PERFORMANCE WORK STATEMENT (PWS)
HOIST INSPECTION, TESTING, AND CERTIFICATION
NELLIS AIR FORCE BASE, NEVADA
30 July 2015

1.0 DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, labor, supplies, equipment, material transportation supervision and all other necessary items to perform on site annual and monthly inspections, weight load testing, non-destructive inspection and certification of all hoists listed on the attached Bid Schedule.
1.1 DESCRIPTION OF WORK: The contractor shall comply with the Occupational Safety and Health Act 1970 29 CFR 1910.179; and the American National Standard Institute (1973) B30.2, B30.9, B30-10, B30.11, 830.16 and B30.17 during the performance of all work. The contractor shall inspect, weight test, perform non destructive inspection to meet the manufacturer's specifications and recommendations and shall certify the following items IAW Air Force Occupational Safety and Health Standard (AFOSH) 91-46 Materials Handling and Storage Equipment, and AFOSH 127-20 (Chapter 7) Lifting Devices, per attached Bid Schedule.
1.2 ANNUAL INSPECTIONS: May require the removal of covers and (or) parts to gain access. Complete disassembly of the hoist or major components, not designed to be removed or opened for the sole purpose of the inspection is not normally required. However, if the inspection authority finds evidence of damage or wear that constitutes a potential hazard;
further inspection to determine cause shall be required. The following items shall be inspected:
1.2.1 Verify all controls and operation mechanisms for proper operation.
1.2.2 Verify hoist operations under a no-load condition to test all functions of the hoist including hoisting and lowering, operation of brakes and testing of limit, locking, and safety devices.
1.2.3 Verify operation of upper limit switch. (The block shall be "inched" into the limit switch to prevent possible damage.)
1.2.4 Verify operation of deadman switch up and down.
1.2.5 Verify that travel limit switch works in all directions.
1.2.6 Verify that deadman travel brake operates properly.
1.2.7 Lower hook almost to floor, inspect cable/chain for obvious defects.
1.2.8 Verify that all labels on hoist and associated support structures are legible.
1.2.9 Verify the supporting structure of the hoist including trolleys, monorail, crane or beam have a load rating of at least equal to that of the hoist.
1.2.10 Inspect the supporting structure of the hoist including trolley, monorail, crane or beam for signs of weakness, wear, and (or) misalignment, runway splice plates, and structure connections.
1.2.11 Verify that the rated load is permanently marked on the hoist and load block, and is clearly legible from an operating position.
1.2.12 Inspect all load bearing components of the hoists for damage.
1.2.13 Inspect all safety devices for malfunction.
1.2.14 Inspect air systems for deterioration or leakage.
1.2.15 Inspect for loose belts, bolts, nuts, or rivets.
1.2.16 Inspect for cracked, worn, or damaged drums or sheaves .
1.2.17 Inspect for worn, cracked, or distorted parts such as hook blocks, suspension housing, outrigger, hand chain wheels, chain attachments, yokes, suspension bolts, shafts, gears, bearings, pins, rollers, locking and clamping devices, bumpers, switch baffles, interlock bolts, and trolley stops.
1.2.18 Inspect for excessive wear on motor and load brakes.
1.2.19 Inspect braking mechanisms for evidence of slippage under load.
1.2.20 Inspect for worn, glazed, or oil contaminated friction disc or worn pawls, cams, or ratchets on brake mechanisms, corroded, stretched or broken pawl springs.
1.2.21 Inspect the supporting structure and trolley for continued ability to support imposed loads.
1.2.22 Ensure information concerning operating procedures is posted by all hoists by using a label affixed to the hoist, controls, or block (see ANSI Standard 830-16).
1.3 Inspect Pendant Control System
1.3.1 Ensure the pendant station is supported by a cable, chain, or rope that will protect the electrical conductors against strain.
1.3.2 Ensure that the electrical and support cable is in good condition.
1.3.3 Ensure that all function labels on pendant station are present and legible.
1.3.4 Inspect electrical apparatus for evidence of pitting or any deterioration of controller contacts, limit switches and push button stations.
1.3.5 Inspect hoist electrical pendant and/or control box for proper grounding.
1.4 Inspect Rope Assemblies
1.4.1 Inspect wire ropes for reduction of rope diameter below nominal diameter due to loss of core support, internal or external corrosion, or wear of outside wires.
1.4.2 Inspect for broken outside wires and the degree of distribution or connection of such broken wires.
1.4.3 Inspect for worn outside wires.
1.4.4 Inspect sections of rope which are normally hidden or difficult to see (such as parts passing over equalizer sheaves).
1.4.5 Inspect for corroded or broken wires at end connections.
1.4.6 Inspect for corroded, cracked, bent, worn, or improperly applied end connections
1.4.7 Inspect load-carrying ropes for wear, twist distortion, broken wires, or improper dead ending to the hoisting drum and other attachments.
1.4.8 Inspect for severe kinking, crushing, cutting, or un-stranding, or any excessive wear to rope.
1.4.9 Inspect for birdcaging, or any other damage resulting in distortion of the rope structure.
1.4.10 Inspect for evidence of any heat damage from any cause.
1.5 Inspect Welded Link Chain
1.5.1 Inspect load-bearing chain for wear, twists , breaks, cracks, or other damage to links. Check chain for deposits of foreign material that may be carried into the hoist mechanism.
1.5.2 Test the hoist under load in hoisting and lowering directions and observe the operation of the chain and sprockets. Ensure the chain feeds smoothly into and away from the sprockets.
1.5.3 Inspect the chain for gouges, nicks, weld splatter, corrosion, and distorted links. Check links to inspect for wear at the contact points.
1.5.4 Inspect hoist chains including end connections for excessive wear, twist, and distorted links interfering with proper function.
1.5.5 Inspect for excessive wear of chain, rope, load sprockets, drums, sheaves, and chain stretch.
1.6 Inspect Hooks
1.6.1 Hooks having more than 15 percent in excess of normal throat opening or more than 10 degree twist from the plane of the unbent hook shall be failed.
1.6.2 Hooks shall be examined/inspected for the following:
1.6.3 Distortion such as bending, twisting or increased throat opening.
1.6.4 Wear.
1.6.5 Cracks, severe nicks, or gouges.
1.6.6 Proper safety latch engagement and damaged or malfunctioning latch.
1.6.7 Proper hook attachment and securing means.
1.6.8 Lubrication of swivel point.
1.6.9 Defects, chemical damage, cracks, and defective safety latches.
1.6.10 Inspect hook retraining nuts, collars, pins, welds, and riveting used to secure the retaining member.
1.6.11 Perform a Non-Destructive Inspection according to the attached Bid Schedule.
1.7 MONTHLY MONORAIL HOISTS INSPECTIONS. The track and its support shall be inspected monthly for signs of weakness, wear and (or) misalignment, runway splice plates, and structure connections. (See attached bid schedule for hoists requiring monthly inspection). The following items shall also be inspected:
1.7.1 Verify that all bolts are tight and secure and there are no missing bolts or anchors.
1.7.2 Inspect for rail wear, and wear checks or fractures in rails, and broken out pieces of rails.
1.7.3 Inspect for stress marks on rail clips, welded clips, and bolted clips.
1.7.4 Verify that splices are tight and secure and welds are holding and in good shape.
1.8 LOAD TESTS. The contractor shall perform annual load tests on all hoists listed on attached Bid Schedule.
1.8.1 The hoists shall be load tested at 100 percent of the rated load. Test weights utilized for load tests shall be tagged or adequately marked indicating total weight in pounds and owner (or) agency identification number. Reinforcing (rebar) steel or water filled containers shall not be used for test weight lift points.
1.9 PREVENTIVE MAINTENANCE. During the annual inspection the contractor shall perform preventive maintenance per manufacturer's recommendations as follows:
1.9.1 All moving parts of the hoist for which lubrication is specified by the manufacturer shall be lubricated during each annual inspection period. Hoist lubricating method shall be checked for proper delivery of lubricant. Machinery shall be stationary while lubricants are being applied. Safety precautions shall be taken at all times.
1.9.2 The overload protection part of the hoist shall be checked for oil discoloration, viscosity , and quantity. If discrepancies are found replacement of oil will be accomplished according to manufacturer's recommendations.

3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES . No Government furnished property or services/utilities isassociated with this contract.
4.0 GENERAL INFORMATION
4.1 CERTIFICATION
4.1.1 At the completion of service for every line item, the contractor shall provide a completed copy of the service ticket to the COR. The information on the service ticket shall include as a minimum: building number, hoist manufacturer, model number, serial number, type of inspection performed (monthly or annual), results of all items checked, exact wire rope or chain load test with measurements and exact hook opening measurements.
4.1.2 At the completion of inspection and load test, a service sticker/ticket shall be affixed to the electrical shut-off box providing the following information: company name, hoist manufacturer name, hoist serial number, certification date, expiration date of certification , contractor inspector/technician name, type of inspection performed (i.e.
monthly or annual).
4.2 CERTIFICATES. The contractor shall provide to the COR an original certificate within ten working days of completion of each inspection. The hoist certification shall include the following: The contract line item number, building number, using organization, certification date, hoist's manufacturer and serial number, inspector's signature, company name and address.
4.3 HOISTS FAILING CERTIFICATION. Should any hoists fail the certification, contractor shall provide in writing to the COR the reason the lift failed and an estimated cost of repairs. The contractor shall submit the cost estimate no later than ten working days after completion of the last inspection. The cost estimate shall include a line item cost breakdown of labor, required parts, and description of work to be accomplished on each hoist that failed. The contractor shall verbally inform the facility manager and the COR when a hoist fails inspection.
4.4 LABOR RATE FOR REPAIR CALLS. Labor rates per hour shall be set by the
Department of Labor to repair hoists for the total five year period of this contract and shall be incorporated in this order. Payment for the repair calls shall not be charged to this order. Only those hoists on the Bid Schedule shall be repaired using the Government Purchase Card (GPC). Payment will be made upon completion of work; all other hoist/equipment not on the Real Property records or the Bid Schedule shall be paid for by the using activity with their Government Purchase Card.
4.5.1 RESPONSE TIME: The contractor shall respond to unscheduled repair calls within three (3) hours of notification. The contractor shall not accomplish repairs until verbal approval is received from the COR.
4.6 WORK SCHEDULE.
4.6.1 The contractor shall begin annual requirements of this performance work statement as listed on attached bid schedule in January. Inspections shall be scheduled for the annual inspection to ensure current certifications do not expire and hoists are available for use. A proof load test in the range of not less than 100% to not more than 125% of the rated capacity shall be performed on all equipment listed on the Bid Schedule.
4.6.2 Monthly monorail inspections shall begin in October on those hoists indicated on the attached bid schedule as monthly's.
4.6.3 All work shall be accomplished between the hours of 7:00 AM. and 4:00 P.M., Monday through Friday, excluding federal holidays. For hoists located in AREA II, work shall be accomplished between the hours of 7:00 AM. and 3:00 P.M., Monday through Friday, excluding federal holidays.
4.6.4 The contractor shall provide a written work schedule to the COR for approval at a minimum of two weeks prior to the inspections. Any changes to this schedule shall be coordinated with the COR at least 48 hours in advance of work to be performed.
4.7 CONTRACTOR AND CONTRACTOR PERSONNEL QUALIFICATIONS. The contractor and contractor employee(s) actually performing the crane/hoist inspection , maintenance, load testing and /or repairs shall be a "Qualified Person:", or a journeyman mechanic working under the direct and physically present supervision of a "Qualified Person", as defined in AFOSHSTD 91-46, Attachment 1. As a minimum, and in the absence of a recognized degree in an applicable field, a "Qualified Person" shall have a current certificate of professional standing, such as an "Overhead Crane Inspector Certification" issued by a reputable and qualified inspector training agency, such as the Crane Institute of America , and an acceptable experience record of working on, repairing and maintaining cranes and hoists having characteristics equivalent to the range of cranes and hoists encountered at Nellis AFB for a period of not less than three (3) years . Contractor shall provide to the contracting officer a copy of the certification or license for each employee prior to performing the annual inspection prior to performing any work on this contract.
4.8 SECURITY REQUIREMENTS
4.8.1 Personnel. Some of the hoists may be located in secure areas. If required the contractor shall provide the following information when required on all contractor personnel working in these areas to the Contracting Officer, in writing, at least two (2) weeks before commencing any work on this contract:
4.8.2 Employee Full Name (no nicknames)
Social Security Number (SSAN) Date and Place of Birth Citizenship (if naturalized citizen, state date of naturalization and number.)
4.8.3 Identification: Contractor personnel performing work on this contract must have two forms of ID for access to secure areas; one listing shall be a state driver's license, with picture and the other shall be the Social Security Card.
4.8.4 Vehicles: All contractor vehicles shall be required to have company logo clearly displayed in the performance of this contract.
4.8.5 Equipment: Additional clearance information shall be required on all subcontractors vehicles; trailers, etc. used to supply or transport the weights or man lift used in the performance of the load test located in Area II. No cell phones or recording devices shall enter any facility during the inspection of the hoist.
4.8.6 Some delays may be encountered in gaining access and leaving secured areas even with prior coordination.
4.8.7 ID Passes, Badges and Vehicle Stickers: The contractor is responsible to obtain such clearances and/or passes as required for his/her employees to enter military installations on Nellis AFB prior to start of work. The contractor shall be responsible for all passes issued to his/her employees under this contract. Appliiations shall be coordinated through the Contracting Officer or designated representative. At the time of application, Security Forces may conduct a background check on employees. Upon termination of employment, the contractor shall be responsible for returning identification cards and base passes to Security Forces within 24 hours. Security and base access requirements are contained in Air Force FAR clause 5352.242-9000 "Contractor Access to Air Force Installations." Contractor employees will be required to obtain and display identification badges. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.
4.9 HOURS OF OPERATION. All work shall be accomplished between the hours of 7:00
A.M. and 4:00 P.M., Monday through Friday, excluding federal holidays. For hoists located in Area II, work shall be accomplished between the hours of 7:00 A.M. and 3:00 P.M., Monday through Friday, excluding federal holidays. The contractor may work, with prior approval of the
Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government.
4.9.1 Recognized Holidays. The contractor is not required to provide service on the following days: New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
4.10 ENVIRONMENTAL CONTROLS. Contractor shall comply with all Federal
Environmental requirements and with the Resource and Recovery Act (RCRA) standards for handling and disposing of hydraulic oil as required. Contractor is responsible for all waste products generated. The contactor shall not dispose of hazardous material or hazardous waste on Nellis AFB and shall comply with all Federal, Local, and State regulations/laws and procedures regarding disposal of waste generated.
4.11 GOVERNMENT OBSERVATIONS. Government personnel, other than contracting officers and COR’s, may from time to time, with CO coordination, observe contractor operations.
However, these personnel may not interfere with contractor performance or make any changes to the contract.
4.12 SAFETY REQUIREMENTS. In performing work under this contract, the contractor shall:
4.12.1 Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.
4.12.2 Perform work in a safe manner as required by OSHA 2206, General Industry, Occupational Safety and Health Standards (29 CFR 1910).
4.12.3 Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.
4.13 CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING CRISIS
IAW DoDI 3020.37. Additionally, IAW AFI 63-124, para 2.7.1, and DODI 3020.37 this requirement is not considered mission essential.
4.14 PUBLICATIONS
4.14.1 Compliance with all publications, regulations and operating instructions provided by the Government is required when:
4.14.2 They pertain to the procedures for materials expediting herein and where the contractor is authorized by the performance work statement to accomplish the work specified in the publication, regulation or operating instructions.
4.14.3 The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements.
4.14.4 The contractor is required to acquire and work from the latest version of the publication.
4.15 CONTRACTOR PERSONNEL
4.15.1 Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name in English.
4.15.2 The contractor shall not employ any person who is an employee of the U.S.
Government if employing that person would create a conflict of interest. Additionally. the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DOD 5500-7-R Joint Ethics Regulation. The contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106.
4.15.3 The contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor's ability to perform; however, their absence at any time shall not constitute an excuse for nonperformance under this contract.
