STATEMENT OF WORK

Mill & Overlay Gunfighter Ave

General

1. Overview

The project involves a mill & overlay, saw cutting, full depth patching, paint striping, and installation of thermoplastic markings on Gunfighter Avenue and Aardvark Avenue consisting of asphaltic concrete pavement. The overall project area is approximately 19,834 square yards (SY). There is approximately 19,834 SY of mill & overlay area at 1-inch depth, 3,193 SF of full depth patching at approximately 4 inch depth, 3,450 SF of paint striping, and placement of 38 thermoplastic markings. The project is located on Mountain Home AFB, ID. The original pavement construction date is estimated to be 2006 with several full depth patches made within the last year.

2. Requirements

a. The Contractor is required to adhere to all standards and specifications listed in the contract documents.

b. Coordinate all work in advance with the construction inspector, project manager, and contracting officer.

c. The Contractor must first accomplish full depth patches prior to performing a mill & overlay to achieve appropriate compaction.

d. The Contractor shall preserve and protect adjacent appurtenances when performing construction activities. Adjacent appurtenances include, but are not limited to: concrete curb and gutter, sidewalks, storm water inlets, rumble strips, vehicle barrier systems, valves, manholes, railroad tracks, landscaped areas, and adjoining asphalt pavements.

Adjacent appurtenances that are damaged by construction activities will be repaired or replaced at the Contractor’s expense.

e. All haul routes shall be swept continuously while hauling, paving, patching, and milling is taking place, as well as surfaces that are being traveled on and areas where construction is occurring.

f. Upon completion of construction activities, the Contractor shall return any staging or storage areas to existing conditions or better at no cost to the Government.

g. If a nuclear density gauge is to be used, the Contractor shall submit written notification to the Contracting Officer via NRC Form 241 (Attachment 6), a minimum of 30 days prior to requirement on the construction site. Notification must include, at a minimum:

i. Description of the device

ii. A description of the proposed activities on Nuclear Regulatory Commission

(NRC) Form 241, Report of Proposed Activities in Non-Agreement States.

Include proposed activities, locations of use, and traffic routes to be used.

iii. The procedures established to ensure the health and safety of personnel while the device is on MHAFB.

iv. A current and valid copy of the applicable NRC or Agreement State license. The license must state MHAFB by name, or a copy of the NRC Form 241 must be an

adjunct to the Agreement State license for those areas of exclusive federal jurisdiction. In lieu of a license, submit a written certification of exemption from NRC licensing requirements, and cite the applicable exemption of 10 CFR

150.10 or 150.11.

v. The most current leak detection test results. Provide new results to the Contracting Officer if updating of the leak detection test is required during the project.

vi. A copy of that part of the contract describing work to be done at MHAFB and the inclusive dates of the work.

vii. Agreement State licensees using NRC regulated materials must supply a copy of the NRC Form 241 approved by MHAFB’s NRC Region. (10 CFR 150.20).

A nuclear density gauge shall not be brought onto the installation without approval from the Government. The Contractor shall make all necessary notifications, as required by all federal, state, and local regulations. All Contractor personnel transporting and/or operating the nuclear density gauge must be trained, certified, and/or licensed throughout the period of performance of the project. Nuclear density gauge must be transported in a nuclear density gauge transportation case and clearly marked.

Project Scope

1. Perform a site visit to field verify and survey existing conditions, to include existing traffic striping and thermoplastic layout, identified patch repair areas, inductive loop traffic detectors, existing utilities, and all necessary areas to accomplish work.

2. Perform 38 large full depth patches as in Appendix A; total asphalt thickness is approximately 4 inches. Contractor will perform saw cutting, debris removal, recompact base course, apply prime/tack coat, patch with hot-mix asphalt, and compact. The Government will mark each patch boundary upon project award; the Contractor shall maintain markings as needed. Patches shall match existing grade. Patches adjacent to storm water inlets or concrete curb and gutters will be patched in a manner to ensure positive drainage and not inhibit removal of inlet covers.

3. Perform approximately 9,000 LF of crack sealing; cracks widths range from approximately ¼ - 1 inch wide and 1 inch deep. The Contractor shall ensure cracks are prepared in accordance with the contract specifications prior to applying sealant. Crack sealing will be performed after milling is complete, and prior to paving the new 1-inch wearing course. Crack sealing around storm inlets or manholes will be accomplished such that it will not inhibit the removal of any inlet or manhole covers.

4. Mill & Overlay approximately 19,834 SY of asphalt pavement at 1 inch depth via fine or micro milling. Millings will remain the property of the Government. Coordinate milling operations with the construction inspector and project manager at least 7 days prior to performing milling operations. The Contractor shall coordinate with the project manager before milling operations for the hauling and storing of asphalt millings; the Contractor shall perform all hauling, loading, and unloading of asphalt millings. Millings shall be stockpiled no more than 10 feet high at the location shown in Attachment 3. Pavement overlay mix design shall contain no Reclaimed Asphalt Pavement (RAP). The milled surface shall match the existing slope of the pavement surface (mill for depth, pave constant thickness). The overlayed pavement area shall match existing grade of adjacent appurtenances, to include adjacent asphalt pavements and concrete curb and gutter. Where existing asphalt patches have been made by the Government, the

Contractor shall ensure that when performing mill & overlay operations, the surface smoothness specification requirements will be met and positive drainage will be achieved.

Traffic signal lights are located at the intersection of Gunfighter Ave & Hope Drive, and Gunfighter Ave & Aardvark Ave. There are an estimated 11 existing inductive loop traffic detectors within the project area. In the event that the inductive loops are damaged during construction activities (i.e. milling), the Contractor shall repair the inductive loop via replacement. The Contractor will replace any inductive loop detector cable with those meeting specifications listed in Attachment 5, or approved equal.

5. Determine in-place density of 3 samples, each at 4 inch or 6 inch diameter at locations identified by the Government. Fill all core holes with hot-mix and compact using a standard Marshall hammer to a mat density as specified. Tack coat dry core holes before filling. Each random core will be full thickness of the layer being placed. When the random core is less than 1 inch thick, it will not be included in the analysis. In this case, another random core will be taken.

After air drying to meet the requirements for laboratory-prepared, thoroughly dry specimens, cores obtained from the mat and from the joints will be used for in-place density determination in accordance with ASTM D2726/D2726M. Provide test reports to the Government within 5 business days after completion of testing.

6. Perform paint striping and placement of thermoplastic markings. Paint striping consists of approximately 1,750 SF of white paint and 1,700 SF of yellow paint. Thermoplastic markings will only be used for stop bars, crosswalks, and directional arrows. All thermoplastics will be white in color. The estimated thermoplastic count is listed in Appendix C. All paint striping and thermoplastic markings shall match existing locations unless approved by the Contracting Officer.

7. Provide a bid option for a mill & overlay, full depth patching, and paint striping of the bid option area 1 identified; bid option area is located immediately outside of the Main Gate. The bid option area for mill & overlay measures approximately 2,550 SY at a 1 inch milling depth.

Total paint striping is approximately 500 SF of white paint. Full depth patches are listed in Appendix A. All paint striping shall match existing locations unless approved by the Contracting Officer. All requirements and specifications listed for the required project area shall be applied to the bid option area. Asphalt mix for the bid option area shall conform to specifications listed in Attachment 7; use of RAP is permissible within this area only.

8. Provide a bid option for a mill & overlay, full depth patching, full depth repair at the intersection of Falcon St and Gunfighter Ave, and paint striping of the bid option area 2 identified; bid option area is located from the intersection of Gunfighter Ave and Aardvark Ave to Gunfighter Ave and Falcon St. The bid option area for mill & overlay measures approximately 12,597 SY at a 1 inch milling depth. Full depth repair at Gunfighter Ave and Falcon St measures approximately 588 SY at an approximate 4 inch depth. Contractor will perform saw cutting, debris removal, recompact base course, apply prime/tack coat, pave with hot-mix asphalt, and compact. Full depth patches are listed in Appendix B. Total paint striping is approximately 300 SF of white paint and 2100 SF of yellow paint. Thermoplastic markings will only be used for stop bars, crosswalks, and directional arrows. All thermoplastics will be white in color. The estimated thermoplastic count is listed in Appendix C. Perform approximately 6,000 LF of crack sealing; cracks widths range from approximately ¼ - 1 inch wide and 1 inch deep. The Contractor shall ensure cracks are prepared in accordance with the contract specifications prior to applying sealant. Crack sealing will be performed after milling is complete, and prior to paving the new 1-inch wearing course. Miscellaneous Requirements

1. Submittals. In addition to the required submittals listed in the project specifications, the Contractor shall also submit the following for approval by the Contracting Officer prior to beginning any construction related activities:

a. Traffic Striping and Thermoplastic Marking plan

i. Striping plan must match existing layout

b. Asphalt Mix Design or currently used Idaho Transportation Department (ITD) Superpave hot mix

c. Traffic Control Plan. The Contractor shall coordinate with the Project Manager, as needed, when developing the traffic control plan to include any requested traffic signal changes by the Government. The Contractor will provide a traffic control and phasing plan such that all facilities along the route will have continuous uninterrupted access. All traffic control plans shall conform to the current MUTCD.

i. Traffic Control Plan – Asphalt Patching

ii. Traffic Control Plan – Cold Milling

iii. Traffic Control Plan – HMA Paving/Overlay

iv. Traffic Control Plan – Paint Striping & Thermoplastic Placement

d. Environmental Management Plan

2. The Contractor will schedule a pre-construction meeting within 5 days after issuance of Notice to Proceed (NTP) with the Contracting Officer at Mountain Home AFB. The Contractor will be responsible for meeting preparation, recording meeting minutes and providing a typed electronic copy to the Government within 3 business days. At a minimum, topics to be discussed will include construction schedules and key submittals (construction schedule and construction phases, traffic control plans, HMA mix design, milling and paving equipment, etc.)
