1. DESCRIPTION OF SERVICES. The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform integrated solid waste management at Wright Patterson Air Force Base. The contractor shall perform to the standards in the contract as well as all local, state, and federal regulations, international agreements, Status of Forces Agreements (SOFA) and appropriate Final Governing

Standards (FGS) or, in their absence, the Overseas Environmental Baseline Guidance Document

(OEBGD). Collection areas are listed in Appendix A.

1.1. COLLECTION REQUIREMENTS.

1.1.1 Main Base

1.1.1.1. Integrated solid waste management. Empty Municipal Solid Waste (MSW) containers/dumpsters based on historical data for the first 30 days of performance. The contractor shall establish initial vehicle routes and collection schedules upon contract award. They are to be submitted to the Contracting Officer (CO) five (5) calendar days prior to start of contract performance. The contractor may propose adjustments to the schedule at any time but as a minimum the Contracting Officer’s designated Representative (COR) and contractor shall monitor fill capacities during the first 30 days of performance and during one other period to be agreed upon during the same performance period. No changes are allowed to the schedule or haul route without CO or COR coordination. Historical frequency tables are located at Appendix

A, Table A-2. When the contractor completes service at a location there shall be no litter, waste, and recyclable material on the ground within a 10 foot radius of the container.

1.1.1.2. Food Waste Containers. Empty containers used to dispose of food waste. Food Waste containers and pickup days are identified in Appendix A.

1.1.2. RESERVED

1.1.3. Over and Above Requirements. The following services shall be priced separately in accordance with the cost contained in blocks 19 through 24 on the SF 1449. The contractor shall be compensated IAW the price specified on the SF 1449 for that service. Services designated

"IDIQ" shall be awarded as Indefinite Delivery / Indefinite Quantity items; the contractor shall be compensated IAW the unit price when these services are requested.

1.1.3.1. Unscheduled Pickup and Special Events (IDIQ). In addition to the scheduled solid waste collections, the CO or COR may request the contractor make unscheduled collections and disposals. For unscheduled events, the contractor shall respond within one (1) business day of notification from the CO or COR. For special events, the contractor shall also provide (or re-position as directed) solid waste containers to any building on WPAFB, as requested by CO or

COR. These containers shall be placed in locations for a length of time designated by the CO or

COR. The CO shall notify the contractor in writing at least seven (7) days in advance of the special event date.

1.1.3.2. Construction and Demolition Debris (C&D) (Indefinite Delivery/Indefinite

Quantity). When requested by the CO or COR, the contractor shall use government furnished roll-off containers for collection of C&D. The contractor shall place containers at the designated site within one business day of notification by the CO or COR. The contractor shall recycle or dispose of all C&D at a permitted landfill or recycling facility. The contractor shall provide weight tickets of all roll-off containers to the COR. Contractor shall be on an “on call” basis within one (1) business day of notification by the CO or COR. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.

1.1.3.3. Public/Common Use MSW Receptacles. Contractor shall empty, collect, and dispose of all waste and replace liners in government furnished containers located at the locations designated in Appendix A before their capacity is exceeded. Estimated tasks & frequencies are included in Appendix A.

1.1.3.4. RESERVED

1.1.3.5. Ash Collection. The contractor shall accomplish ash collection by using trucks that are leak-proof and covered to prevent windblown ash. Trucks must meet state regulations and federal guidelines for transfer of municipal solid waste ash, as applicable. Ash shall be disposed of in government approved Ash containers. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.

1.1.3.6. Additional Pickups (IDIQ): In the event unusual conditions cause a container to require pickup before the scheduled time, the CO or COR may request in writing (email/ Fax sufficient) pickup within 24 hours and the contractor will be compensated at a unit rate based on the container size.

1.1.3.7 On-Call Collection (Compactors). Compactors listed as on-call in Appendix A, Table A-2 shall be picked up within twenty-four (24) hours of notification by the COR. Each compactor shall be cleaned inside and outside at an off-base location and return compactors within four(4) hours of the container being picked up.

1.1.3.8 On-Call Collection (Asbestos). Asbestos shall be disposed of in accordance with local, state and federal regulations in force at the time of disposal. The government shall provide one

30 yd covered container for collection of asbestos waste material. This container shall be identified on all sides as containing asbestos material. The location of this container shall be as specified by the CO/COR. Relocation of this container shall be done as specified by the

CO/COR at no additional cost to the government. Verification that the asbestos waste is double bagged and properly tagged is the responsibility of WPAFB Civil Engineering in accordance with EPA 40 CFR-61M, 5 Apr 1984 and Ohio EPA OAC 3745-20. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.

1.1.3.9 Surge Requirement Capabilities. Refuse collection services to support national commitments up to and including a declared war that would generate surge requirements shall be provided upon notification by the CO/COR. These services shall be provided 24 hours a day, 7 days a week as required for the duration of the emergency. Surge requirements beyond the capability of the contractor may be augmented by the government when mission accomplishment is endangered. The CO/COR shall advise the contractor of the required level of effort verbally and follow-up in writing.

1.1.3.10 Composting. The contractor shall provide the following services for yard waste:

1.1.3.10.1 Off-Base Disposal of Composting Materials Government furnished collection containers shall be emptied of compost materials. The compost shall be disposed of at an

Environmental Protection Agency (EPA) authorized site. Compost collection shall be on an on-call basis, as requested by the COR. Contractor shall respond within 24 hours of notification.

These compost collection stations are located at building 10879 Area A and 20745 Area B. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.

1.1.3.11 Saw Dust Collection. The contractor shall collect saw dust from designated locations.

These saw dust collection locations are identified in Appendix A. Table A-2. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations.

The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.

1.1.3.12 Fuzz Box Collection The contractor shall provide service to the “Fuzz Box” compactors located at buildings 10828 and 20306. Product in these containers is a finely shredded paper product that is no longer recyclable. These containers shall be taken to a separate dumping site from the landfill which possesses a license to dump such product.

1.1.4. Inclement Weather Schedule. The contractor shall collect MSW and recyclable material during periods of inclement weather. In cases of severe weather, the CO or COR may authorize exceptions. When exceptions are granted all missed collections shall be performed within 24 hours after the severe weather has terminated, unless the CO authorizes additional time.

1.1.5. Route Parameters. Routes shall be established so as to gain access to the base through gate(s) 12A, 15A, 16A, 1C, 38C located on State Route 444, 1B located on Springfield Street, 19B located on National Road and 22B off I-675. These base gates may close due to Force

Protection (Security levels) conditions at any time and adjustments shall be made. The collection schedule shall be established so collection operations begin no earlier than 0100 (1:00 a.m.) and are completed by 1700 (5:00 p.m.), Monday through Friday in each area. Collection outside these hours shall require prior coordination of the CO or COR.

1.1.6. Points of Collection. Collection stations for MSW materials are listed in Appendix A, Table A-2 The contractor shall position bulk containers for customer ease in depositing MSW.

The contractor shall also position containers to minimize interference with adjacent parking areas, sidewalks, roadways, overhead utilities, trees, and other potential obstructions.

1.1.7. Cubic Yard Capacity. Proposed capacity of containers is indicated in Appendix A, Table A-2. The contractor is encouraged to identify changes to the capacity of containers and/or pickup frequency to improve the overall efficiency of MSW removal, and to submit these changes to the CO or COR. No changes shall be made until the proposed change is coordinated with the CO or COR.

1.1.8. Government-Approved Containers. Collection of MSW and recyclable materials in all areas shall be from Government-provided, CO or COR accepted containers, recycling bins or plastic bags.

1.1.9. Maintaining Containers and Collection Area. The contractor shall return all MSW containers to their original location after servicing, in an upright position with the lids securely in place (closed). The contractor is responsible for cleaning up any spills, debris, etc., on the ground within ten (10) feet of the container when servicing a location. The contractor shall collect debris placed at collection stations; for example, tree branches, sacks, cartons, boxes, cans, tied bundles, or other containers. It is intended that the majority of debris will be placed in containers. The contractor shall notify the COR of locations where debris is frequently placed outside of containers so the COR may notify facility managers and, if necessary, the contractor can adjust the container size or pickup frequency with CO or COR approval. The contractor keeps collection areas free of solid waste and debris.

1.1.9.1. Apparent Serviceable Material. The contractor shall notify the COR within two (2) hours of any materials which have obvious value and appear to have been inadvertently placed near the collection point should not be disposed of, unless designated as refuse by the COR. If the contractor is in doubt as to the value of the material, the contractor shall check with the COR.

The contractor shall be liable for intentionally disposing of serviceable items or material.

1.1.10. Weighing of Vehicles. All vehicles used in the collection of MSW, C&D, organics and recyclable materials shall be weighed on state certified vehicle scales before departing the base.

Incoming and outgoing vehicle weights shall be recorded on weigh tickets provided by the operator at the weigh station. The contractor shall submit a monthly report (PWS, para 1.5

Reports and Records) that includes weight tickets that reflect the disposed tonnage figures, date of disposal, type of vehicle and cost of landfill tipping fees. The contractor shall be reimbursed for tipping fees according to submitted invoices. Base Weigh Station is located at Building

10293 in Area A.

1.1.11 Missed Pickups. The COR must be notified in writing and by phone if a schedule picked up was missed. Refuse at missed pickup points shall be collected at no additional cost to the government within two (2) working hours from notification by the CO/COR.

1.2. RECYCLABLE MATERIALS PROCESSING. The base operates a Qualified Recycling

Program (QRP) to reduce the volume of non-hazardous solid waste generated. It is the intent of the base to conduct direct sales of recyclable commodities through an internal QRP. For these services, the contractor will not sell any recyclable commodities. All commodities on this contract are picked up and delivered to Building 10293, recycling facility, for WPAFB to sell.

1.2.1. Materials. Recyclable items shall be collected and transported using 30 yard open top containers. All recyclable items will be transported to the Base Recycling Center, Building 293 located in Area A. The following items shall be recycled: metal, glass, cardboard, and wood.

Contractor may collect and transport materials not listed above with prior coordination of the CO or COR. All recyclable items shall be stored, shipped and disposed of in accordance with federal and local environmental regulations.

1.2.2. RESERVED

1.2.3. Base Recycling Center. The government will provide the contractor use of the base recycling center as shown in Appendix B to be used as the drop off point for all recycling on base.

1.3. DISPOSAL.

1.3.1. Off-Base Disposal. The contractor shall transport and dispose of all MSW at a permitted off-base disposal site. A copy of the landfill(s) operating permit that material is transported to shall be provided to the CO. The contractor shall transport recyclable commodities to an on-base material recovery facility. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.

1.3.1.1. On-Call Collection (Ash Collection). Government furnished collection containers shall be emptied of ash. The ashes shall be disposed of at an Environmental Protection Agency

(EPA) authorized site. Ash collection shall be on an on-call basis, as requested by the COR.

Contractor shall respond within 24 hours of notification. These ash collection stations are listed in Appendix A, Table A-2.

1.3.1.2 On-Call Collection (Asbestos). Asbestos shall be transported in a government supplied covered container. Asbestos shall be buried in an Ohio EPA landfill. A certified copy of the landfill permit for asbestos disposal shall be submitted prior to award of the contract. Asbestos removal shall be coordinated through the office of Environmental Management WPAFB 257-

5531. Asbestos container shall be picked up, emptied, and returned within 24 hours of notification by the COR.

1.4. EQUIPMENT MAINTENANCE. The contractor shall maintain all contractor and government provided containers ensuring they are free of unpleasant odors, dirt, debris, and pests. All containers must remain in good, workable condition, with no leaks. Containers shall be easily accessible to customers. The contractor shall perform all cleaning, painting, repair and other maintenance tasks off-base. Trucks and trailers used for hauling and collecting shall be free of odor, dirt, debris, and pests. Contractor vehicles and equipment shall not have any waste or equipment leaks such as oil or hydraulic fluid. Should any contractor vehicles or equipment have any leaks, the contractor shall cleanup. Any repair maintenance that is performed on government containers will require a submission of an estimate to CO/COR before the work is slated to begin.

1.4.1 Maintenance and Repair of Government Furnished Containers. An original inventory will be done by the government and given to the contractor upon award of contract. By the end of the first month, a joint inventory between the COR and contractor will be completed. The contractor shall provide an updated inventory report, which will list all broken or damaged government containers to the COR. The COR shall then designate the containers as (1) condemned, (2) serviceable, or (3) repairable. All broken or damaged government containers identified shall be repaired by contractor at the government's expense. The contractor shall provide documentation showing that the repair costs are fair and reasonable.

1.4.2 Condemned Containers. Condemned containers shall be removed from service and replaced with serviceable government-furnished containers within 24 hours of notification by the

COR. The replacement container shall be placed at the collection station at the same time the condemned container is removed. Containers designated as condemned shall be emptied and transported to the recyclable center at Building 10293 in Area A. The contractor shall purchase containers at the lowest price obtainable, reasonable competition and take all discounts and rebates possible, which shall be credited to the government. Documentation showing competition, catalog price lists, and/or other means of verifying price reasonableness must be provided to the Government. Containers purchased from the contractor shall be as specified by the government at time of purchase. All containers shall become the property of the government.

The government reserves the rights to obtain replacement containers through other sources as determined by the CO/COR. Contractor shall be reimbursed for purchase of these containers.

1.4.3 Repairable Containers. Repairable container shall be removed from service and replaced with a serviceable government-furnished container within 24 hours of notification by the COR.

The substitute container shall be placed at the collection station at the same time a container is removed.

1.4.3.1 The contractor shall repair containers at the expense of the government, and return containers to original location. Repair shall be completed within seven (7) working days from the time of notification by the COR. Verbal estimate for the repairs shall be provided within 24 hours of a notification by the COR. A follow-up written estimate of repair shall be submitted to the COR within 72 hours of the original request.

1.4.3.2 All damaged repairable containers not noted on the original or joint inventory and determined by the COR to be caused by contractor abuse, negligence, or misuse, shall be repaired at the expense of the contractor.

1.5. REPORTS AND RECORDS. The contractor shall provide a single monthly report detailing total tonnage of solid waste (MSW, C&D, composting and recycling) collected, disposal for landfill, tipping fees and weight tickets. Format for these monthly reports shall be coordinated with the CO or COR. Reports to be due by the 12 th of the month to the COR.

2. SERVICE SUMMARY. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

2.1. Performance Assessment.

2.1.1 Periodic Validation. This method requires the COR to employ a “spot check” style of evaluation based on the contractor’s schedule. Periodic validation will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends.

Any unsatisfactory inspection (defect) result shall be recorded, and the Contractor shall re-perform the service after notification by the CO or COR.

Exceeding the performance threshold for any performance objectives may result in a warning or letter of concern from the CO.

Exceeding the performance threshold of these performance objectives for any two or more consecutive or non-consecutive months during a contract period may constitute an immediate

Progress Meeting with the Multi-functional Team. All remedies shall be in accordance with the

FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.

2.1.2. Customer Complaints. The COR will receive and investigate complaints. The COR shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement with the COR and the Contractor. Validated customer complaints shall be re-performed after notification by the CO or COR.

Exceeding the performance threshold for performance objective in any one (1) month period may result in a warning or letter of concern from the CO.

Exceeding the performance threshold for performance objective for any two or more consecutive or non-consecutive months during a contract period may constitute an immediate Progress

Meeting with the Multi-functional Team. All remedies shall be in accordance with the FAR

52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.

2.2. Periodic Progress Meetings. The CO, COR, other government personnel as appropriate, and the Contractor shall periodically meet to discuss the Contractor’s performance. The following issues should be discussed; opportunities to improve the contract, any modifications required of the contract, unsatisfactory inspections and valid customer complaints against each performance objective observed and steps taken by the Contractor to prevent occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends.

Contracting will put the minutes in writing, signed by the CO and any other signatures as deemed appropriate, distributed to the functional area and the contractor and a copy placed in the official contract file. Should the contractor not concur with the minutes, the contractor will provide a written notification to the Contracting Office identifying areas of non-concurrence for resolution.

3.0 Government Furnished Property and Services

3.1 Government Furnished Containers

3.1.1. Government-Approved Containers. Collection of refuse shall be from government-provided, government-approved containers. The government shall provide without cost, containers, equipment, materials and services listed herein or in Appendix C.

3.1.2. Emergency Medical Service. The government shall provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill during the performance of this contract. The contractor shall reimburse the government for the cost of medical treatment and patient transportation service at the current inpatient or outpatient treatment rate, as appropriate.

3.1.3. Security Police and Fire Protection. The government shall provide general on-base security police service, fire prevention and protection, inspection and maintenance of government-furnished fire extinguishers and systems, pest control, and grounds maintenance.

While on WPAFB for Emergencies, call 257-9111 for Fire Department and Security Police support. For WPAFB Fire Department routine support call 257-3033. The Security Police routine call number is 257-6516.

4. GENERAL INFORMATION.

4.1 QUALITY CONTROL. The contractor shall develop and maintain a quality program to ensure integrated solid waste management are performed in accordance with commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. At any time during contract performance, the government has the right to require revisions of the Quality Control Plan at no cost to the government should the incorporated plan fail to control the quality of service provided. The plan shall include, but is not limited to, the following:

a. A description of the inspection system, addressing all services listed in Service

Summary (SS)

b. Frequency of inspections

c. Title of the individual(s) who shall perform the inspections and their organizational placement.

d. A description of methods for identifying, correcting and preventing defects in the quality of service performed before the level becomes unacceptable.

e. On-site records of all inspections conducted by contractor. The inspection records format shall include:

1. Date, time and location of inspection.

2. Signature block for the person who performed the inspection.

3. Rating of acceptable or unacceptable.

4. Area designated for deficiencies noted and corrective action taken.

5. Total number of inspections

4.2. QUALITY ASSURANCE. The government shall inspect and evaluate the contractor’s performance to ensure services are received in accordance with requirements set forth in this contract. The CO or COR shall inspect by validating actual work performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this

PWS. The CO or COR will use the contractor’s work schedule or modified version thereof, to record validation results. Results of the validation then become the official Air Force record of the Contractor’s performance. When a performance threshold has not been met or contractor performance has not been accomplished, the COR will initiate and provide the Contracting

Officer a Corrective Action Report (CAR) for issuance to the contractor. The contractor shall respond to the CAR IAW instructions provided and return it to the Contracting Officer within ten

(10) calendar days of receipt.

4.3. GOVERNMENT REMEDIES. The contracting officer shall follow FAR 52.212-4, Contract Terms and Conditions-Commercial Items, for contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

4.4. HOURS OF OPERATION.

4.4.1 Normal Hours: The hours of operation are Monday through Friday, 0100 to 1700, except

Federal Holidays. The contractor may work, with prior approval of the CO/COR, extended hours to ensure timely completion of work at no additional cost to the government.

4.4.2 The contractor shall provide service for containers that are regularly scheduled for pickup on a holiday. Pickups shall be made the next day following the holiday. The contractor is not required to provide service on the following days:

Memorial Day Thanksgiving Day

Independence Day Christmas Day

4.5. SECURITY REQUIREMENTS.

4.5.1 Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle

Passes. The contractor shall comply with all requirements and procedures IAW FAR 5352.242-

9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference.

4.5.2 The Contractor shall complete a Request for Identification Credential for each employee of the contractor requiring access to Wright-Patterson Air Force Base. The request shall be submitted to Pass and Registration (Bldg. 10286). The government shall provide a completed

Identification Credential (AFMC Form 387), which shall be issued, displayed, and surrendered as directed in AFI 31-101. Also DD form 254 (Contract Security Classification Specification) shall be filled out by vendor (attachment to solicitation) and returned to the CO/COR for access to controlled areas at Wright-Patterson Air Force Base.

4.5.3 The Contractor must obtain favorable National Agency Check (NAC) investigations for its employees who require entrance to restricted areas. The Wright-Patterson Air Force Base

Security Police shall be the point of contact for the NAC. The NAC shall be obtained through the Defense Investigative Center, P.O. Box 1083, Baltimore, MD 21203-1083, and shall provide inquiries on employee’s background worldwide. (Note: Completion of a NAC requires approximately four to nine months. A follow-up on the NAC shall be performed every 60 days until completed. Proof of follow-up shall be forwarded to the CO/COR).

4.5.4 All contractor employees shall comply with all security requirements imposed by the local commander at all times while his employees are on the installation and shall follow instructions of the local organizational commander pertaining to security.

4.5.5 Contractor shall provide adequate training and instruction for personnel to perform designated tasks. Flight-line driver training shall be required for personnel who must drive on flight-line. Training is held by Base Operations (937-257-6206) on the 2nd floor of 30206 (Area

A) the 1 st and 3 rd Wednesday of each Month.

4.5.6 The contractor shall safeguard all government property provided for contractor use. At the end of each work period, government facilities, property, equipment and materials shall be secure. Any damage caused by contractor personnel to the building, finishes, furnishings, equipment, etc., the contractor shall repair, clean, replace, or restore damaged items to the condition prior to the item being damaged. The contractor shall conform to the provisions of AFI

31-101, Integrated Defense, 08 Oct 09, for safeguarding the government-furnished facilities and material contained therein.

4.6. Continuation of Essential Department of Defense (DoD) Contractor Services During

Crisis. According to Department of Defense Instruction (DoDI) 1100.22, 12 Apr 10, and Air

Force implementation thereof, it is determined that the contractor will be required to perform during crises situations. Service may be required to support an activation or exercise of contingency plans outside the normal duty hours. Services shall include moving of dumpsters from current locations, providing dumpsters for mission support, emptying of dumpsters.

The base could be closed due to security problems or other events which would necessitate the closing of the base. Should one of these situations occur, the Contractor should listen to or watch one of the television or radio stations listed below for notification of a possible base closure from the Government. The Contractor is responsible for notification of his/her employees.

4.7 CONTRACTOR EMPLOYEES. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the CO/COR as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. List of dispatch personal shall be made available in writing to the government at the start of contract. Communication between dispatch and the COR shall occur within no more than

5 minutes from original attempt of contact during normal business hours, Monday – Friday.

4.7.1 All contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company in large letters on the upper back or chest area and identification badges bearing, at a minimum, the name of the individual, company name, and the words “Base-wide

Refuse Collection & Disposal Service”. The Project Manager, alternate and supervisor(s) shall, at a minimum, wear identification badges bearing the individual's name, company's name and the words, “Base-wide Refuse Collection & Disposal Service”. The contractor shall be responsible for providing and controlling all badges and uniforms.

4.7.2 The contractor shall provide a person who shall be responsible for the performance of the work. The name of this person and alternates who shall act for the contractor shall be designated in writing to the Contracting Officer/COR within 10 days after date of award. Telephone numbers shall be provided where the Project Manager and alternate(s) may be contacted 24/7.

4.7.3 The contractor representative or alternate shall be available within 30 minutes during normal duty hours to meet on the installation with government personnel designated by the

Contracting Officer/COR to discuss problem areas. After normal duty hours, the contractor representative or designated alternate shall be available within 60 minutes.

4.8. MULTI-FUNCTIONAL TEAM. A multi-functional team may be established between the government and the contractor to ensure joint cooperation and a sound customer focus of all parties involved in the execution of this contract.

4.9. CONTRACTING OFFICER AND CONTRACTING OFFICER REPRESENTATIVE.

The Contracting Officer will appoint a primary and alternate representative for management of the day to day activities of the contract. The identity, title, and authority of this representative will be provided in writing to the contractor after contact award.

4.10. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO within 30 minutes of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five (5) calendar days of each occurrence.

4.10.1 VOLUNTARY PROTECTION PROGRAM (VPP)

4.10.1.1 APPLICABILITY: This section of the PWS applies to “Applicable Contractors” as defined below.

4.10.1.2 DEFINITIONS:

Applicable Contractor. A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant

(installation).

Days Away, Restricted, and or Transfer Case Incident Rate (DART). The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.

Total Case Incidence Rate (TCIR) - Total number of recordable injuries and illness cases per

100 full-time employees that a site has experienced in a given time frame.

Voluntary Protection Program - The VPP promotes effective worksite-based safety and health.

In the VPP, management, labor, and OSHA establish cooperative relationships at workplaces that have implemented a comprehensive safety and health management system. Approval into

VPP is OSHA’s official recognition of the outstanding efforts of employers and employees who have achieved exemplary occupational safety and health.

4.10.1.3 REQUIREMENTS:

Wright-Patterson AFB is in the process of pursuing VPP recognition or has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all “applicable contractors” operating on Air Force Installations. The contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The contractor shall follow the safety and health rules of the installation or VPP site. Detailed information on VPP is available on the OSHA website at http://www/osha.gov/dcsp/vpp/index.html.

It is the contractor’s sole responsibility for compliance with the Occupational Safety and Health

Act (OSHA) (Public Law 91-596). The contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer not later than 10 calendar days after contract award. The contractor’s plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:

o Demonstrate a management commitment to employee safety and health;

o Identify the application of the safety and health plan to subcontractors;

o Identify the roles and responsibilities of the following individuals with respect to safety and VPP: Management, Supervisors, Employees, and Safety Coordinator;

o Identify applicable safety rules and regulations;

o Include a worksite hazard analysis to include base-line hazard identification and required control measures;

o Include a job site analysis to include hazards of tasks required to control measures;

o Identify employee safety and health training requirements and the documentation process;

o Include a workplace inspection frequency, to include identifying the individual conducting the inspections;

o Identify individual(s) responsible for corrective action of hazards;

o Identify first aid/injury procedures;

o Identify procedures for accident investigation and reporting;

o Identify emergency response procedures; and o Identify the process for tracking controlled hazards in contractor work areas.

The contractor shall submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the contracting office for consolidation and submission as part of the installation’s annual VPP

Safety and Health Management report. TCIR and DART rates are due by the 15 th of January of each year.

The contractor’s Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.

The contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.
