PERFORMANCE WORK STATEMENT (PWS)

FOR

Repair Chiller B9301 at Tinker Air Force Base, OK

1. DESCRIPTION OF SERVICES

1.1 GENERAL

1.2 The work to be performed under the terms and conditions of this contract consists of furnishing all plant, labor, equipment, instruments, materials, transportation, and incidentals necessary for repairing one (1) chiller and all required field connections and other associated items, including start-up, located at Building 9301 Tinker Air Force Base (TAFB), Oklahoma.

The chiller is a 2,000 ton York Chiller. This involves removal of the heads and dry purge the chiller to remove the moisture that has been built up. Checks for leaks and recharge the chiller with refrigerant. The controls will also have to have a complete check out before start-up.

1.3 All work shall be completed 90 days from the award of the contract.

1.4 Contractor shall submit all reports in IAW paragraph 4.

2. BASIC SERVICES

2.1 The Contractor shall provide labor and materials necessary to perform repair of existing Chiller.

2.2 The Government has recovered the refrigerant and the chiller has been prepped for the contractor.

2.3 The Contractor shall purge chiller and condenser with dry nitrogen.

2.4 The Contractor shall put the system under a vacuum of minimum 600 microns. System must hold vacuum minimum of 1 hour formatted

2.5 Contractor shall repair any leaks in tubing. Note: When the machine was brought down about three years ago, there were no known leaks. It was brought down with faulty oil pump that has since been replaced. Since the machine has stood idle those three years it would be judicious to state there may be 2 – 5 expansion leaks or there may be none. Any major leaks shall be treated as out of scope.

2.6 Contractor shall report to Government for verification any leaks found in water tubes or compressor gaskets.

2.7 Contractor shall replace any gaskets that are disturbed or opened during maintenance.

2.8 Gaskets shall not be reused.

2.9 Contractor shall replace vane actuator and vane potentiometer on actuator and recalculate.

2.10 Contractor shall replace all new oil, refrigerant filters and refrigerant.

2.11 Contractor shall perform all yearly maintenance items as stated in OEM (Original Equipment Manufacturer).

2.12 The Contractor shall start up chiller and calibrate controls.

2.13 The Contractor will conduct an operational demonstration of all system components and/or functions to test and verify the system is fully operational, as well as, safe to operate.

2.14 The contractor shall provide the Government a report of the results of the system operational test. The report may be formatted at industry and/or contractor format.

3. SPECIAL REQUIREMENTS

3.1 Code Requirements: Contractor shall be required to follow current National Fire Protection Agency (NFPA) 70 National Electric Code (NEC), Unified Facilities Guide Specifications (UFGS), Unified Facilities Criteria (UFC), ANSI/ASHRAE/IES Standard 90.1, 2016 TAFB Standards and International Building Code (including IMC and IPC) requirements for the work to be performed.

3.2 Remaining portions of the building shall remain occupied throughout repairs. Contractor will be required to consult with the Government Project Manager (PM)/Construction Inspector and the Using Agency POC to develop a timeline that accommodates the continued performance of user’s mission function during the renovation.

3.3 At ten days after Substantial Completion Contractor shall be responsible to bring the chiller up to 80% of rated capacity for two days (48 hours) of continuing operation.

4. SERVICE SUMMARY (SS)

The Contractor shall provide verification of system operational test.

2.14 The documentation report shall be due NLT

five (5) business days after completion.

The Contractor shall have established a complete Quality Control Plan (QCP) to ensure the requirements of this contract are provided as specified.

8.1 The Contractor’s QCP shall comply

with the elements specified in this PWS. The report shall be due NLT ten (10) business days after contract award.

As part of the Safety Program the Contractor shall establish a Safety Plan and checklist IAW Appendix B of this PWS.

12.1 The Contractor shall submit the Safety Plan

with the contract proposal.

In the event of an accident/mishap, the Contractor shall comply with PWS Appendix B requirements.

12.2 The Contractor shall provide a written or

email copy of the mishap / incident notification.

5. GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES

5.1 The Government will not provide any GFP equipment, parts, or materials to the Contractor to include the overhead crane for use.

5.2 The Contractor shall be responsible for safeguarding all Government property provided for Contractor use.

5.3 Plan information such as floor plan layouts; basic dimensions; design requirements for structural, mechanical, and electrical data; and location of utilities provided by the Government for this installation are based on available as-built documentation, field observation, and equipment selected as design basis by the Government. This information shall be considered approximate and shall be field verified by the Contractor.

This information will be provided prior to the beginning of the work.

6. GENERAL INFORMATION

6.1 SPECIAL QUALIFICATIONS:

6.1.1 The Contractor shall ensure site supervisory personnel are responsive to emergencies, problems (personnel, workmanship, etc.), or to answer questions, between the hours of 0700 and 1600 CST. A primary on-site supervisor and an alternate shall be provided to the Contracting Officer (CO) and Government PM NLT 5 days after contract award to include cell phone numbers. A 24-hour response is required on all other shifts, weekends, and holidays, unless otherwise specified.

6.1.2. The Contractor shall require all motorized vehicle operators to possess and maintain current certifications and state driver’s licenses. Vehicle operators shall comply with posted base speed limits, the base policy of no texting while driving, and mandatory seat belt usage by all vehicle occupants.

7. SECURITY REQUIREMENTS:

7.1 The Contractor shall be responsible to gain access to TAFB through the CO.

8. QUALITY CONTROL

8.1 The Contractor shall have established a complete Quality Control Program (QCP) to ensure the requirements of this contract are provided as specified. The Contractor's QCP shall comply with the minimum quality management systems elements specified in the American National Standards Institute (ANSI) American Standards Organization (ASO), and International Standard Organization (ISO) standards. The QCP may be formatted at industry and/or contractor format.

8.1.1 The Contract Manager may be required to meet with the CO and the Government QAP during the Period of Performance (PoP) of this contract. Meetings will be as often and as necessary as determined by the CO. The written minutes of all performance evaluation meetings may be prepared by the Government and signed by the Contract Manager, CO, and QAP. Should the Contractor non-concur with the minutes; the Contractor shall so state any areas of non-concurrence in writing to the CO NLT ten (10) calendar days of receipt of the signed minutes. The minutes will be included in the contract file.

8.2 The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints.

8.3 Inspection and acceptance will be made by 72nd Air Base Wing (ABW) Civil Engineering

(CE) assigned personnel.

8.4 Unsatisfactory performance shall result in re-performance by the Contractor at no additional cost to the Government.

9. HOURS OF OPERATION

9.1 The Contractor shall have access to perform services during normal working hours up to Force Protection Condition (FPCON) Charlie (C). Normal working hours at TAFB are typically 7:00AM – 4:00 PM Central Daylight Time (CDT) Monday through Friday. The COR will contact Contractor, if work is required on the weekend or holidays. Outages may require non-standard working hours to prevent disruption to production.

9.2 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities on the following federal holidays: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. If the holiday falls on Saturday, it is observed on Friday.

9.3 All visits shall be scheduled by the CO. Schedules may have to be modified in the case of incidents or emergencies, to include natural hazards, furloughs, or similar circumstances.

10. GOVERNMENT OBSERVATION

10.1 Government representatives, other than CO and COR(s), may from time to time with CO coordination observe Contractor operations. However, these shall not interfere with Contractor performance nor are these representatives authorized to make any changes to the contract.

11. ENVIRONMENTAL

11.1 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection as it relates to the work required in this PWS. In the event environmental laws/regulations change during the term of this contract, the Contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the Contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

11.2 The Contractor shall not use any Class I Ozone Depleting Substances (ODS) as identified in AF policies in the performance of this contract, nor shall the Contractor deliver any Class I ODS in any item or as a part of any service. If the Contractor determines that Class I ODS’s will be required under this contract, advance approval from the CO must be obtained before use.

11.3 Notification of Environmental Spills. If the Contractor spills or releases any substance contained in 40 CFR 302 “Designation, Reportable Quantities, and Notification”, including hazardous materials, into the environment, the Contractor or its agent shall immediately call

911. If the call is made from an on base phone it will go directly to TAFB dispatch. If using a cell phone it will go off base, ask the 911 dispatcher to be transferred to TAFB. The liability for the spill or release of such substances rests solely with the Contractor and its agent.

11.4 Material Storage and Use. The Contractor shall follow manufacturer’s guidelines and industry standards for control of humidity, temperature, cleanliness, and materials handling to include the handling of hazardous materials.

12. SAFETY PROGRAM

12.1 The Contractor shall implement a safety program that ensures protection of Government personnel and property. As part of the Safety Program the Contractor shall establish a Safety Plan IAW Appendix B of this PWS. The Contractor shall include and submit the Safety Plan with the contract proposal. The Safety Plan may be formatted at industry and/or contractor format.

12.2 In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence. A written or email copy of the mishap/incident notification shall be sent NLT three calendar days to the Government Representative, who will forward it to the TAFB safety office. Format shall be IAW Section B.

12.3 Occupational Noise Exposure: It may be necessary to control the occupational noise exposure to personnel and to comply with Federal regulations (Title 29, Code of Federal Regulations, Parts 1910.95 and 1926.52). Sound levels at the worksite where personnel may be located shall not exceed 120 decibels (adjusted) (dBA) when measured on the A-scale of a standard sound level meter at slow response.

13. MANPOWER REPORTING

13.1 The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract.

13.1.2 Reporting inputs will be for the labor executed during the PoP for each Government Fiscal Year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported NLT 31 October of each calendar year.

13.2 Uses and Safeguarding of Information: Information is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.

APPENDIX B

UNITED STATES AIR FORCE TINKER

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

1. NOTE: The Contractor will implement a safety program plan that ensures protection of Government personnel and property. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

2. Competent personnel to provide surveillance of the Safety Plan.

B. Mishap Notification

(1) The Contractor shall notify AFSC/72ABW (405-734-3301) or the Tinker AFB Security Forces (405-734-3737) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA COR (Contracting Officer's Representative) NLT one hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the Government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OC- ALC/SE. For information not available at the time of initial notification, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) ) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

1. The Contractor shall cooperate with any and all Government mishap investigations.

Additionally if requested by Government personnel or designated Government representative (GR), i.e., the ACO, PCO, or DCMA COR, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

2. The Contractor shall provide copies of Contractor data related to mishaps, such as Contractor analyses, test reports, summaries of investigations, etc. as necessary to support the Government investigation.

3. The Contractor shall support and comply with the safety investigation and reporting requirements of AFI 91-204, Chapters 1 – 5.

SECTION II – SPECIFIC REQUIREMENTS

PEDESTRIAN CROSSWALKS: All Contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road.

Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.

MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4; DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07; and AFI 91-207 AFMC SUP1, The US Air Force Traffic Safety Program, 1 Oct

07. Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition: AFI 13-213, Airfield Management, 29 Jan 08, Paragraphs 1.3.6 and 4.4.2.1 applies to all Contractors, sub-Contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Tinker Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.

HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.

The authorized GR (OC-ALC Safety Office) shall approve the Contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification / Reporting Procedures.
