SECTION SF 30 BLOCK 14 CONTINUATION PAGE

Solicitation HDEC08-11-R-0003 to furnish all supervision, personnel, equipment, and supplies necessary to perform shelf stocking, receiving/storage/holding area (RSHA), and custodial operations for the Fort McCoy Commissary located on Fort McCoy, Wisconsin is hereby amended as follows:

1. Questions of a technical and/or contractual nature are provided at Enclosure 1.

2. Attendees at the February 9, 2011 site visit are provided at Enclosure 2.

3. Contract closing date has been extended to March 17, 2011 at 3:00PM.

SUMMARY OF CHANGES

ENCLOSURE 1

ATTENTION PROSPECTIVE OFFERORS:

Reference is made to solicitation HDEC08-11-R-0003 for shelf stocking, RSHA, and custodial operations at the Fort McCoy Commissary located Wisconsin.

The following questions and answers, pertaining to the above-referenced solicitation, are provided for your information. Offerors are cautioned that these answers do not alter the solicitation. Only the amendment to the solicitation will change the requirements or any other content of the RFP. In the event of an inconsistency between statements contained in any amendment on the solicitation and this document, the amendment shall take precedence.

Question 1:

The Baby Food reference in section 4.3.3.3, under Merchandise Rotation, is already listed in Exhibit 4-1 as being ‘not a part of the contractor responsibility’

Answer 1:

The contractor is responsible for the stocking and rotation of baby formula. As stated in Exhibit 4-1 – List of Categories/Items Authorized for Vendor Stocking - Contractor is not responsible for stocking and rotation of “Baby foods, baby supplies, e.g., rubber pants, bottles and bottle accessories”

The reference to 4.3.3.3. Merchandise Rotation. Indicates how baby formula will be rotated. ”… When the Contractor is responsible for stocking baby formula, the Contractor shall rotate Contractor-stocked baby formula by the code date indicated on the product to ensure product is by date sequence.”

Question 2:

1.3.1.2. Training. The Contractor shall ensure that his/her personnel, who operate power industrial trucks, are trained, qualified, and certified IAW Occupational Safety and Health Administration (OSHA) requirements. The Contractor shall maintain a record of this training and provide a roster of trained personnel to the Store Director.

Exhibit 2-2 List of Government Furnished Equipment

a) Question 2A: Is the Government responsible for maintenance and repairs of Government Furnished Equipment?

Answer 2A: Exhibit 2-2 page C2-4 states,” The Government will provide the shared equipment listed below to the Contractor for use when performing work under this contract.” The Government is responsible for the maintenance and repair of the equipment listed.

b) Question 2B: Please explain any co-management requirements for Government Furnished Equipment (GFE). What is the notification procedure for repair requests?

Answer 2B: Contractor is responsible for the notification of any needed repairs on GFE to a Commissary Government Official. Under paragraph 1.3.1.2. Training – Contractor shall provide training to personnel who operate Government-furnished vehicle or material handling equipment.

c) Question 2C: Please provide the most recent maintenance history on all Government Furnished Equipment.

Answer 2C: Government is responsible for the maintenance of all Government Furnished Equipment (GFE). The following information is provided regarding maintenance of GFE:

Hand Pallet Jacks (Qty 4) No maintenance required.

Electric Pallet Jacks (Qty 2) Distilled water added to batteries as needed,; both machines were serviced in March 2010 which included were greased and adjusted.

Electric Fork Lifts (Yale) (Qty 1) No maintenance required

Snow Blower (Qty 1) Oil change performed on November 12, 2010; drive belt was replaced December 12, 2010.

Question 3:

3.1.2 End Caps and Other Off Shelf Displays: The Contractor shall move empty end caps and other off shelf display pieces; sweep and mop underneath, wax if needed to maintain floor care standard; clean outer surfaces of end caps and display pieces; and return empty pieces to their original locations.

Question: During the site visit Contractors were told that they will be responsible for moving/dismantling stock from end caps and shelf displays to clean underneath – please clarify this requirement?

Answer 3: 3.1.2. End Caps and Other Off Shelf Displays. The Contractor shall move empty end caps and other off shelf display pieces; sweep and mop underneath, wax if needed to maintain floor care standard; clean outer surfaces of end caps and display pieces; and return empty pieces to their original locations. The Government will coordinate its display dismantling/building schedule with the Contractor; so that the Contractor can perform this task after display merchandise has been removed from the end caps or off shelf display pieces. The Contractor is not required to move stock to or from end caps or other off shelf display pieces if the requirement to build/dismantle displays is not in the contract. This applies to all end caps and other off shelf display pieces and racks used to display items for sale, regardless of whether commissary employees, Contractor personnel, or vendor stockers normally stock these displays and racks.

Question 4:

4.3.3.4 Height Stocking, as observed during the site visit, product appears to exceed the 6.5 foot height requirements, is this in compliance with safety requirements?

Answer 4: The height standard is considered acceptable within the safety limits established with the DeCa safety regulation. The PWS height indicated in 4.3.3.4. is an old standard, to be amended at a future time.

Question 5:

1) 13.1 Snow and Ice Removal: The Contractor shall provide snow and ice removal from front of the building, entrances/exits, to include emergency exits, loading docks and snow/ice accumulation on roof overhangs at entrances, exits and loading docks as required.

Question:

Is the Contractor responsible for snow removal at the lower loading dock and the upper portion of the loading dock?

Answer 5: Contractor is responsible for snow and ice removal for the entire loading dock, both upper and lower dock

Question 6:

How should the Contractor bill during ‘sidewalk sales’?

Answer 6: Sidewalk sales are not a requirement of the Perfromance Work Statement.

Question 7:

How should the Contractor expect reimbursement payments associate with Rapid Gate processing cost?

Answer 7: Rapid Gate is a reimbursable expense, upon the receipt of the paid invoice from the contractor as issued by Rapid Gate Inc., the appropriate line items will be established in the contract, and contractor will be reimbursed.
