PERFORMANCE WORK STATEMENT FOR
GROUNDS MAINTENANCE
1.0 Description of Services. The Contractor shall provide all personnel, equipment, tools, supervision, and other items and services necessary to ensure that grounds maintenance is performed at Offutt AFB in a manner that will promote the growth of healthy grass, trees, shrubs, and plants and present a clean, neat, uniform and professional appearance in accordance with this Performance Work Statement (PWS). The contractor's performance will be based on the Contracting Officer Representative (COR) and Contracting Officer's (CO) evaluation of the results required by the Services Summary (SS) and not the frequency or method of performance.
It is at the contractor's discretion to determine the levels of frequencies to attain the level of service required by this PWS. The evaluation of results will be based on COR surveillance, tenant satisfaction, and customer complaints. Final results of the evaluations will be the determining factor for the success or failure of this contract. Base maps of areas maintained are provided in Appendix E, Area Maps and Site Plans. Specific ground area measurements are established in Appendix A, Ground Maintenance Estimated Quantities.
All work performed by the contractor shall be performed in accordance with all applicable laws, regulations, standards, instructions, and commercial practices. The growing season at Offutt AFB is considered to be 1 April through 31 October. Services that occur only during the growing season shall begin on 1 April and end on 31 October each year. All damage caused by the Contractor or its employees to government property and personal property (to include facilities, fences, equipment, trees and shrubs, etc.) due to Contractor operations shall be repaired or replaced by the Contractor within 15 days after identification/notification at no additional cost to the Government. If a plant should die or become unhealthy due to damage, the Contractor will be responsible for replacing the damaged plant with a plant of same size and type.
Plant replacement shall occur within 15 days after identification and/or /notification.
All fence lines shall be trimmed to prevent vegetation growth in the fence, including removal of vegetation growing up the links of the fence. All areas shall be trimmed concurrent with mowing. Application of non-selective herbicides to control grass, weeds and vegetation around obstacles to include fence lines, shall only be allowed with COR approval. Application of herbicides shall comply with all provisions in this PWS section 1.6.2. Adjuvants and dyes may also be used to enhance and track chemical applications. The contractor shall utilize equipment that will not cause rutting in the grounds area during mowing operations. The contractor shall maintain all standards identified in this PWS during dry and rainy season.
1.1. Maintain Improved Grounds. The contractor shall provide services identified below for all improved grounds as identified in Appendix E.
1.1.1. Turf Areas. Grass and vegetation shall be maintained between 2 to 4 inches, on improved grounds (including drainage ditches). Grass clippings shall be removed or mulched when visible after mowing, before leaving work area. All tree limbs, pine cones, litter and other debris, natural or manmade, shall be picked up and disposed of prior to each mowing. Ditches and drainage ways shall be free of shrubs, trees, silt and trash to in order to keep water free flowing. Contractor shall dispose of collected material in appropriate waste containers designated by the COR.
1.1.2. Trim. Grass and vegetation shall be trimmed around trees, shrubs, buildings, fences, poles, posts, fire hydrants, parking lot bumper blocks, boulders, and other fixed obstacles on improved grounds. Grass, weeds and saplings shall be trimmed around all trees. Trimming height shall match or be less than surrounding area grass heights. Litter, trash or other debris shall be picked up and disposed of properly during trimming operations.
1.1.3. Maintenance of Cemetery. Headstone trimming shall be accomplished in such a way to prevent grass from staining headstones and to protect the historical integrity of the area. Ground plaques shall be free of clipping and debris, and be visible after mowing. All other areas will be accomplished as established in the PWS for Maintaining Improved Grounds.
1.1.4. Edge. Sidewalks, driveways, curbs, patios, pavilions, and other concrete or asphalt edges shall be edged in improved grounds. Edge no more than ½-inch from the hard surface edge, maintaining an even contour with the edge surface, uniform in appearance and free of scalping, rutting, and uneven or rough cutting. Grass, vegetation and dirt from edging operations shall not be left on paved surfaces. All areas shall be edged concurrent with mowing.
1.1.5. Running Track. The base running track is located at the parade grounds. The contractor must follow the manufacturer guidelines when mowing in this area. They are as follows: Limit vehicular traffic to only light-weight maintenance equipment and mowers. Mower blades/decks must be elevated to the maximum height and all equipment shall cross track on plywood protected paths. The plywood shall be removed after crossing so that it does not interfere with runners on the track. Ensure no chemicals come in contact with the track surface. Do not allow weed eater cutting line to contact track surface.
1.2. Maintain Semi-Improved Grounds. The contractor shall provide services identified below for all semi-improved grounds as identified in Appendix E.
1.2.1. Semi-Improved. Grass, weeds and vegetation shall be maintained between 4 to 10 inches on semi-improved grounds (including drainage ditches). Ditches shall be free of shrubs, trees, silt and trash in order to keep water free flowing. This includes a 100 foot strip around the Ammo Depot fence as outlined in Appendix E.
1.2.2. Trim Semi-Improved. Grass and vegetation shall be trimmed around trees, shrubs, buildings, fences, poles, posts, fire hydrants, parking lot bumper blocks, boulders, and other fixed obstacles on semi-improved grounds. Trimming height shall match or be less than surrounding area grass heights.
1.2.3. Semi-Improved Ribbon. Maintain 10 foot ribbon as identified in Appendix E. All areas shall me maintained to the standard of 4 to 10 inches as outlined in (Para 1.2.1 & 1.2.2). The 10 foot demarcation point shall begin from the nearest fixed object of the unimproved grounds area (fence, back of curb, edge of the sidewalk, running trail and or building).
1.3. Maintain BASH Airfield. The contractor shall provide services identified below for all BASH Airfield grounds as identified in Appendix E.
1.3.1. BASH Airfield. Grass, weeds and vegetation shall be maintained between 7 to 14 inches.
The grass shall not be shorter than 7 inches in height to prevent dust and debris on the airfield and never be more than 14 inches in height to prevent bird habitation. The contractor shall begin mowing operations at the active runway paved surface and move away from active runway to the outermost area. This causes insects and animals to move toward the inside of the field and away from the runway. The contractor shall be responsible for insuring that all foreign objects and debris produced by grounds maintenance is cleared off all taxiways, and aircraft parking aprons before leaving the work area. Ditches and drainage ways shall be free of shrubs, trees, silt and trash in order to keep water free flowing (this includes 3 concrete drains on the airfield) as identified in appendix E.
1.3.2. Mow Taxiway, Runway Edge, Lights and Signs. Grass, weeds and vegetation shall be maintained in a 15 foot diameter circle between 4 to 7 inches, around runway edge lights and signs.
All lights and signs shall be mowed concurrent with BASH Airfield. (Para 1.3) No loose vegetation or debris created from operations shall be left on paved areas.
1.3.3. Trim BASH. Grass and vegetation shall be trimmed around trees, shrubs, buildings, fences, poles, posts, fire hydrants, parking lot bumper blocks, boulders, and other fixed obstacles. Trimming height shall match or be less than surrounding area grass heights. Litter, trash or other debris shall be picked up and disposed of properly during trimming operations.
1.3.4. Airfield Coordination. The Contractor shall coordinate all activities on the airfield by contacting Airfield Management. Flight-line drivers training must be completed prior to operating vehicles or mowers on the flight line. All employees working on airfield grounds shall be required to have an AF Form 483, Competency Card (flight-line driver’s license) with them at all times.
Vehicles must have a valid POV pass issued by Airfield Management prior to operating on the flight line. No equipment will be left unattended while working in these areas. The Contractor shall be responsible for ensuring that all foreign objects and debris produced by ground maintenance is cleared off all taxiways, runways, and aircraft parking aprons.
1.3.5. Airfield Tower Communications. When operating near runways, overruns and taxiways, the Contractor shall be radio-equipped to maintain constant two-way radio contact with the control tower and Airfield Management. The Contractor shall maintain contact with the control tower when operating on or within 150 feet of the runway. The Government will sign-out all communication equipment to the contractor required for this contract. The Contractor shall respond immediately to the directions of the Air Traffic Control Tower Personnel.
1.4. Maintain PL Area. The contractor shall provide services identified below for all PL grounds as identified in Appendix E.
1.4.1. PL Area. Grass, weeds and vegetation shall be maintained between 4 to 8 inches, in designated Protection Level (PL) including B524, B470 and B499. All fence lines shall be maintained between 4 to 8 inches and 30 foot clear (both sides). Extra care shall be taken around sensitive equipment installed in this PL area.
1.4.2. Trim PL Area. Grass and vegetation shall be trimmed around trees, shrubs, buildings, fences, poles, posts, fire hydrants, parking lot bumper blocks, boulders, and other fixed obstacles.
Trimming height shall match or be less than surrounding area grass heights. Litter, trash or other debris shall be picked up and disposed of properly during trimming operations.
1.5. Maintain Un-Improved Grounds. The contractor shall provide services identified below for all Un-Improved grounds as identified in Appendix E.
1.5.1. Un-Improved Grounds. Grass, weeds, and vegetation shall be mowed between 10 to 14 inches on un-improved grounds 3 times a year and scheduled 6 to 8 weeks apart (including drainage ditches). The first mowing shall begin on 15 May or at the COR’s discretion. Ditches shall be free of shrubs, trees, silt and trash in order to keep water free flowing.
1.5.2. Un-Improved Trim. Grass and vegetation shall be trimmed around trees, shrubs, buildings, fences, poles, posts, fire hydrants, parking lot bumper blocks, boulders, and other fixed obstacles on unimproved grounds. Trimming height shall match or be less than surrounding area grass heights. All areas shall be trimmed concurrent with mowing. Litter, trash or other debris shall be picked up and disposed of properly during trimming operations.
1.6. Miscellaneous.
1.6.1. Leaf Removal. (This will only be executed as funds become available and as directed by the CO or COR via a work order) Upon execution of this service: leaves, sticks, branches, tree debris and other natural materials shall be removed from improved grounds and fenceline surrounding the flight-line. Leaf removal areas will include rock beds, under/around all shrubs, parking lots, ditches and fence lines. Sparse amounts of leaves may be mulched in place if the residual mulch does not excessively cover turf. The leaf removal season is from October to April.
The Contractor shall dispose of collected material at the base’s natural debris collection area or other area/container as designated by the COR. The contractor shall have 7 business days from the work order date to complete this task.
1.6.2. Herbicide Application. (This will only be executed as funds become available and as directed by the CO or COR via a work order). Upon execution of this service: the contract shall apply a Herbicide Application to control/eradicate invasive weed within Offutt AFB; total acres to be treated is approximately 527 acres. Application shall not exterminate existing grass growth and shall not have gaps more than 12 inches between application bands. The Contractor shall submit an Herbicide Application plan to the COR and approved by the CO, prior to any herbicide application being allowed. The contractor shall have 21 business days from the work order date to complete this task.The plan must be updated before any new herbicide is used. The weed control plan shall detail:
(1) The type of herbicide(s) to be applied; with the name & EPA number & application rate (amount per acre)
(2) Herbicide Application Schedule
Contractor personnel handling and applying herbicides shall possess a current State of Nebraska certification, business license and training in the appropriate herbicide categories. A copy of all state certifications/licenses of personnel applying herbicides shall be provided to the COR seven days prior to the application of any herbicide. No uncertified technicians will be authorized to mix or apply herbicides.
Storage and Mixing. Contractors may only add water to the previously mixed herbicide chemical on base; all chemical mixing (not to include the addition of water) shall be accomplished prior to entering the base. Storage of herbicides by the contractor is not allowed on base. Contractors must have a spill kit on each vehicle capable of containing 110% of the greatest container volume. All wastes shall be the responsibility of the Contractor and will be disposed of off-site in accordance with all Federal and State regulations pertaining to hazardous chemicals, wastes, and herbicides. In no manner will the Government be responsible for off-base disposals.
1.7. Tree Trimming and Removal. (This will only be executed as funds become available and as directed by the CO or COR via a work order).
1.7.1. Tree Standards. The contractor shall perform all tree work in accordance with contract specifications and the following tree care standards, latest edition, unless otherwise directed by the CO or COR.
ANSI A300 - Standard Practices for Tree Care Operations including referenced Combined Federal Regulations (Utility Pruning and Emergency Service Restoration) ANSI A300 (Part 1) - Tree Pruning ANSI A300 (Part 3) -Tree Support Systems (Cabling, Bracing, and Guying) ANSI Z60.1 - American Standards for Nursery Stock ANSI Z133.1 - Safety Requirements for Tree Care Operations
1.7.2. Pruning/Trimming. Upon execution of this service: The contractor shall complete pruning/trimming within two (2) business days from the work order date. The trees to be trimmed will be surveyed/documented by the contractor and COR to determine the amount of hours it will take prior to beginning work. Pruning/trimming includes removal of dead and/or broken limbs or removal of limbs for required clearances. Pruning or trimming shall be accomplished in accordance with the American National Standards Institute ANSI A300 Part 1 industry standards.
1.7.3. Tree and Stump Removal. Upon execution of this service: All trees and tree debris, stumps, and stump-grinding debris shall be disposed per the applicable local laws. The Contractor shall perform stump and perimeter roots removal by completely removing, by cutting and grinding all growth to a minimum of 4 inches below grade. Stumps shall be ground within one (1) business day of the tree removal date. Stump-grinding debris shall be removed the same day grinding is performed. Backfill with topsoil and compact to match existing grades. Trees identified for removal shall be considered within the following categories, Large, Medium, and Small, as based on their diameter. The diameter of the tree shall be determined by measuring 4.5 feet above the ground.
Large Trees: Diameter 36" and above, remove within 7 business days from the work order date.
Medium Trees: Diameter 24" to less than 36", remove within 7 business days from the work order date.
Small Trees: Diameter 3" to less than 24", remove within 5 business days from the work order date.
1.8. Contractor Provided Equipment and Supplies
1.8.1. Contractor-Furnished Equipment/Vehicles. The contractor shall provide and maintain all equipment & vehicles necessary to perform the requirements of this contract. The contractor may perform cleaning, minor repair and other normal maintenance requirements of equipment on site. Maintenance and repair of contractor vehicles or their privately owned vehicles shall not be accomplished on site. Only equipment used in the performance of the contract shall be stored on government property. Contractor vehicles shall have the company name prominently displayed on both sides of the vehicle and be maintained to present a neat, professional appearance.
1.8.2. All equipment used in the performance of this contract shall be in good operable condition and carry a U/L (Underwriters Laboratory) listing, if applicable. Equipment found to be unsafe and unable to function as designed, shall not be used in performance of this contract. The COR may inspect the contractor’s equipment and vehicles at any time and direct the removal of any unsafe or unusable equipment or vehicle from the installation.
1.8.3. The contractor shall coordinate availability of a sufficient quantity of equipment and vehicles to effectively fulfill the scope of this contract. Equipment failure or maintenance requirements shall not alleviate the contractor from meeting the performance standards contained in this contract.
1.9. Miscellaneous Requirements
• Practice water conservation
• Turn off unnecessary lights
• Report fire hazards, conditions, and items in need of repair to the COR
• Turn in lost and found articles to the COR
• The Contractor shall notify the COR of any condition, including adverse weather or special requests from government personnel that may interrupt or delay performance under this PWS.
Once the condition is resolved the Contractor shall resume interrupted work as soon as practical.
When this period exceeds 24 hours the COR must approve the delay.
1.10. Additional Information: The areas indicated in Appendix F are known to have standing or retained water all year round. The contractor is still responsible to maintain the services in and around the water areas to meet the standards of the surrounding areas.
1.11. One-Time Service CLIN: The one-time service CLIN is added to only the base year for the contractor to get the areas indicated in Appendix F and the fencelines back into standard.
The areas in Appendix F are overgrown with cattails, weeds, and small woody encroachment including saplings. There are approximately 44,500 linear feet of fencelines (perimeter/inside fences). All of the dead/growing vegetation and debris is to be removed from the fences.
1.12. Environmental Management System: See appendix C ENVIRONMENTAL SPECIAL CONDITIONS for the requirement.
2. Service Summary (SS): The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The SS and the contractor’s Quality Control Plan provide information on contract requirements, the expected level of contractor performance and the expected method of government surveillance and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to ensure the services conform with the standards in the contract. During the first initial 30 days of the contract, two additional errors on each task shall be allowed in an effort to identify normal phase-in problems.

2.1. Quality Control. The contractor shall develop and maintain a Quality Control Plan (QCP) to ensure grounds maintenance services are performed in accordance with commonly accepted commercial practices and services identified in this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur. The contractor shall submit a written Quality Control Plan to the contracting officer for review, feedback and approval. The contractor shall submit the plan no later than the pre-performance conference. The plan shall specifically address the contractor’s strategy to provide quality workmanship, continual process improvement and for correcting deficiencies as required. The Contractor will identify to the CO/COR the responsible quality control inspector to notify in case of customer complaints.
2.2. Payment Adjustments. In the event of a defect the contractor will be required to re-perform the service. Re-performance is the preferred method for correcting defective services in accordance with Inspection of Services, FAR 52.246-4(e). If the contractor has exceeded the allowable defects per Table 1.0, Service Summary Table, for the month for any of the performance thresholds stated in the Service Summary, 1/30th of the CLIN price will be deducted per day until defect is corrected.
3. Government Property and Services
3.1. Government Facilities. The government will provide office and storage space in Building 301 (D). No alterations to the office space shall be made without the specific written permission from the functional commander and the CO as coordinated and approved via
The Civil Engineer work order, AF Form 332. Office space shall only be used for base operations. Storage space shall only be used to store equipment for this contract only. In case of alterations necessary for compliance with OSHA, such permission shall not be reasonably withheld. The Contractor shall return the office space to the government in the same condition as received, fair wear and tear and approved modifications. The contractor will have 5 business days after the contracts end to vacate the provided space.
3.2. Government Supplies and Equipment. None.
3.3. Government Utilities. The Government will provide electricity, water and sewage service as necessary for accomplishment of work in accordance with this contract.
3.3.1. Utility Conservation. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.
3.4. Telephone Service. The Government shall only provide base and local commercial telephone service. Long distance service, if desired, shall be at the contractor’s expense.
3.5. Security, Fire and Medical Services. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis.
3.6. Refuse Collection and Disposal. The Contractor shall use existing bulk containers to dispose of trash or refuse generated from accomplishment of services detailed in this PWS. The contractor shall adhere to all base level recycling programs to include disposal of yard/tree waste in specified dumpster collection stations or at the recycling collection center on base. The COR shall identify these specific dumpsters for yard/tree waste.
3.7. Mail Service. The Contractor shall be responsible for coordinating with the US Postal Service for the delivery of mail to the Contractor's facility or post office box.
4. General Information
4.1. Hours of Operation.
4.1.1. Normal Base Hours. Base hours of operation are 0730 hours until 1615 hours, Monday- Friday excluding holidays. The Contractor may find it necessary to deviate from the normal base hours of operation, to ensure timely completion of work under this PWS at no additional cost to the government. A contractor representative shall be available to answer questions until 1615 hours on normal duty days.
4.1.2. Recognized Holidays. The contractor is not required to provide service on federal holidays, the holidays are:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed.
4.1.3. Base Closures. Work in progress but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
4.1.4. Retrieving Identification Media. The contractor shall retrieve all government identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc. The contractor must turn in badges for all employees under the contract for final payment to be made.
4.1.5. Continuation of Essential DOD Contractor Services during Crisis. BASH Airfield mowing operations accomplished under this contract shall be performed during any wartime operation(s). Wartime operations are those actions, including contingency planning, which will be required to support current or future United States Air Force wartime requirements; in addition to functions that will be continued during wartime-related contingencies. Notwithstanding any other provision of this contract, the Contractor agrees that, in the event of the occurrence of any wartime-related contingency, the Government shall have the unilateral right to extend performance of this contract as long as the wartime contingency(ies) exist. As referenced in DOD 3020.37-4.2
4.2. Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.
4.3. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately owned vehicle while on Offutt Air Force Base.
4.4. Reporting Requirements. Contractor personnel shall report to the appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, Contractor personnel, resources, and classified or unclassified defense information.
4.5. Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
4.6. Contract Manager. The Contractor shall establish and maintain an office through which the contract manager or alternate(s) can be contacted during work hours. The contract manager or alternate shall be available during normal duty hours to meet on the installation within 2 hours with the government personnel designated by the Contracting Officer to discuss problem areas.
The Contractor shall provide the CO telephone number(s) where surveillance results and complaints can be reported. The Contractor shall also provide to the Contracting Officer the names and phone/pager numbers of Contractor POCs for after business hours including nights, weekends, and holidays. This information will be kept updated by the Contractor whenever personnel changes occur. The contract manager or alternate shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract.
4.7. Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract.
This shall be accomplished through the wear of distinctive clothing, overcoats, rain gear, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
The government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general wellbeing, or operational mission of the installation and its population.
4.8. Key Control. The Contractor shall establish and implement methods of making sure all keys/combinations issued to the Contractor by the government are not lost or misplaced and are not used by unauthorized persons. The Contractor shall not duplicate any keys issued by the government. The Contractor shall immediately report to the COR or Contracting Officer any occurrences of lost or duplicated keys. In the event keys, other than master keys, are lost or duplicated, the Contractor may be required, upon written direction of the Contracting Officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the Contractor. In the event a master key is lost or duplicated, the government shall replace all locks and keys for that system, and the total cost will be deducted from the monthly payment due the Contractor.
4.9. Schedules. The contractor shall submit a schedule for all services described in this PWS to the Contracting Officer for information only, at the pre-performance conference and at each change in the growing/non-growing season. The schedule shall identify plots, acreages, or base areas and days of the week service will occur. W h e n c hanges to the schedule occur the contractor shall notify the COR or CO via telephone, and electronic copies to be followed within 5 days after implementation. The schedule may be submitted electronically using a file format compatible with Government software programs such as "Microsoft Office" software.
4.9.1. Notification of Change. The COR shall have the ability to adjust the contractors improved grounds mowing schedules for occurrence based (non-permanent) such as DV and/or VIP visits to either the route or a specific area on base at no additional cost.
5. Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes. The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference. The contractor is responsible for controlling and accounting for all identification cards and restricted area badges. When employee(s) leave the employment of the contractor, the contractor is responsible for recovering the ID cards and all other entry/security documents from the terminated employee and immediately (within one duty day) deliver these items to the Contracting Officer. If loss or failure to turn in these items within ten (10) calendar days, the Government shall deduct the amount of $25.00 per item, per employee from the next applicable invoice. The fee is to cover the Government’s administrative and inventory cost.
5.1. Personal Security Requirements. Contractor personnel shall obtain and possess at all times while on Offutt AFB, an authorized identification (ID) card and Restricted Area Badge (only in applicable areas). Each contractor employee will be required to fill out a DD Form 1172, Application for Uniformed Services Identification Card, prior to issuance of an ID card.
This form will be available from the Quality Assurance Evaluators office or the contracting office
5.2. Restricted Area Badges. The contractor shall request a United States Air Force (USAF) Restricted Area Badge for all personnel who require unescorted entry into a restricted area(s) for specific work, for the duration of the contract. It is the responsibility of the Using Agency Facility Manager (UAFM) to assist the contractor and their employees in initiating a DD Form 254, DOD Contract Security Classification Specification, through the Unit Security Manager (USM). If for any reason the UAFM cannot assist the contractor in the processing paperwork for restricted area badges, it is the restricted area user’s responsibility to provide escorts. The contractor will wait no longer than twenty (20) minutes after initial notification for an escort to gain access into the restricted area. It is the contractor’s responsibility to fully document all cases where entry to restricted areas was not granted. All contractor personnel shall have proper identification prior to entry into restricted areas. It is the responsibility of each UAFM to brief all contractor personnel working in their area on procedures and policies of their particular restricted area.
5.3. National Agency Check (NAC). The contractor shall obtain a secret National Agency Check (NAC) investigation for its employees for unescorted entry to restricted areas. The contractor will conduct and assume the cost of background investigations for required clearances. The contractor will allow a minimum of three hundred sixty five (365) days after application for security clearances to be granted. The UAFM will provide escorts until clearances are granted. The contractor will ensure the individual to receive the NAC has completed a SF-86 by using Electronic Personnel Security Questionnaire (EPSQ). 5.4. Privately Owned Vehicles. All contractor personnel are authorized to drive privately owned vehicles on Offutt AFB as long as they have a valid driver license, valid registration and proof of insurance.
6. APPENDICES. The government will make all publications, forms, references and report formats listed available. Publications can be accessed on-line at http://www.e-publishing.af.mil/.
Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract. The Contractor shall immediately implement those changes in publications, which result in a decrease, or no change in the contract price. Prior to implementing any such revision, supplement, or amendment that will result in an increase in contract price, the Contractor shall submit to the Contracting Officer a price proposal and obtain prior approval.
Price proposals shall be submitted within 15 calendar days from the date the Contractor receives notice of the revision, supplement, or amendment giving rise to the increase in cost of performance.

1. SAFETY DATA SHEETS (formerly called: MATERIAL SAFETY DATA
SHEETS).
1.1 Definitions.
Priority chemical means a chemical identified by the interagency Environmental Leadership.
Toxic chemical means a chemical or chemical category listed in 40 CFR Chapter 1, Subpart D, § 372.65.
Hazardous material includes the following:
• Hazardous materials as defined by The Department of Transportation in 49 CFR 172
• Hazardous substances, extremely hazardous substances, toxic chemicals and hazardous waste as defined by EPA in Title III of the Superfund Amendments and Reauthorization Act and the Resource Conservation and Recovery Act, in 40 CFR
• Hazardous Chemicals and Hazardous Substances as defined by the Occupational Safety and Health Administration (OSHA) in 29 CFR 1910, Subpart H - “Hazardous Materials”
1.2 The Instructions for Implementing Executive Order 13423 requires Federal facilities to comply with the provisions of the Emergency Planning and Community Right-to-Know Act of 1986 (EPCRA) (42 U.S.C. 11001-11050) and the Pollution Prevention Act of 1990 (PPA) (42 U.S.C. 13101-13109).
The Instructions Implementing Executive Order 13423 Section VIII paragraph C(2) and the Federal Acquisition Regulation (FAR) 52.223-5 -- Pollution Prevention and Right-to-Know Information requires contractors to provide all information needed by the Federal facility to comply with the following:
(1) The emergency planning reporting requirements of Section 370 of EPCRA (40 CFR Part 370).
(2) The emergency notice requirements of Section 302 of EPCRA (40 CFR Part 302.6)
(3) The list of Safety Data Sheets required by Section 311 of EPCRA (refers to 29 CFR 1910.120)
(4) The emergency and hazardous chemical inventory forms of Section 370 of EPCRA (§ 370.40-45)
(5) The toxic chemical release inventory of 40 CFR Part 372, Subpart E, § 372.85 of EPCRA, which includes the reduction and recycling information required by Section 6607 of the Pollution Prevention Act of 1990 (42 USC 13106).
1.3 Safety Data Sheets. (SDS)
Fifteen (15) working days prior to bringing hazardous materials onto Offutt AFB the contractor shall submit the Safety Data Sheets and usage information on all hazardous materials they (including all subcontractors) intend to use throughout the life of the contract. The 55 CES’ Construction Management Element (55 CES/CEPM) person assigned to that contract, or the contract’s Contracting Officers Representative (COR), will review the information with the Environmental Element (55 CES/CEIEC).
1.4 Storage and Handling.
All hazardous materials shall be stored and handled in accordance with all Occupational Safety and Health Administration (OSHA) and National Fire Prevention Association (NFPA) requirements. In addition to those requirements, all storage and staging areas shall be provided with secondary containment and shall be located away from traffic areas and protected from any incursion into the area that could damage the containers or cause a spill. Secondary containment shall be provided by the contractor to contain at least 110% of the volume of the largest container stored so as to contain any spills or leaks. When stored outdoors, all hazardous materials shall be covered to avoid contact with the elements and to keep precipitation from entering the secondary containment structure.
1.5 Hazardous Material Storage Plan.

1.6 Spill Reporting.
Contractor shall immediately report all spills of hazardous materials to the 55 CES Fire Department and to 55 CES/CEIE. The 55 CES/CEIEC will file all reports required for environmental regulatory compliance. The Contractor is responsible for the cleanup of all spilled material including restoration of the any areas contaminated by the spill including costs incurred by the government for any assistance or remedial action taken by the government on behalf of the contractor. All clean up and restoration shall be done to the satisfaction of the government. Handling and disposal of spill clean-up material shall be in accordance with Paragraph 3.
2. REPORT OF HAZARDOUS MATERIAL CONSUMED.

3. DISPOSAL OF REGULATED WASTES.
Contractor shall comply with all federal, state, local, and Offutt AFB regulations and requirements concerning the proper handling, accumulation, and disposal of regulated wastes.
These include, but are not limited to:
• Title 40 Code of Federal Regulations (CFR) Parts 260/280 (40 CFR 260-280) Federal solid and hazardous waste regulations.
• Title 49 Code of Federal Regulations (CFR) Parts 171-173 (49 CFR 171-173) Federal transportation regulations.
• Title 40 Code of Federal Regulations (CFR) Part 112 (40 CFR 112) Spill Prevention Control and Countermeasure Plan
• State of Nebraska Administrative Code Title 128 State solid and hazardous waste regulations.

3.1 Training.
The contractor is responsible for ensuring all contract personnel who handle or are exposed to hazardous waste are trained in accordance with applicable regulations and requirements.
Supplemental hazardous waste training is available at the Environmental, Safety and Occupational Health Training Network (www.esohtn.com). This computer-based training module should not be considered to encompass all of the hazards associated with hazardous waste management. The contractor shall maintain hazardous waste training records for all contract personnel. These records will be maintained at the jobsite and available to 55 CES personnel upon request.
3.2 Accumulation, Storage, and Sampling Wastes.
Contractor activities which generate hazardous waste must operate an Initial Accumulation Point (IAP) for the collection of wastes. The IAP must have its siting approved IAW AFPAM 32- 7043 and coordinated with the Fire Department, Ground Safety, Bioenvironmental Engineering and Environmental Compliance. The operation of the IAP must conform to 40 CFR 262.34 and the Offutt AFB Hazardous Waste Management Plan (HWMP). The 55 CES/CEIE will provide waste stream analysis and waste management consultation to properly manage contractor-generated wastes to ensure that all regulatory requirements are fulfilled. Waste sampling will be accomplished by the contractor at an accredited laboratory at no additional cost to the government. Waste analysis records will be maintained by the contractor and forwarded to
55 CES/CEIEC.
Waste accumulated at the IAP will be turned over to 55 CES/CEIE or the designated hazardous waste contractor for disposal through DLA Disposition Services.
Items that can be considered regulated (hazardous) wastes include, but are not limited to, paints, thinners, solvents, adhesives, acidic and caustic compounds, fuels and oils intended for disposal, lead acid batteries, fluorescent lamps (light bulbs), etc.
3.3 Containers and Containment
Contractor shall supply all waste storage containers, which shall be new or refurbished and labeled as Department of Transportation (DOT) approved. Containers shall be compatible with the material/waste stored in them. Secondary containment (meeting the requirements of paragraph 1) for all containers shall be provided. All additional storage and handling requirements of the Offutt AFB HWMP shall be followed.
3.4 Disposal Procedures
For hazardous wastes (including universal wastes), the contractor shall contact 55 CES/CEIEC for disposal. Sampling results, safety data sheets and waste stream information will be provided.
Waste may not be accumulated in excess of 55 gallons. Prior to completion of this Contract, the contractor shall deliver all hazardous waste for disposal within three days to the 90-Day accumulation point, Bldg 594. All drum movements to any 90-day storage facility by the contractor will be at no additional cost to the government. Disposal cost for hazardous waste disposal shall be the responsibility of the government.
Prohibited wastes are liquids or liquids containing solids that are prohibited by Omaha code from being disposed of to the sanitary sewer. Contractor shall submit test results for 55 CES/CEIEC to determine proper disposal. Contractor will be directed to deliver the container to the 90-day accumulation points on base at no additional charge to the government. Disposal cost for prohibited waste disposal shall be the responsibility of the government.
Special wastes (e.g. fuel-contaminated soil) are wastes which may be disposed of at specially-designated landfills, with the landfill's prior approval. The contractor must submit a request for disposal along with all required testing analysis and receive approval of 55 CES/CEIEC for disposal of special wastes to either landfill. The Contractor is responsible for all costs associated with disposal of special wastes.
Non-regulated wastes (e.g. C and D wastes) shall be disposed of at a Nebraska Department of Environmental Quality (NDEQ) approved landfill. Contractor is responsible for all costs associated with the disposal of non-regulated wastes
4. SPILL PREVENTION CONTROL AND COUNTERMEASURES (SPCC) PLAN.
4.1 Condition of Equipment.
All contractor equipment (including containers and vehicles) shall be kept in good condition while on the base. Any equipment found leaking or dripping any hazardous material (e.g. oil, fuel, etc.) shall be taken out of service immediately until repaired and all spilled material shall be cleaned up in accordance with the requirements called out under the Hazardous Material section.
4.2 SPCC Plan.
The contractor is responsible for handling, using, and storing all regulated petroleum oil (including fuel) on the facility in accordance with the requirements of 40 CFR 112 and Offutt’s Spill Prevention Control and Countermeasures (SPCC) Plan.
