STATEMENT OF WORK
Project: CZQZ11-0099
RPR FIRE STATION GARAGE DOORS
27th Special Operations Civil Engineering Squadron 506 N. Air Commando Way
Cannon AFB, NM 88103
20MAY2016
1.0 GENERAL INFORMATION
1.1. PURPOSE: The purpose of this Statement of Work (SOW) is to notify approved Construction agents of a potential project at Cannon Air Force Base (CAFB) requiring construction. This SOW describes Air Force requirements and objectives for the documentation and construction of the project. The detailed requirements for this project can be found in the solicitation for this project and attached documents as listed below:

1.2. PROJECT LOCATION: CAFB is located in Curry County, approximately six miles west of Clovis, NM. The TA Ramp is located in the northwest area of the flight line.
1.3. PROJECT DURATION: This work is to be completed in 180 days or less.
1.4. DESCRIPTION OF THE WORK: The Work consists of complete construction services necessary to complete the contained Drawings and Specifications with the work summarized below:
Existing Site Description
The existing Fire Station, building 158, overhead doors and accessories are failing faster than can be fixed. Several doors are out of service hampering fire truck responses. Doors operate a minimum of 20 times a day during the cold months. The Work consists of complete construction services necessary to complete the contained Drawings and Specifications with the work summarized below are in need of repairs due to exceeding over 10000 open and closures for the
18 bays.
Site Construction
Replace one overhead insulated door to match existing overhead doors with light panes Replace all control boxes, motors, weather stripping, door numbering, and 480Volt/3 phase safety switches for 18 doors.
Install a minimum of new 15 HP motors complete with all required switches and safety devices to ensure safe and long term operation long term operation. (Min. 10000 open and closing).
Install two C Channels on the existing overhead door. Place one member in the angle area where the rails move away from the walls and one along the top of each set of rails to bind the rails together to prevent separation while the doors are opening and closing.
Install new J Boxes to allow for the relocation of Driver Awareness lights and controls to a lower level on the North Side of the Apparatus bay and install new J Boxes on the doors on the South Side of the Apparatus Room to allow for the removal and relocation of the Drivers Awareness Lights and controls to the correct side of each of the doors on the South Side to match the same height as indicated on the North Side doors. Install safety warning lights on the driver's side at the 9' mark so it is clearly visible.
Repair controls wiring on doors to operate remotely with the Monaco alarm system in the Fire Alarm Communications Center. Manual override controls at each bay shall also be repaired to bypass Monaco system upon network failure.
Repair Apparatus Room exhaust ventilation interlock system and restore to original condition where the system activates when a door is opened to remove exhaust fumes from the bays.
Prior to completion of the project, provide a 1354 with as much detail as possible. This document will assist the government in capturing all Real Property items into our base inventory.
Restore all construction area to include demolished area with equal or better condition prior to construction start (i.e. new paint to match exiting, touch up scratches on metal framing, update electrical Panel Schedules.)
Develop, outage, and phasing plan for construction and demolition. Demolition work shall not commence until all new construction electrical and controls have been rough-in and ready final terminations to minimize electrical outages and mission impacts to the Fire Station.
Upon notification of award, the contractor shall notify the FAA through the online notification system of the project details. The government Project manager can assist with some components of this document; however the contractor is responsible for proper completion and notification in accordance with Specifications.
The project requires a Temporary Airfield Construction Waiver which will be completed by the Government. This may take up to 8 weeks to complete.
All vehicle drivers will be required to obtain Airfield Drivers license and all contractor vehicles will be required to obtain POV passes for the airfield. Information will be required to be submitted 72 hours prior to attending classes. Classes are held on the 1st an 3rd Tuesday of every month. POC will be MSgt Thurmond located in building 135.
Contractor shall comply with the current CAFB Access program for all workers needing access to the airfield for work under this contract.
Contractor shall comply with requirements in the Specifications for the use of cranes on the airfield apron.
Contractor shall provide airfield barricades on the Airfield Side of Construction (low profile with lights) around areas of construction in accordance with requirements on Construction Drawings.
1.5 SUSTAINABILITY: Sustainability concepts have been incorporated into this design in accordance with LEED (Leadership in Energy and Environmental Design) requirements for Certification. A LEED Silver rating or better, in accordance with the Air Force “Memorandum of Understanding for High Performance & Sustainable Buildings" dated 24 January 2006, and is required of projects meeting LEED minimum program requirements (MPR’s).
1.6 LEGAL COMPLIANCE: All construction must be in compliance with all public laws and the Code of Federal Regulations.
2.1 CONSTRUCTION REQUIREMENTS:
2.2 BASE ACCESS AND SECURITY: The Contractor shall ensure that all employees comply with all security requirements imposed by Cannon Air Force Base (CAFB) when entering, occupying and departing the base.
All contractor personnel shall be screened for security clearances as determined by CAFB.
2.3 PROTECTION OF GOVERNMENT PROPERTY: The Contractor shall use reasonable care to avoid damaging government-owned property including buildings, equipment and vegetation at CAFB. If the Contractor fails to use reasonable care and causes damage to any government-owned property, the Contractor shall replace or repair the damage at no expense to the government. If the Contractor fails or refuses to make such repair or replacement, the cost may be deducted from the contract price.
2.4 REMOVAL OR DISPOSAL OF GOVERNMENT PROPERTY: Unauthorized removal or disposal of government property is a crime. Government property includes all materials and equipment purchased directly or indirectly by any agency of the federal government for any purpose. No government property is to be removed or disposed without written permission from the AF Contract Officer Representative (COR) and CAFB Police Department listing each item and quantity of materials and equipment to be removed or disposed.
2.5 PROJECT SCHEDULE: Reference SOW Paragraph 3.2.
2.6 SAFETY: The Contractor shall comply with UFGS 01 35 26 Governmental Safety Requirements and the Occupational Safety and Health Administration (OSHA).
2.6.1 Contracts are subject to job site inspections by OSHA and Air Force representatives. In the event of a conflict between the OSHA standards and Air Force requirements, the most stringent shall apply. Resolution of OSHA citations for violations of OSHA standards is a Contractor responsibility and shall provide for no basis of a claim against the Government.
2.6.2 All construction materials and equipment shall not block facility egress points and emergency vehicle access.
2.7 DAILY WORK SCHEDULE: Workdays shall be from 0730 to 1600, Monday to Friday. No weekend work allowed unless approved by the PM. The Contractor shall provide written notification to the Contracting Officer (CO) fourteen (14) calendar days lead time for work schedule changes including request for weekend work.
2.8 CONSTRUCTION WASTE MANAGEMENT: The Contractor shall recycle and/or salvage nonhazardous construction and demolition debris. Construction debris shall be contained on-site, collected and disposed off-site each day or as often as conditions dictate. The Contractor shall develop and implement a construction waste management plan that, at a minimum, identifies materials to be diverted from disposal and whether the materials will be sorted on-site or off-site. Submit the Waste Management Plan to the CO for the project record.
2.9 WORKMANSHIP: The work for this project shall be executed in the best and most workmanlike manner by qualified and efficient tradesmen who are skilled in their respective trades. Certified journeymen in each respective trade, or apprentices under the direct supervision of certified journeymen, are required for the performance of the work of each trade.
2.10 COORDINATION: The Contractor shall coordinate the work of all trades in a manner that will result in a professional and expeditiously completed project. The work shall be in strict accordance with prevailing industry standards and manufacturer’s instructions.
2.11 ADMINISTRATION: The Contractor shall prepare submittals as required by the project specifications, the contract for construction or as requested by the CO. Submittals may include but are not limited to product data, installation and maintenance instructions, shop drawings, calculations, samples and test reports.
Submittals shall be presented to the CO for review and approval at least two weeks prior to purchase or installation. Failure to present submittals to the CO in a timely manner will not justify any claims for a schedule delay or added costs.
2.12 VERIFICATION OF DIMENSIONS AND CONDITIONS: The Contractor may visit the premises to become thoroughly familiar with details of the work and working conditions, verify dimensions in the field, and shall advise the CO of any discrepancies before starting the work.
2.13 DELIVERY AND STORAGE: All equipment and materials delivered and placed in storage shall be stored with protection from the weather, humidity and temperature variation, dirt and dust, and any other contaminants. Store all materials in a secure, clean and dry location. Contractor shall submit a location with dimensions for government approval
2.14 TEMPORARY FENCE: Job site fencing is required around the site perimeter as well as site signage.
2.15 CONCRETE TRUCKS: Cleaning out of concrete trucks on CAFB is prohibited. Concrete truck chutes, only, may be rinsed at the construction site. Wastewater and concrete from this rinse shall be collected in a high-density polyethylene (HDPE) plastic-lined box or pit provided by the Contractor at the site. At the end of pouring operations, the Contractor shall excavate all the waste and liner and properly dispose of same. The Contractor shall dispose of all concrete debris to an authorized off base site and shall remove any and all concrete debris and residue at the end of the project at no additional cost to the Government. The pit shall be completely backfilled and the site restored to original conditions.
2.16 USE OF PREMISES: During the contract, areas will be closed to vehicle parking and vehicular traffic for construction. Coordinate and cooperate with building user’s and other Contractors scheduling and/or phasing operations to minimize conflicts.
2.17 PHASING PLAN: Contractor to provide a phasing plan before the start of construction, in accordance with Specifications and drawings.
2.18 CONSTRUCTION PERMITS: A properly approved and coordinated construction permit (AF Form 103, Civil Engineer Work Clearance Request) will be provided by the Base Civil Engineer prior to excavation activities. Open excavations shall be marked with barricades, flashing yellow lights (during night hours) and utility outages will be scheduled.
2.19 SUBMITTALS: The Schedule of Material Submittals lists the required submittals that are to be forwarded to the CO. Each submittal shall include information required by the indication on the schedule of material submittals and as described in the technical specifications on the drawings. No approval will be given on an item unless submittal information shows compliance with specified requirements. If, at any time during the duration of this contract, the Contractor desires to change materials from those originally approved, his new materials shall be submitted to and approved by the CO prior to incorporation into the project.
3.1 DELIVERABLES
3.2 PROJECT UPDATES: Weekly project updates will be emailed from the contractor project manager to the Government Project Manager and Contracting (CA/CO). This email will consist of the work that was completed that week and a one week look ahead as well as any impact on the completion date of the project.
3.3 PROJECT SCHEDULE: Design Schedule (AF 3064 and Gantt) - Within 14 business days after notice to proceed, provide a legible design schedule in printed and PDF format for design services to include dates of anticipated site visits, team meetings, design charrettes, design submittals, reviews, and other milestone dates.
Design schedule shall be in the form of an AF 3064 and Gantt chart. The schedules must be sufficiently detailed to enable tracking of design progress and the anticipation of critical tasks.
3.4 SURVEY REPORTS: Contract shall submit assessment reports for the following, potholing will be required for:
Electrical site survey of the existing conditions, new proposed duct banks, and locations where lighting poles will be installed. Two electronic copies must be submitted to Govt.
3.5 ELECTRONIC DRAWINGS: Upon completion of the documents for construction, the DBE shall provide electronic drawing files in a format compatible with AutoCAD 2010 including all external references required to view and print the drawing files properly. Include all necessary *pc3 or similar files needed to properly set line weights for printing. All drawing files shall be in a single folder or in a folder structure that will enable external referencing. Drawing files shall be organized in accordance with the DoD A/E/C CAD Standard (ERDC/ITL TR-09-2) available in PDF format on many federal websites. The DOD standard is closely based on the National CAD Standard. In addition, the DBE shall deliver to the PM:
a. Reproducible electronic drawings in PDF format.
3.6 AS-BUILT CONSTRUCTION DRAWINGS AND SPECIFICATIONS: Upon completion of construction, provide as-built drawings and specifications incorporating any departures from the original contract documents. Additionally, each page of the as-builts shall be labeled “As-Built.”
3.5.1 Electronic As-built Drawings and Specifications: Provide two copies of electronic drawing files in a format compatible with AutoCAD 2012 including all external references required to view and print the drawing files properly. Include all necessary *.ctb, *.pc3 or similar files needed to properly set line weights for printing. Drawing files should be organized in general accordance with the Department of Defense A/E/C CAD Standard (ERDC/ITL TR-12-X) available in PDF format on many federal websites. In addition, provide drawings in PDF format for on-screen viewing and reproduction. Provide final project specifications in PDF and Microsoft WORD *.doc or *.rtf format.
3.5.2 The file naming convention and file structure shown in the As-built Submittal Guidelines shall be used in lieu of what is found in the Department of Defense A/E/C CAD Standard.
3.5.3 Hardcopy As-built Drawings and Specifications: Provide four (4), two (2) full size and two (2) half size printed copies of the as-built drawings and two (2) copies of the final specifications.
3.6 OPERATION AND MAINTENANCE MANUALS: The Contractor shall prepare O&M manuals, warranties and other project closeout requirements as defined in the contract for construction.
3.7 GEOGRAPHIC INFORMATION SYSTEMS (GIS): All new facilities and utilities shall be located by the Contractor in an electronic format that is compatible with the current base GIS software and includes descriptive information required by the Air Force. When the Contractor is unable to provide GIS information as described, the Contractor shall notify the PM so that other arrangements can be made. In such cases, the Contractor shall assist Air Force agents or personnel in the generation of GIS information if requested by the PM. Subject to change, the following are the basic requirements for GIS information to be delivered to the Air Force on two electronic copies on two CD’s:

d. Units – Meters
e. Data in attribute Table will be SDSFIE version 2.6 Compliant
f. Shapefile Geodatabase format
