Where possible, trend analysis should be included. To the maximum extent possible, effective information technology tools should be used to streamline processing of survey-respondent lists, identification of topics, conduct of the surveys, and reporting of results back to each Head of participating DoD organizational elements. In most cases, raw survey results should also be available via web displays for both respondents and government staff. After the results are received, analyzed, and summarized, the respective results are provided Heads of DoD organizational elements. This survey capability should be overseen by a dedicated professional technical staff with expertise and experience relating to surveys and statistical methods, including, but not limited to, evaluating previous survey projects (current and previous respondents), specific tasks performed, range of type surveys, samples of results, size and diversity of survey population, and qualifications of analytical staff (educational degrees in statistics, analysis, etc).
All surveys must be in compliance with applicable DoD regulations, including DoDI 8910.01
"Information Collection and Reporting" and DoDI 1100.13 "Surveys of DoD Personnel." The contractor will assist O&MP with the timelines and process of obtaining all requisite clearances and approvals.
H.12. Responsiveness:
An integral part of successful performance under this contract is not only the production of quality products described in Section C, Performance Work Statement, but the short notice, quick turn timelines, initiative, attitude, ability and readiness to rapidly reprioritize. The activities of contractor personnel in the day to day oversight and administration of assigned portfolios. While the end product or deliverable is vital to successful performance, day-to-day oversight also includes client interaction and responsiveness. Accordingly, the contractor is required to proactively maintain assigned portfolios and be responsive to all entities with business dealings related to the assigned portfolios.
Participating contractors should be capable of responding to emerging unknown demands with as little as two weeks advance notice. As a result, the participating vendors should have a significant pool of employees who are both familiar with the activities of O&MP and available to quickly support the full scope of work.
L.1.1.3. Past Performance (FACTOR B):
a. The contractor may submit no more than three (3) typewritten pages for the past performance section with reference to section c below. All other past performance input must come from your customers through the Past Performance Questionnaire, Attachment 5. There is no page limit for the Past Performance Questionnaire.
b. The offeror’s customer must submit past performance information on the attached past performance questionnaire. Each past performance questionnaire must correspond to each experience example submitted (reference L.1.1.2.b (Experience)). The POCs identified on the questionnaire must be able to provide information on the quality of your performance.
c. Offerors are authorized to describe any problems encountered on the identified contracts and any corrective action taken. Each description shall not exceed one (1) page in length.
d. Offerors must either provide past performance information or the offeror must state affirmatively that they possess no relevant past performance. Offerors with no relevant past performance shall receive a past performance rating of neutral.
e. Send past performance information to catherine.stevens@whs.mil by September 7, 2012 at 5:00 P.M. EST. Failure to submit past performance information by this date or failure to send the prescribed email indicating that an offeror lacks relevant past performance means the proposal will be late; therefore it will be ineligible for award.
mailto:Elizabeth.Johnson@whs.mil
a. Notes:
1. WHS/AD reserves the right to determine which contracts submitted by the
Offeror are relevant to the requirements and to utilize only those references.
2. In the conduct of its past performance evaluation of Offerors, WHS/AD may use a variety of information sources in addition to information provided by the
Offeror. These sources may include, but are not limited to, technical reports, commercial or any available published information, and information derived from present or past Government or commercial customers of the Offeror.
WHS/AD may use past performance information obtained from sources other than those identified by the Offeror. Those Offerors who have no relevant past performance history will not be evaluated either favorably or unfavorably on past performance.

M.5.3. Past Performance (Factor B):
Past performance is the degree to which the contractor has satisfied its customers. Our evaluation will be subjective and based on your reputation with your customers. Some or all of your customers may be contacted and asked if they believe you were capable, efficient, and effective, if your performance conformed to the terms of your contract, if you were reasonable and cooperative, and if you were committed to customer satisfaction.
The Government reserves the right to determine which past performance examples submitted by the Offeror are relevant to the requirements. In the conduct of its past performance evaluation of
Offerors, the Government will evaluate past performance information submitted by your customers. However, the Government may use a variety of information sources in addition to information provided by the Offeror’s customers. These sources may include, but are not limited to: technical reports, commercial or any available published information, and information derived from present or past Government (i.e. federal, state, or local) or commercial customers of the Offeror.
The Government may consider past performance information regarding predecessor companies, key personnel who have relevant experience or subcontractors that will perform major or critical aspects of the requirement when such information is relevant. Failure of a contractor to disclose a relevant Government contract with poor past performance may affect the contractor's past performance rating.
A vendor who has past performance within DoD accomplishing organizational and management planning tasks is considered more important than performance with other Government agencies.
In accordance with FAR 15.305(a) (2) (iv), an Offeror without a record of relevant past performance or for whom information on past performance is not available will not be evaluated favorably or unfavorably on past performance (neutral evaluation).
