The purpose of this amendment is to change the previous Mohawk style from “Patcraft style: I029” to Angled Perception Tile 12BY36.

STATEMENT OF WORK

CARPET REPLACEMENT

BUILDING 792

2 AUGUST 2019

PART I – GENERAL

1.1. PURPOSE

This Statement of Work (SOW) defines the scope of work to accomplish: Building 792 the installation of new carpeting. The building is located on Moody AFB. The contractor shall provide all labor, material, equipment, management, supervision, coordination and transportation required to accomplish this work.

1.2. WORK SCHEDULE, PLANS, AND WORKING HOURS

Workdays for this contract will be standard duty hours Monday through Friday 0730-430 unless specified below. Building 792 will have occupants. The contractor shall not begin any work until he/she is 100% material complete. There are multiple rooms requiring work under the scope of this project. The contractor will only be allowed to work in a single room at any given time. Written permission from the designated Government Representative shall be obtained before making any deviation from the aforementioned schedule.

1.3. AVAILABILITY OF UTILITY SERVICES

All reasonable quantities of utilities will be made available to the contractor. The Government shall not be held responsible for interruptions of utility services, and will not be liable for contractor delays, damages, or increased costs incurred by any such interruption of service. The contractor shall be responsible for providing all labor, equipment, and materials to make necessary temporary utility connections. The temporary connections shall be coordinated with the Government Representative before being made and shall be removed upon project completion.

1.4. CLEAN-UP

All debris and rubbish not specified to become Government property resulting from the installation of new flooring shall be disposed of by the contractor off Government property.

The Government will designate a location for a contractor provided dumpster, if necessary.

Disposal will be in compliance with all applicable Federal, State, and local hauling and disposal regulations. Site clean-up shall occur daily.

1.5. SAFETY REQUREMENTS

Safety requirement shall be the applicable sections of EM 385-1-1, Oct 1992 and Occupational Safety and Health Administration (OSHA) Regulations. OSHA regulations are available for review at the Base Safety Office, Building 926. EM 385-1-1 is available for reference in the Civil Engineering Office, Building 385. The contractor shall ensure that any lifting devices or ladders meet any and all applicable safety standards. The contractor shall be required to use those lifting devices that will allow for the existing modular systems furniture to be raised for the installation of the new materials requested. The contractor will not be required to disconnect any electrical system connections to or from the modular systems furniture.

1.6. SITE CONDITIONS

A site visit will be offered pursuant to Federal Acquisition Regulation (FAR) Clause 52.237-

1. Regardless of site visit attendance, contractors acknowledge satisfaction as to the nature and location of the work, the general and local conditions, access to job site; disposal, handling and storage of materials; availability of labor, water, electric power or similar physical conditions at CAFB; the character of equipment and facilities needed preliminary to and during the performance of the work; and all other matters that can in any way affect the work or costs related to this requirement upon submission of offers. Contractors further acknowledge that any damage to the existing structure in and around facilities will be returned to existing condition prior to the completion of subject work at no additional cost to the Government in accordance with FAR Clause 52.237-2.

1.7.WARRANTY

The contractor shall provide the Government with a one-year warranty on workmanship.

The contractor shall provide any manufacturer’s warranty to the Government.

PART II – TECHNICAL

2.1 BUILDING 792, 23 MAINTENANCE GROUP

2.1.1 Removal of existing office furniture and equipment for the installation of new materials. The furniture shall be relocated within the work area to allow for the installation of new materials. The contractor will be allowed to move furniture outside of the facility while the installation of new materials are being installed. The contractor will be require to reinstall that furniture back to its original configuration prior to the end of the work day. The occupant shall be responsible for the disconnection and reconnection of all office equipment. The contractor shall use applicable lifting devices to raise and lower any existing modular furniture within the work area. The contractor will not be allowed to dissemble existing modular furniture during the course of the contract. All furniture and equipment will be placed back in its original configuration after installation of new materials.

2.1.2 The contractor shall accomplish the following work for the installation of flooring in building 792. The work area is the entire building, corridors and offices.

Restrooms are not part of this contract.

2.1.2.1. Removal of existing cove base (3200 linear feet) of 4” glued on cove base.

The contractor will carefully remove all existing cove base, taking care not to damage the wall finish or paint above the area where cove base being removed. Should the paint or wall become damaged during the removal process, the contractor will be responsible for repairing the damages, ensuring that the repairs match the existing finish and color. Color stock numbers can be obtained from the CO and COR as required. Removal of (2300 square yards) of existing glued down carpet. Removal of (150 square feet) of porcelain tile.

2.1.2.2. Prepare 20,700 square feet of floor space to receive new flooring, carpet or

LVT.

2.1.2.3. Purchase and install on of the following four options of 2300 square yards of carpet tile. From Mohawk styles: Angled Perception Tile 12BY36, color:

Perception 750, Shaw manufacture: Geometric tile color Doowop :00770, JJ Flooring manufacture: Geometric tile color Outfitter style: 7064, Geometric tile Color Platinum style:924, or Milliken manufacture: Organic Pattern Family style color: Southern Analog. Purchase and install 1175 square feet of LVT. The LVT shall be by Patcraft, style: I439V Crossover color: 00740 Woodland or equal. Purchase and install 3200 linear feet Johnsonite 4 1/2” Rubber cove base, color 32 Pebble or equal.

2.1.2.4. The Contractors shall verify all dimensions listed above. The Government does not guarantee the above dimensions are 100 percent accurate.
