STATEMENT OF WORK
Flooring Replacement
Building 10306
CE Work Task # 11927396
SECTION 1: PROJECT INFORMATION
1. PROJECT LOCATION: Project shall be completed as a routine requirement at building 10306, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
2. UNIQUE REQUIREMENT: Also, 773 CES must perform a site visit upon completion of the project to ensure all work is done in an acceptable manor.
3. PROJECT DESCRIPTION: Remove floor tile and adhesive from room 6. Abate
PCB contaminated paint in bay area, utility room, and room 6. Remove concrete and metal trim from the channels in the bay area and room 6. Back fill all open channels with concrete. Fill in all voids, divots, and expansion joints in concrete. Polish concrete in bay area, utility room, and room 6 to a shine. Abate any other hazardous materials found at project location.
4. DURATION/PERIOD OF PERFORMANCE: All work to include site cleanup and restoration will be completed within 60 days. Working hours are Monday-Friday
0730-1630.
SECTION 2: GENERAL REQUIREMENTS
1. WORKMANSHIP: All work shall be accomplished in accordance with all applicable building and fire codes. Contractor shall field verify all measurements, quantities, or value stated by the Government. Liquidated damages shall be assessed for work that is not finished within the scheduled performance period.
2. WORKSITE: The contractor shall limit the work to the areas indicated by the government. The contractor shall make every effort not to disturb or obstruct portions of the facilities still occupied by the government. The contractor shall make efforts to minimize interference to Government operations. The contractor shall coordinate and schedule work with the facility manager prior the start of any operation. The contractor shall dispose of all construction debris off base at an approved disposal facility.
3. SITE SECURITY AND PROTECTION: The contractor shall make every effort to secure the work site, allowing only authorized personnel to enter the work site. The contractor shall protect the site from damage from weather, vandalism, and/or theft.
4. DAMAGE TO EXISTING SITE CONDITIONS: The contractor shall be responsible to repair any and all damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the government.
5. HEALTH/SAFETY: The contractor shall be responsible for compliance with the regulations of the United Sates Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA) and Air Force Occupational
Safety, Fire Protection and Health Program (AFOSH) Publication. The contractor shall maintain a clean and debris free site at all times. The contractor shall keep driveways and entrances into and around the work site clear at all times. The contractor shall not use driveways or entrances as employee parking or storage of materials unless otherwise stated. The contractor shall minimize storage of materials and equipment on site.
SECTION 3: DESCRIPTION OF WORK AND SPECIAL INSTRUCTIONS
1. General Requirements
1.1. Remove approximately 380' of floor tile and adhesive in room 6.
1.2. Abate approximately 3,200" of floor in room 6, bay area, and utility room for
PCB contaminated paint.
1.3. Remove concrete and metal trim from the channels running in the bay area and room 6 approximately 260' x 16".
1.4. Back fill channels with concrete and ensure finish concrete is level and flat with existing floor.
1.5. Fill all expansion joints, voids, or holes in concrete in room 6, bay area, and utility room flush with existing floor.
1.6. Polish concrete floor to a shine in room 6, bay area, and utility room.
1.7. Apply a seal coat to the concrete floor after polishing.
1.8. Work area highlighted on supplied map.
1.9. Contractor shall haul rubbish and debris from work area.
1.10. Dispose of all demolition debris shall be in accordance with state and federal guidelines.
1.11. Building may be occupied, coordination with building occupants before work begins to minimize impact on US Government mission.
1.12. Fill all expansion joints with RS 88 Polyurea or equal. Fill voids holes or other imperfections. Polish floor and seal with a material that will stand up to industrial vehicle traffic such as skid steers, motor vehicles, floor jack and other heavy equipment items. Floor must be resistant to fuel, oil, hydraulic fluid, and other petroleum materials.
1.13. New yellow warning paint required at the eye washstand and electrical panel.
