NOTE: This must be included for ALL services contracts, to include task orders issued against existing IDIQs or GSA schedules

QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1. Contract or Task Order Title (Add contract/task order number at award): Program Protection Services Support Contract

2. Work Requirements:

Task 1

· Subtask 1 – Onsite Program POC/Program Protection/Program Security Support Assistant

· Subtask 2 – Program Protection Security Support Assistant

· Subtask 3 – Information Systems Security Manager Assistant

· Subtask 4 – Special Security Representative (SSR)

Task 2

· Subtask 1 – Program Manager, Program Protection/Program Security Support

· Subtask 2 – Program Protection/Program Security Support Assistant

· Subtask 3 – Physical Security/Security Education and Training Specialist

· Subtask 4 – Personnel Security Specialist

· Subtask 5 – Information System Security Specialist

3. Primary Method of Surveillance: The government primary method of surveillance for all tasks shall be:

· Periodic Inspection: These services are monitored weekly, monthly, quarterly, semiannually, annually, etc. Periodic types of activities are perfect for periodic inspection because not only are they infrequent, but there is normally a predetermined, specified time frame within which the tasks must be accomplished. Therefore, you know exactly when to conduct the evaluations. Periodic inspections automatically become 100 percent evaluations or "100 percent checks." Inspections should be divided and scheduled by frequency: annual, semiannual, quarterly, monthly, weekly and as required. Sometimes services are required for which the time or frequency cannot be predicted, such as accident investigations, one-time special tasking by higher headquarters, etc. These would be labeled "as required inspections." Others are known and predictable such as the quarterly status report or the monthly travel report currently included in some DISA service contracts.

4. Scope of Performance:

The contractor will provide qualified personnel in support of DISA’s Program Protection analysis services and information system security for the Agency and the associated security support needed to perform this mission.

5. Performance Standards:

· Reports are submitted on time with no errors. Problems that occur are minor and are resolved in a satisfactory manner 90% of the time

6. Mandatory or regulatory compliance items:

The contractor will provide initial proof of compliance with regulatory and compliance items contained in the contract. The Contracting Officer's Representative (COR) and the Contracting Officer (KO) will evaluate the initial submission and will conduct ad hoc reviews of the contractor's compliance throughout the remainder of the contract. The contractor will monitor compliance as required by FAR 52.222-50, Combating Trafficking in Persons, and report any issues to the KO.

7. Acceptable Quality Level (AQL):

The AQL are structured to allow the contractor to manage how the work is performed while providing negative incentives for performance shortfalls. For all activities, the desired quality level is established at one hundred percent (100%). All are keyed to the relative importance of the task to the overall mission performance. Some activities or tasks may not be well defined. The AQL allows for the government to be flexible in determining scope, quality and performance.

8. Evaluation Method:

Government shall use the monitoring methods cited to determine whether the performance standards/service level/AQLs have been met. If the contractor has not met the minimum requirements, it may be asked to develop a corrective action plan to show how and by what date it intends to bring performance up to the required levels. The contractor will be monitored for compliance to required FAR clause 52.222-50, Combating Trafficking in Persons, and report any issues to the Contracting Officer.

9. Incentives (Positive and/or Negative):
