Combined/Synopsis Solicitation
IAW FAR 12.603
This is the first amendment to the combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotes (RFQ), in accordance with FAR Parts 12 and 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-06, effective 12 July 2021.

The contractor shall complete all of the required deliverables to provide a Biomedical Equipment Technician (BMET) for the 48th Medical Support Squadron at RAF Lakenheath, England, in accordance with the Performance Work Statement (PWS) dated 07 April 2021.

The purpose of this amendment to solicitation FA558721Q0044 is as follows:Block 14 (Description of Amendment/Modification):
1. Revise quote sheet to include update of the GBP to USD conversion rate from: 1.1998 to 0.8002.
2. Add a section F. to the Instructions., Conditions., and Notices to Offerors, that will provide information on offeror questions.
3. Extend the solicitation due date from 01 September 2021 at 11:00 EASTERN STANDARD TIME (EST) or 4:00 PM, BRITISH SUMMER TIME (BST) to 16 September 2021 at 11:00 AM EST, or 4:00 PM, (BST).

Addendum 52.212-1 - Instructions to Offerors - Commercial Items (Jul 2021)
A. To assure timely and equitable evaluation of the offers, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including:
1. Terms and conditions.
2. Representations and certifications.
3. Current UK business registrations, licenses, and/or insurance required to legally operate and employ personnel within the UK or show rationale for non-applicability.
Failure to meet a requirement may result in an offer being ineligible for award.
The Government's terms, conditions, and respective clauses contained within this solicitation are prescribed in accordance with United States Federal Acquisition Regulations (FAR), and are not subject to conditionally proposed revisions or changes requested by offerors. Offerors' specific terms and conditions that may be contingently proposed for subsequent inclusion into the contract, if awarded, will not be considered by the Government in its evaluation of proposals. Offerors that are not able to comply with the aforementioned conditions, will be determined as noncompliant with the solicitation requirements, and therefore may be removed from consideration for award.
B. The proposal shall consist of three (3) separate attachments:
Attachment I - Technical Capability Attachment II - Price Attachment III - Offeror Representations and Certifications

For Attachment I - Technical Capability, the attachment shall provide documentation that can demonstrate how an Offeror plans to fulfill the Government's two (2) technical capabilities that are identified in paragraph 1.3.7.1: Education and Training in the Performance Work Statement (PWS).
For Attachment II - Price, the attachment shall be completed using the Revised Quote Sheet attached to this RFQ (Attachment 2). In doing so, the offeror accedes to the contract terms and conditions as written in this solicitation, with attachments. The solicitation constitutes the model contract.
The resultant contract will be a firm-fixed-price award. Accordingly, proposed pricing by offerors shall be firm-fixed-price and not subject to revision and/or negotiation, if selected for contract award. Additionally, multiple awards will not be considered. So quotes shall include pricing for all CLINs. Ensure that unit prices extend no more than two (2) decimal places.
For Attachment III - Offeror Representations and Certifications, complete the necessary fill-ins and certifications in the provision FAR 52.212-3, Offeror Representations and Certifications - Commercial Items along with the quote. For other provisions and clauses in the solicitation, the offeror is required to submit the pages that require an offeror fill-in. This attachment will not be used to contain extra content that could not fit in Attachments I or II.

D. Submission Instructions:
1. Electronic copies of all proposal information for each attachment shall be submitted using Microsoft Office 2010 or later, and/or Portable Document Format (PDF) using Adobe Acrobat 9.0 or later. Offerors shall not submit any documents in PDF that cannot be electronically searched.
Offerors may use JPEG images for charts or screenshots. The title of the electronic documents shall include the RFQ number, attachment number and offeror's CAGE code (i.e. FA558721Q0044, Attachment I: Technical Capability, CAGE code). Each attachment shall be contained in its own single searchable file, with copy/paste capability, and stored in a folder that corresponds to the proposal attachment it represents. JPG images and company proprietary information, such as plans and manuals, are excluded from the searchable with copy/paste capability. Offerors shall insert the file name in the header of each document.
2. Electronic offers shall be submitted to ashley.joyner_mendes@us.af.mil and maria.calimlim@us.af.mil.
3. It is the offeror's responsibility to confirm receipt of the quote from the Government, prior to the submittal due date and time. If sending multiple emails, label each email as "1 of X," "2 of X," etc. so that receipt of all parts can be verified.
4. All offers shall be complete, clearly presented, and contain sufficient detail for effective evaluation, as detailed in 52.212-2 of this solicitation. Offers shall be neat, indexed and assembled in an orderly manner.
Late proposals will be processed in accordance with FAR 52.212-1(f) - Late submission, modifications, revisions, and withdrawals of offers.
THE SUBMITTAL DUE DATE FOR QUOTES IS THURSDAY, 16 SEPTEMBER 2021, BY 11:00 AM EST or 4:00 PM, BRITISH SUMMER TIME (BST).
E. Format for Attachments I, II and III shall be as follows:
- Paper Size: 8.5 inches by 11 inches, or European equivalent.
- Font: Times New Roman, Size 11 font, or equivalent. Copies and Page Limits:
Attachment I - Technical Capability - Cannot exceed 10 pages. Supporting documentation such as certifications, licenses, insurance, or registration does not count towards the page limit. The offeror shall provide responses to the Technical Capabilities for evaluation as listed in Attachment 3.
Attachment II - Price - Cannot exceed 5 pages. The offeror shall complete Attachment 2, Revised Quote Sheet which is counted towards the page limit.
If the page limit is exceeded, then the excess pages will be removed from the proposal and will not be evaluated. When Interchanges Notices (INs) are necessary, the Government will issue them by electronic means to prospective offerors. Page limitations may be placed on responses to INs and such limitations will be provided at the time the EN is issued. There is no page limit for Attachment III. Only one copy of Attachment I, II and III is required.
Cross-Referencing. Offerors shall not cross reference between Attachment I and II. Each attachment shall be written on a stand-alone basis, so that its contents may be evaluated without cross-referencing the other attachments. Information required for proposal evaluation, which is not found in its designated attachment, will be assumed to have been omitted from the proposal.
Offerors are required to meet all solicitation requirements. Failure to meet a requirement may result in an offer being ineligible for award.
F. Questions
Questions will no longer be accepted after 31 AUGUST 2021. ALL QUESTIONS AND ANSWERS WILL BE POSTED VIA AMENDMENT ON BETA.SAM.GOV. Terms of the solicitation and specifications remain unchanged unless the solicitation is amended in writing. If an amendment is issued, normal procedures relating to the acknowledgement and receipt of solicitation amendments shall apply.

Evaluation Factors for Award

Addendum 52.212-2 - Evaluation Commercial Items (Oct 2014)
The evaluation of offers will be conducted IAW FAR 13.106-2.
Offers that meet the technical capabilities stated below will be compared with other offers. The quote that provides the best value for the Government, considering technical capability and price, will be selected for award.
Technical Capability: For Attachment I - Technical Capability, the attachment shall provide documentation that can demonstrate how an Offeror plans to fulfill the Government's two (2) technical capabilities that are identified in paragraph 1.3.7.1: Education and Training in the Performance Work Statement (PWS).
Revised Quote Sheet: There is a probability of adequate price competition for this acquisition. If at any time during this competition, the contracting officer determines that adequate price competition no longer exists, then offeror's may be required to submit information to the extent necessary for the contracting officer to determine the fairness and reasonableness of the price.
Offeror Representations and Certifications: complete the necessary fill-ins and certifications in the provision FAR 52.212-3, Offeror Representations and Certifications - Commercial Items along with the quote. For other provisions and clauses in the solicitation, the offeror is required to submit the pages that require an offeror fill-in. This attachment will not be used to contain extra content that could not fit in Attachments I or II.
The Government will evaluate the submitted prices using the Revised Quote Sheet provided (Attachment 2).
Evaluation of each option period will not obligate the Government to exercise that option. In the event there is a difference between a unit price and the extended total amount, the unit price will be held as the intended offer and the total of the CLINs will be recomputed accordingly. The offeror's total evaluated price shall be determined by adding the prices for every CLIN, as highlighted in the Revised Quote Sheet. Failure to price all CLINs on the Revised Quote Sheet provided will render the offer as non-responsive.
The Government may award without interchanges and reserves the right to hold interchanges, if deemed necessary by the contracting officer. Furthermore, the Government intends to award a contract to the responsible offeror whose offer, conforming to the solicitation, is most advantageous to the Government, technical and price factors considered.
