The amendment extends the contract term from July 1, 2024, through June 30, 2025, and includes several key changes: removal of NPP services, reduction of Family Connections services capacity in Maricopa County to 50 referrals at a time, and rate increases for various service levels. The revised rates include FC Level 1 (Urban) increasing from $5,787.50 to $6,150, FC Level 2 (Urban) rising from $7,106.25 to $7,550, and adjustments to No Contact and Unable to Engage rates. The rate increases will only apply to new referrals on or after July 1, 2024, and the amendment also updates terminology, grammatical elements, and specific scope of work details related to nursing assessments and team communications.
