Text version
1) Is 5yr testing truly desired on all traction elevators within the first year? This is an abnormal request which is well beyond normal code requirements.
Yes, paragraph 4.5 in the PWS, mandates this requirement. This is now an Air Force mandated requirement with the start of the new contract.
2) Three (3) year testing is not typical in the State of Ohio for traction or hydraulic elevators. Is the current contractor performing this service or do the bid specifications need modified?
The current contract does not require the 3 year testing. This is a new requirement mandated by the Air Force. While the State of Ohio does not require the three year testing, Wright Patterson AFB will be enforcing this requirement. This requirement is listed in paragraph 4.5 in the PWS.
3) Are all tests witnessed by a Qualified Elevator Inspector? Who covers the fees paid to this third party inspector? If our technician has QEI certifications, will this meet contractual requirements or must it be a third party?
A QEI is required to sign off on all inspections. The fees incurred from obtaining a QEI will be the responsibility of the contractor. If the contractor has a QEI on staff with certifications, the contractor is free to assign inspections to their QEI. The only contractual requirement is that a QEI performs inspections.
4) Is the preventive maintenance frequency at each contractor's discretion or monthly?
The preventive maintenance is performed at a minimum monthly occurrence. If the contractor feels it is necessary to implement more than a monthly preventative maintenance check, then that decision would be at the discretion of the contractor.
5) Can you provide pricing for maintenance and billing rates before the two most recent extensions as well as pricing from previous option year 5?
FA8601-09-D-0009: $2,164,452.20
FA8601-14-P-0206: $42,700.00
FA8601-14-P-0282: $168,410.00 (as of 14 Oct 14. The POP for this contract ends 31 Oct 14)
Detailed CLIN/unit pricing must be requested via an official Freedom of Information Act request.
6) (Attachment 3) Additional Instructions to Offerors : Page 3, (e) Binding and Labeling: Will a hard copy of the Offer Format and Organization Table need to be sent if we are submitting electronically, if so how many copies do you require.
Yes. Two copies (one original, one copy). See question #12 for further guidance.
7) (Attachment 4) Evaluation Factors: Page 4, Total Evaluated Price (TEP) (ii): Will we be required to add the six month extension option line on the table and leave blank for the notional proposed price.
You may fill in the price for the 6 month extension, with the knowledge that we will double check it – the instructions for this table state that “the Government will take the proposed price of the final option period (CLINS 0001 through 4004), prorate it to a six-month value, and add that amount to the sum of all CLINS.” If the price that you put into this line is greater than the prorated 6 month value, we will correct it.
8) (SF) 1449 Offeror Representations and Certifications: Page 17, The first paragraph states that only paragraph (b) of the provision should be completed if the offeror has completed (SAM), can you tell me where (b) is located.
Para (b) is located at the bottom of page 19 of the SF 1449. It appears as though there is a spacing typo because it is right up against para (2) for the definition of a Women-Owned Business Concern above it. It appears at the bottom of the page as “(b)(1) Annual Representations and Certifications….” Then there is a space and then “(2) The offeror has completed…..”.
9) (Attachment 3) Additional Instructions to Offerors: Page 4, would we just be submitting with our bid items "ii" and "iv" If you do not use subcontractors (v) then yes, the only information you would submit with your proposal is (ii) and (iv). Past Performance Questionnaires (vi) are submitted directly to me.
10) (Attachment 3) Additional Instructions to Offerors: Page 6, item (vi) states that we need to complete and return provision 52.209-7, information Regarding Responsibility Matter. Is this a form somewhere, as I can't find it in the documents provided?
This can be found on page 16 of the SF 1449 (Listed as FA8601-14-R-0020_WPAFB_Basewide_Elevator_Maintenance.pdf in the solicitation posting on FBO).
11) (Attachment 3) Additional Instructions to Offerors: Page 2, paragraph 3(b) says that electronic copies should be provided of each of the three Volumes, and Volume II, pricing must be submitted in PDF and Excel format. Do you have an Excel spreadsheet you would like for us to use?
Yes. It will be uploaded to FBO.
12) (Attachment 3) Additional Instructions to Offerors: Page 3, para e, says that the offer format should be submitted as "original" in binding and labeled in a 3-ring binder. Can you please clarify how many 'bounded and labeled" binders you would like to receive? Please clarify that these are in addition to the electronic copy that is required.
TWO copies of each binder (or volume) must be sent: One stamped ORIGINAL and one stamped COPY.
This is IN ADDITION TO your electronic submittal.
So just to clarify – you MUST submit an electronic copy of your entire proposal (all three volumes) as well as mail in or hand deliver TWO hard copies (one original and one copy) bound and labeled as directed in paragraph e on page 3 of Attachment 3: Additional Instructions to Offerors.
