there are no discrepancies with parts or components during testing and operation.

6. Provide all fluids as required for machine start-up and dispose of excess and residues.

7. Dispose of all shipping containers and associated items used in the shipment of the machines.

8. Provide on-site operator, mechanical maintenance, troubleshooting, and repair training for up to seven people.

3. INSPECTION AND ACCEPTANCE OF SERVICES: Inspection shall take place at destination and shall be contingent upon verification of no damage in transit, correctness and completeness of the order, and conformance to preservation, packaging, packing, and marking requirements. Final acceptance shall take place only after successful completion of installation and testing.

4. PERSONNEL: All personnel shall receive visitor’s passes and clearance from USCG

Coast Guard Yard Security located at the Main Gate. List of personnel needing access shall be provided to the COR twenty-four (24) hours prior to requiring access. Personnel will be required to provide full name, picture I.D. (valid driver’s license preferred), and date of birth.

For non US citizen’s, a passport will be required. The following forms of IDS are authorized:

- CAC - Foreign Passport containing appropriate visas

- TWIC - Green Cards

- Passport - MOST state IDs

The following state ID will not be accepted

- Maine - Missouri*

- Minnesota - Washington*

- Montana

*- Enhanced Drives Licenses (EDL) from Missouri and Washington will be accepted.

5. PHYSICAL SECURITY: The contractor shall coordinate with the COR to obtain vehicle passes for each vehicle used on USCGC Yard during the setup and training period.
