About this file
This is a Massachusetts Substitute Form W-9, a tax identification and certification document used by the Commonwealth of Massachusetts for vendors conducting business with state agencies. The form is required from all vendors, regardless of contract type or value, and serves as the primary mechanism for collecting taxpayer identification information and certifying compliance with state and federal tax requirements. Vendors must complete all required fields including business name, federal tax classification, legal and remittance addresses, and appropriate taxpayer identification numbers (Social Security Number for individuals or Employer Identification Number for entities). The form also requires vendors to confirm registration with the System of Awards Management (SAM) and obtain a Unique Entity Identifier if receiving federal grant funds as of April 4, 2022.
The form includes certification statements requiring vendors to attest under penalties of perjury that the provided taxpayer identification number is correct, that they are not subject to IRS backup withholding, and that they are U.S. citizens or U.S. persons. Massachusetts state employees must additionally certify compliance with Massachusetts State Ethics Commission requirements. The form must be signed and dated by an authorized representative of the vendor. Completion and submission of this form is a mandatory prerequisite for contract award and payment processing with Commonwealth of Massachusetts agencies; failure to provide accurate information may result in backup withholding or contract suspension.
