About this file

The document is an Emergency Flex Funds Authorization form from the Arizona Department of Child Safety (DCS) for the Family Connections (FC) program. It outlines the guidelines and procedures for accessing emergency financial assistance for families involved in or at risk of child welfare intervention.

The Emergency Flex Funds are designed to provide up to $300 per referred family within a six-month service period to address critical concrete needs not available through other community resources. Eligible services include childcare, utility and rent assistance, clothing vouchers, food boxes, household supplies, cell phone minutes, homemaking, housing services, and cleaning or repair services when a household is unsafe. Reimbursement requires detailed documentation, including itemized receipts, a justification for each expense, and required signatures from the FC Consultant and Supervisor. If DCS is involved, their approval must be attached, typically via email.

Text version

A service component of the Family Connections program is the availability of Emergency Flex Funds. These funds are outlined in the Scope of Work, Section 2.15:

FC Emergency Flex Funds may be used to purchase needed items or resources that will address concrete needs of a family that relate to meeting the child’s basic needs, child safety and/or Conditions for Return, when these resources are not available through other sources, including community resources, or in-kind donations from community collaborators. Use of FC Emergency Flex Funds will not exceed $300 per referred family in a six (6) months service period.

Concrete services include, but are not limited to, childcare, utility assistance, rent assistance, clothing/uniform vouchers, food boxes and nutrition, household furniture and supplies, cell phone minutes, homemaking/housekeeping, housing services, cleaning and repair service when the household is unsafe or unsanitary. For more examples of concrete services, please reference Family Connections Program Manual;

Chapter 8.

Remember:

Expenses, including copies of receipts, shall be submitted for reimbursement to DCS via the monthly billing. In some instances, an approved item may not be approved due to no receipt being available.

The FC provider shall maintain in the family record original receipts for all concrete supportive services paid through the FC contract.

An assessment is necessary to identify concrete needs.

If DCS is involved, approval from DCS MUST be attached and can be in the form of an email. If DCS is not involved, the

Contractor will produce supporting documentation at the request of DCS.
