SABER Q&A 3

FA5004-16-R-C003 QUESTIONS AND ANSWERS 3

Q1. In reference to the concise summary of the entire proposal. Is there information that you are specifically looking to be included? Is there a page limit on this?

A1. Section L, Paragraph B.1.(a) is changed to: “The offeror shall provide a concise summary or master table of the contents of the entire proposal.” This is to allow the Government to quickly determine whether all required documents have been submitted and where they are located within the proposal. For a page limit, please reference Amendment 1 to this solicitation, A12. “Part I – No page limits apply, but do not submit any other documents or information in this Part other than what is required in accordance with Section L.”

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS

The following have been modified:

L-01

PROPOSAL PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein.

Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors.

Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts; Part I - Summary and Miscellaneous Data, Part II - Price Proposal, and Part III - Past Performance Proposal.

B. Specific Instructions:

1. PART I – SUMMARY AND MISCELLANEOUS DATA.

(a) The offeror shall provide a concise summary or master table of contents of the entire proposal.

(b) Complete blocks 14, 15, and 20a, 20b, and 20c of the RFP Section A, SF 1442. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. These sections constitute the model contract.

(c) Complete block 19 of the RFP Section A, SF 1442, if any solicitation amendment(s) to the original solicitation documents was/were issued. Amendments will not be counted against the page limits.

(d) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections C through I, the offeror shall submit only those pages that require a fill-in.

(e) Provide a copy of the letter from the Small Business Administration (SBA) showing proof of Section 8(a) status.

(f) If none proposed, submit an Affirmation of no Exceptions, Deviations, or Waivers.

(g) Provide a copy of the Financial Institution Reference Sheet (attachment JA-07), per instructions listed on the reference sheet.

(h) Do not submit any other documents or information in this Part.

2. PART II – PRICE PROPOSAL

(a) This part shall contain a completed Section B (Supplies or Services and Prices/Costs).

(b) Insert proposed coefficients in all yellow-filled cells of the Pricing Schedule (Microsoft Excel Worksheet, attachment JA-01) for each exhibit item (e.g. A001). Coefficients shall not be carried out more than two (2) decimal points. Coefficients are adjustment factors multiplied against the unit prices in the Unit Price Guide (UPG) to calculate order prices. Proposed coefficients shall be inclusive of all Division 00 Adjustment Factors of the eGordian® Construction Task Catalog® (CTC) including, but not limited to, overhead, profit, subcontractor overhead and profit, taxes, etc.

Offerors shall review and analyze the Division 00 Adjustment Factors of the eGordian® Construction Task Catalog® (CTC) to accurately establish coefficients for their offer.

(c) Extreme care should be exercised to ensure that no price data of any kind are included in any other Part of the proposal. Do not submit any other documents of information in this Part.

3. PART III – PAST PERFORMANCE PROPOSAL - Provide information for no more than three (3) of the most relevant contracts performed for any customer in the past three (3) years.

(a) Quality and Satisfaction Rating of Contracts - Limited to no more than three (3) pages per contract: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) that demonstrates customer satisfaction with overall job performance and/or quality of completed and/or ongoing contracts. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems, such as cost overruns, extended performance periods, numerous warranty calls, etc. (See Section M Basis for Contract Award, paragraph A.)

(b) Performance Surveys/Questionnaires - Limited to Attachment JA-06 only: Past Performance Questionnaires (PPQs) shall be used by the offeror in obtaining and providing past performance information. The government requires the offeror send out a PPQ to each of the Points-of-Contact (POCs) identified in the Past Performance Worksheet (paragraph (c) below).

The offeror shall send out the PPQ (Attachment JA-06), using the Past Performance Evaluation Cover Letter (Attachment JA-05), to each of its references.

THE RESPONSIBILITY TO SEND OUT THE PAST PERFORMANCE

QUESTIONNAIRE RESTS SOLELY WITH THE OFFEROR.

Once the questionnaires are completed by the POCs, the information contained therein will be considered source selection sensitive and will not be released outside the Government. The POCs shall forward their completed questionnaires directly to the government – NOT BACK TO THE OFFEROR.
