MUNICIPAL SOLID WASTE COLLECTIONS
TECHNICAL QUESTIONNAIRE
Source Selection Sensitive (when filled-out) IAW FAR 2.101 and FAR 3.104
Instructions: The offeror’s technical proposal is limited to a twenty (20) page submission, and must (a) describe the contractor’s approach to conducting solid waste and recycling operations at Vandenberg AFB; (b) provide commentary describing any Notices of Violation (NOVs), fines or penalties levied against any of the entities (the specific division or independent business unit) proposed to provide services under this contract within the immediate three (3) year period preceding the release of this RFP, and; (c) address each of the technical evaluation factors detailed below.
(a) Offeror’s Approach to Conducting Solid Waste and Recycling Collection Operations at Vandenberg AFB.
The contractor shall provide a summary description in the table below of their approach to conducting solid waste and recycling collection operations at Vandenberg AFB. This description shall include the approach to management, operations, scheduling, environmental compliance, and quality assurance.
Offeror’s Approach to Conducting Solid Waste and Recycling Collection Operations at Vandenberg AFB
Narrative:
If your company is not performing ALL the work with in–house personnel, you must explain the contemplated teaming arrangement in Attachment 3, Teaming Arrangement Questionnaire. This includes, but is not limited to, the following teaming relationships: prime/sub-contractor, mentor/protégé, joint venture, etc. Provide the name of each teaming partner, the percentage of the cost of contract performance for personnel (labor) costs for each teaming partner, and a description of the work / effort to be performed by each teaming partner. If a teaming arrangement is not applicable and ALL the work will be performed by your company as the prime contractor, please indicate N/A as your response in the Teaming Arrangements Questionnaire.
Please note: FAR Clause 52.219-14, Limitations on Subcontracting, applies and paragraph (c)(1) of this clause reads as follows: “(c) By submission of an offer and execution of a contract, the Offeror/Contractor agrees that in performance of the contract in the case of a contract for –(1) Services (except construction). At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.”
(b) Notices of Violation (NOVs), Fines or Penalties The contractor shall provide commentary describing any Notices of Violation (NOVs), fines or penalties levied against any of the entities (the specific division or independent business unit) proposed to provide services under this contract within the immediate three (3) year period preceding the release of this RFP, to include the date, case number (if any), a description of the facts giving rise to the NOV, fine or penalty, the final decision, the penalty assessed, and any mitigating circumstances. If there are no NOVs, fines or penalties to report, please indicate N/A as your response to the Notices of Violation (NOVs), Fines or Penalties section in the table below.

(c) Technical Evaluation Factors
For factors 1-4 below, in the first table provide a detailed description of specific tasks performed for that factor. Use the reference table to provide up to three (3) references per factor (inclusive of prime and subcontractor(s)) to support the experience claimed in the first table. If offeror intends or proposes to use an entity other than offeror to fulfill any part of a requirement or technical evaluation factor (i.e. a sub-contractor or other teaming partner), please provide ALL the requested information for that entity as well.
Factor 1, Municipal Solid Waste (MSW), Food Waste, and Recyclables Collections: At least three (3) years of experience within the immediate five (5) year period preceding the release of this RFP performing and managing weekly municipal solid waste (MSW), food waste and recyclables collections for a service district of at least 280 collection locations, to include collecting, hauling and off-base disposal of MSW and recyclables. Provide the requested information in the tables below and use the narrative block below to provide your response to this factor. Reference PWS paragraphs 1.1 through 1.8.
To Be Considered Technically Acceptable for Factor 1: The offeror’s responses evidence the offeror (or the entity other than offeror whom offeror intends or proposes to use to fulfill a requirement or portion thereof) has three (3) years of experience within the immediate five (5) year period preceding the release of this RFP performing substantially the same work as described in the PWS at paragraphs 1.1 through 1.8 for a service district of at least 280 collection locations. References provided must support the experience claimed by offeror.

Provide the requested information in the tables below and use the narrative block below to provide your response to this factor. Reference PWS paragraphs 1.2, 1.2.4, 1.3.1, and 1.4.
To Be Considered Technically Acceptable for Factor 3: The offeror’s responses evidence the offeror (or the entity other than offeror whom the offeror intends or proposes to use to fulfill a requirement or portion thereof) has knowledge of the issues related to MSW stream and environmental contamination related to the collection and disposal of MSW. No more than two (2) NOVs received by a regulatory compliance agency within the past three (3) years will be permitted for the offeror to be considered technically acceptable. Additionally, the offeror must provide evidence of training or experience within the immediate three (3) year period preceding the release of this RFP in planning for, responding to, and mitigating MSW stream and environmental contamination. References provided must support the experience claimed by offeror.

Provide the requested information in the tables below and use the narrative block below to provide your response to this factor. Reference PWS paragraphs 1.1.10, 1.2, 1.2.3, 1.2.5, 1.5, 1.5.4, 1.6, and 1.7.
To Be Considered Technically Acceptable for Factor 4: The offeror’s responses evidence the offeror (or the entity other than offeror whom offeror intends or proposes to use to fulfill a requirement or portion thereof) has three (3) years of experience within the immediate five (5) year period preceding the release of this RFP performing substantially the same work as described in the PWS at paragraphs 1.1.10, 1.2, 1.2.3, 1.2.5, 1.5, 1.5.4, 1.6, and 1.7 for a service district of at least 280 collection locations. References provided must support the experience claimed by offeror.
