INDUSTRIAL SAFETY AND HEALTH REQUIREMENTS

1. Voluntary Protection Program (VPP). The Air Force (AF) is an active participant in the Occupational Safety and Health (OSHA) Voluntary Protection Program (VPP) in accordance with OSHA’s Instruction, CSP 03-01-002, Voluntary Protection Programs (VPP) Policies and Procedures Manual. We recommend all contractors become familiar with VPP. Information on VPP can be accessed at: http://www.osha.gov/dcsp/vpp/index.html. The AF’s intent in this participation is to ensure the safety and health of all employees, including contractors, and communicate our safety and health expectations with contractors. The AF’s interest is to protect AF personnel working in or around contractor operations and to protect AF equipment and property. In the event an unsafe condition is discovered with AF provided facilities or equipment, the contractor shall immediately notify the Contracting Officer and Safety Office associated with that Delivery/Task Order (D/TO) for evaluation. This requirement applies to on and off base D/TOs.

2. Environmental, Safety and Occupational Health (ESOH).

Compliance with Occupational, Safety and Health Act (OSHA) (Public Law 91-596), OSHA Standards (29 CFR Parts 1910 through 1926) and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. AF Occupational Safety and Health Standards (AFOSH STD) are annotated because many of the AF Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of each D/TO and on hazards associated with prescribed tasks. Each D/TO shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as necessary. If the contractor employs bilingual speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages. The Prime contractor shall include a clause(s) in all subcontracts requiring the sub contractors to comply with the safety provisions of each D/TO. This requirement applies to on and off base D/TOs.

3. Contractor Safety Program. A Safety and Health Plan is required to be submitted by each contractor (prime or subcontractor bidding on the work) for individual D/TOs placed against the F2AST contracts. All Safety and Health Plans will be submitted prior to award and reviewed by the Safety Office associated with the D/TO to determine if the plan is acceptable to the government. The contractor shall ensure that each element identified in the D/TO Appendix C is addressed and clearly defines procedures, personnel qualifications and training, facilities to include the electrical wiring and fire protection systems (i.e., fire suppression, fire detection, fire extinguishers, fire walls, etc.), and equipment as well as the following:

· Statement of Safety and Health Policy

· Administrative responsibilities for implementing the safety and health plan; identification and accountability of personnel responsible for safety and accident prevention

· A means for coordinating and controlling work activities of contractor, subcontractors, and suppliers, and their responsibility to comply with the safety and health plan

· Plans for initial safety indoctrination and follow-up safety and health training

· Provisions for frequent safety inspections of work sites, material, equipment, and employees to ensure compliance with the Plan; means for recording inspections and deficiencies and the means, timetable, and individual responsible for their correction

· Responsibilities for investigating and reporting accidents, reporting occupational health exposure, maintaining accident and exposure data, reports, and logs

· Emergency response capability to minimize the consequences of accidents

· Contingency plans for severe weather such as windstorms, lightning, tornados, etc

· Plans for maintaining job site cleanup and safe access

· Public safety requirements such as signs and barricades

· Local requirements that must be addressed

· Prevention of alcohol and drug abuse on the job

· Details of hazard communication program

· Provisions for updating the plan

This requirement applies to on and off base D/TOs.

4. Contract Safety Manager. As part of the Contractor Safety Program, the contractor shall identify in their plan, by name and phone number, a person who will be the primary point of contact for safety and health issues for the on-site operation. This requirement applies to on and off base D/TOs.

5. AF Safety, Fire Protection, and Bioenvironmental Engineering officials may periodically enter a contractor’s workplace to verify working conditions, provided the Contracting Officer authorizes such action. If an improper procedure or unsafe condition exists which places AF personnel or equipment in jeopardy, the contractor will remove personnel and/or equipment from the unsafe condition and immediately notify the Contracting Officer. This requirement applies to on and off base D/TOs.

6. Department of Labor OSHA inspectors are authorized right of entry to inspect any place of employment operated by an AF contractor. They are, for the most part, “No Notice” inspections. NOTE: Notify the Installation Safety Office if an OSHA inspector visits your site unescorted by an AF Safety Technician. This requirement applies to on base D/TOs.

7. If an employee, visitor, or inspector of the base, observes a potential OSHA violation, he/she will report the violation to the Contracting Officer associated with the applicable D/TO, who will then notify the contractor for action/resolution. This requirement applies to on and off base D/TOs.

8. Mishap Notification: The contractor shall notify the ACO and safety office associated with the D/TO telephonically within one (1) hour after initial notification of all mishaps that result in damage to DOD property or injury to personnel. Contact numbers of the ACO and Safety Office associated with the D/TO will be listed for normal duty hours and after duty hours. After telephonic notification, the contractor must follow-up with a written preliminary report within three (3) calendar days and forward report by e-mail to ACO and Safety Office associated with the D/TO. The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap. Mishap notifications shall include as a minimum the following:

(a) Contract, Contract Number, D/TO Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of mishap/incident

(c) Brief Narrative of mishap/incident (Events leading to accident/incident)

(d) Name of employee injured

(e) Supervisor’s name and phone number of the injured employee

(f) Supervisor’s name and phone number of the shop where the property damage occurred

(g) Cause of mishap/incident, if known

(h) Estimated cost of mishap/incident (material and labor to repair/replace)

(i) Nomenclature of equipment and personnel involved in mishap/incident

(j) Corrective actions (taken or proposed)

(k) Other pertinent information.

If required by the ACO associated with the D/TO, the contractor shall immediately secure the mishap/incident scene, damaged property, and impound pertinent maintenance and training records, until released by the presiding authority over the investigation. In addition, the contractor and subcontractor shall cooperate fully and assist government personnel until the investigation is complete. This requirement applies to on and off base D/TOs.

9. Fire Reporting on an AF Installation. All fires on an AF Installation, including extinguished fires, regardless of size, must be reported immediately to the Fire Department located at the local installation. The caller should give his or her name and location of what is on fire. Also give any information that may be requested by the fire department dispatcher. Stay on the telephone until the dispatcher has obtained all necessary information.

10. Pedestrian Crosswalks. All contractors and government personnel on an AF Installation are required to use the crosswalk if available when crossing the road. Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians do not have the right of way unless they are already in the crosswalk. This requirement applies to on base D/TOs.

11. Smoking. Smoking is permitted in designated smoking areas only. These areas are marked as “Tobacco Use Areas” and are located outside. This requirement applies to on base D/TOs.

12. Flammable and Combustible Liquids.

a. All flammable liquids will be stored in suitable metal containers. This requirement applies to on and off base D/TOs.

b. Gasoline or any other low flash point flammable liquid will not be used for cleaning or degreasing purposes. This requirement applies to on and off base D/TOs.

c. Smoking or the use of spark or flame producing equipment in areas where flammable liquids are being used or stored is strictly prohibited. This requirement applies to on and off base D/TOs.

d. Spills of flammable materials will be immediately reported to the ACO and Civil Engineering Environmental Management Section associated with the D/TO. This requirement applies to on base D/TOs.

13. HAZARD COMMUNICATION. In any D/TO where hazardous materials are involved, the contractor must comply with 29CFR 1910.1200, Hazard Communication, which includes training their personnel. Contractors must provide Material Safety Data Sheets (MSDSs) for each chemical that will be used at the work site under each D/TO. MSDSs must be provided to the Bioenvironmental Engineering Group located at the base associated with the D/TO at least 5 working days prior to start date. MSDSs must be the most current available. This requirement applies to on base D/TOs.

14. CHEMICAL SPILLS. No hazardous materials, chemicals or petroleum products shall be spilled on the ground, into a waterway, any sanitary sewer, storm drain system, or solid waste container. Accidental spills shall be reported immediately to the fire department and ACO associated with the D/TO. Contractors are required to ensure cleanup of spills, which they create (including disposal costs). This applies to on base D/TOs.

15. This applies to on and off base D/TOs. The contractor/subcontractor must comply with the following referenced documents while performing work under each D/TO:
