PERFORMANCE WORK STATEMENT

FOR REPAIR MAF ELEVATORs (15)

1.

DESCRIPTION OF SERVICES - The Contractor shall provide all labor, materials, tools, supplies, equipment, parts, vehicles, supervision, transportation, and other items necessary to repair, upgrade and test the 15 Elevators at 15 separate Missile Alert Facilities (MAFs) at Minot Air Force Base in a manner that will ensure continuous and safe operation. The Contractor shall perform to the standards in the contract as well as comply with all local, state, and federal regulations. All MAFs are located in North Dakota and are reflected on the Service Area Map in Appendix B.

1.1. GENERAL SITE INFORMATION. All sites are within 75 miles of Minot AFB. The contracted employees will each need to be eligible to obtain and pass an Entry Authorization Letter (EAL) process. In this process, the Minot AFB personnel will do a security/criminal background check on each employee requesting access to the sites. Each of the 15 sites has a highly secure Missile Alert Facility (MAF) building the contractor will need to process through prior to working on the elevators. During the entire duration that the contract employees are on site, they will be escorted and observed by government personnel. Before going to work on the elevators, the contract employees will go through more security checks. Cell phones or electronic devices of any type are not allowed in these secure areas. The Contractor takes total service responsibility for the equipment identified in this PWS.

1.1.1. Work conducted on Elevators will be in accordance with the American Society of Mechanical Engineers (ASME) A17.1-2007 (or applicable code for year VTE put in service), Safety Code for Elevators and Escalators (ASME A17.1), ANSI/ASME A17.2-2010, Guide for Inspection of Elevators, Escalators, and Moving Walks (applicable to equipment), A17.3-2011, Safety Code for Existing Elevators and Escalators (applicable to equipment), National Fire Protection Association Standards (NFPA), Unified Facilities Criteria (UFC) 3-600-01, Fire Protection Engineering for Facilities, Occupational Safety and Health Administration (OSHA) Standards and the recommendations of the original equipment manufacturer (OEM). Civil Engineering Manual (CEM) 21-SM80B-2-26-2 applies when changing existing components for like changes such as safety tests, switches, and hoist cables; however, it does not apply when installing new upgraded components such as the controller. In the instances it does not apply, the Contractor shall provide the operations and maintenance instructions.

1.2. ELEVATOR REPAIR UPGRADE / IMPROVEMENT - Contractor shall provide Brand Name parts and labor necessary to replace the following equipment for each MAF Elevator:

1.2.1. Controller-A new main elevator control board/enclosure (C. J. Anderson part number: REL-363-755) will be installed to for a direct replacement of the original controller – Otis part number: 30-NIBL/21N1B.

1.2.1.1. Current Elevator Specs are as follows: Passenger / Freight Combination Traction Elevator, 6,000 lb capacity, travels 33-47’ depending on the MAF site, Soft Start 1-Speed, Single Automatic Push Button, 208v AC line voltage, 12 HP/1,800 RPM motor, 47 Amps AC full load, 120v DC brake, manual doors – horizontal slide, one landing and one opening from the front (none from rear), relay logic, NEMA 1 enclosure, controller: car leveling selector system, hatch switch package, and hoist motor 12 HP.

1.2.2. Leveling system- A new leveling system will be provided that interfaces with the relay-based control system - Otis part number: AAA25500AAA/A6098B3

1.2.3. Hoistway switches - Otis part number: A6098B3

1.2.4. Fixtures Hall and Car - All call stations at the elevator landings and in the elevator cab will be manufactured for direct replacement. Hall fixtures--Otis part number AAA147QT1, 23500CH5, Car fixtures--Otis part number AAA25000DC

1.2.5. Hoistway wire and traveling cables - Otis part numbers: AAA174ZG987

1.2.6. Door locks - Direct replacement of Otis part number 144404

1.2.7. Door handles for scissors gates – Direct replacement of handles to be manufactured to match existing handles. Existing handles are Otis part number 264710-706

1.2.8. Pit switch - Otis part number AAA22800H401

1.2.9. Car top Inspection Box with GFCI and Light - Otis part number MBA24831A001, Model ACBG-GFI

1.2.10. Under car Light with GFCI Outlet--Otis part number Model ACBG-GFI

1.3. HOIST CABLE REPLACEMENT/DRUM STYLE MACHINE - Contractor shall provide parts and labor necessary to remove old and worn out hoisting cables and replace with new - Otis part number AAA712W12/AAA712Q7 (or approved equal).

1.3.1. The replacement ropes shall be traction steel of size, construction, and number to ensure proper operation of the elevator and give satisfactory wearing qualities. All ropes shall consist of at least eight strands wound around a hemp core.

1.4. ELEVATOR TESTING/FULL-LOAD SAFETY TEST - Contractor shall conduct a full load, full speed test of all safety mechanisms, over speed governors, car buffers, and counterweight buffers to include the following: 1) Check the governor and test its tripping speed with a tachometer; 2) Test the timing and operation of overload relays under simulated emergency conditions; 3) Inspect and test safety switches and circuits in the machine room, hoistway, elevator car, and pit; 4) Perform buffer compression tests if an oil buffer is used; 5) Inspect guide rails and fastenings; 6) the governor shall be recalibrated and sealed for proper tripping speed, and elevator car balances will be checked.

1.4.1. These tests impose much greater stresses on the equipment than those of normal operation. It is agreed that in making inspections or tests, the contractor assumes any and all liability for personal injury (including death) or property damage because of the action or failure of any part of the elevator equipment. If repairs are necessary to obtain proper operation of the equipment to meet the requirements of these tests, the Contractor will inquire with the Contracting Officer how to proceed. No additional work above what is contracted for and detailed in the PWS will be accomplished without prior modification to the contract and Contracting Officer approval.

1.5. TOP-OF-CAR GUARDRAIL - Otis Part # AAA24730A6 (or approved equal) - A top-of-car guardrail will be installed on top of the elevator car to protect individuals against the hazards of falling into the hoistway. The guardrail will have two levels of rail protection. It will install around the circumference of the top of the elevator car with the exception of the area above the elevator door.

1.6. WARRANTY – Warranty shall cover all parts and labor for one year after completion of each individual MAF site. Contractor shall submit detailed warranty information that identifies specific items covered as well as exclusions from the warranty and the warranty period. All copies shall be dated and signed by an authorized officer of the company. The officer’s name and title shall be typed adjacent to the signature.

1.7. OPERATION/MAINTENANCE INSTRUCTIONS - Contractor shall submit manufacturer’s published instructions.

2.

GENERAL INFORMATION.

2.1. The Contractor is solely responsible for compliance with OSHA standards for the protection of their employees. The Contractor shall ensure specific safety requirements in Air Force Occupational Safety and Health (AFOSH) standards, Air Force technical orders, and CEM21-SM80-19FM Handbook are complied with by Contractor Personnel when non-compliance would clearly present the potential to harm or damage government resources (see AFI 21-101, paragraph 2.16.). The Air Force is not responsible for ensuring that Contractors comply with “personal” safety requirements that do not present the potential to damage government resources. All work will be performed by personnel who are trained and qualified for the systems and equipment.

2.2. ENVIRONMENTAL COMPLIANCE. Contractor shall be knowledgeable of and comply with all applicable Federal, State and local regulations and requirements regarding environmental protection.

2.3. INSPECTION. After the project is completed, a third party Independent Quality Elevator Inspector (QEI) will be hired by the Government to inspect the elevator upgrade work. The Contractor must return to fix any deficient work they performed that does not pass inspection until inspection is passed/certified. Contractor is not responsible for deficiencies by others that are no fault of the Contractor.

3. PERFORMANCE ASSESSMENT - Missile Engineering Inspectors will be on-site with the contractor at all times and will provide an evaluation of the Contractor at the completion of each elevator repair. Prior to starting project, each elevator mechanic shall submit proof of license for journeymen level mechanics and proof that apprentices are actively enrolled in training IAW PWS paragraph 5.12.1.

4. GOVERNMENT FURNISHED SERVICES.

4.1. GOVERNMENT FURNISHED UTILITIES. The Government will furnish water and sewage as necessary for accomplishment of work in accordance with this contract unless unavailable.

4.2. UTILITY CONSERVATION. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.

4.3. TELEPHONE SERVICE. The Government shall only provide base and local commercial telephone service. Long distance service, if desired, shall be at the Contractor’s expense. Contractor shall only be able to use personal cellphones prior to processing through security office to begin work on MAF Elevators.

4.4. SECURITY, FIRE, AND MEDICAL SERVICES. The Government will provide police and fire protection. In the event of a medical emergency, commercial ambulance service for transporting an injured employee to a local hospital is available at the cost of the Contractor.

4.5. SOLID WASTE COLLECTION AND DISPOSAL. The Contractor shall dispose of trash or solid waste generated from accomplishment of services detailed in this PWS. Government-owned dumpsters located outside the fence of the MAF will not be utilized by the Contractor. The Contractor shall dispose of all trash/waste off-site and in accordance with existing local, state, and federal regulations, and trash/waste shall not be placed in Government refuse containers/dumpsters.

5. INSTALLATION INFORMATION

5.1. HOURS OF OPERATION. Working hours for the Contractor will be four 10-hour days normally between the hours of 7:30 a.m. and 6:00 p.m., excluding Fridays, Saturdays, Sundays, and the days federal holidays are observed. Work can take place at only one site at a time unless approved by the Missile Engineering Inspector. The Government will provide the contractor with the site location prior to each upgrade occurrence. Period of performance on this requirement will be from 30 Sep 2015 through 29 Sep 2016. All work must be completed within this time period.

5.1.1. Work scheduled but not accomplished due to weather, exercises, or actual alert will be accomplished as soon as possible after reopening the MAF.

5.1.2. The Contractor shall coordinate with Government escort/inspector, each afternoon, the proposed work relative to the next day’s services.

5.2. FEDERAL HOLIDAYS.

5.2.1. If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day after the holiday observance.

5.3. HEIGHTENED SECURITY AND FPCONs. During periods of heightened security and Force Protection Conditions (FPCONs), the Contractor shall still be responsible for continuing the elevator project unless notified by the Contracting Officer that project has been temporarily suspended or restricted. During times of heightened security, delays at the entry gates should be expected; also movement onto and around the site will be significantly restricted.

5.3. SECURITY REQUIREMENTS

5.3.1. Vehicles transporting the Contractor and his/her employees to the MAF must be parked outside of the perimeter fence in the designated parking area. If for any reason vehicles are not parked in the parking area, they shall be no closer than 25 feet from the perimeter fence and shall not impede traffic to and from the MAF.

5.3.2. When working adjacent to the project site, all security regulations shall be obeyed, motorized vehicles shall be equipped with fire extinguishers, and no smoking will be permitted in other than prescribed smoking areas. No powder-actuated tools will be permitted. The Government inspector/escort will accompany the Contractor and his/her employees at all times.

5.4. CONTRACTOR VEHICLES AND EQUIPMENT

5.4.1. Contractor vehicles shall have company name visibly displayed on each vehicle.

5.4.2. At the end of each workday and the completion of the site, the Contractor shall cleanup work area debris and shall ensure they remove all of their equipment, tools, and work/contract materials.

5.4.3. STORAGE - If approved, mobile equipment must be stored during non-working hours, except noon lunch period or intermittent break periods during the day, in the area set aside on Government property at the site or be removed off-site. The storage area shall clear the perimeter fence by a minimum of 25 feet, and the storage location must be approved by the Government inspector/ escort or Facility Manager on-site.

5.5. ENTRY AND SITE REGULATIONS

5.5.1. Entry. In order to gain entry onto a facility, Contractor personnel and Contractor-owned/ leased/ rented vehicles must be listed on an Entry Authority List (EAL), identified, and physically escorted on-site by authorized Government inspector/escort.

5.5.2. Fifteen (15) working days prior to the initial commencement of work, the Contractor must submit to the Contracting Officer a summarized list of employees and Contractor-owned /leased/ rented vehicles along with a legible photocopy of each employee's Government-issued credentialed photo ID. The employee and vehicle summarized lists and the legible photocopies of the contractor’s employee's Government-issued credentialed photo IDs will be used to prepare the contractor’s EAL referenced above.

5.5.2.1. The Contractor’s summarized list of employees’ information must show each employee’s:

a) Full name exactly as it appears on the credentialed photo identification (ID) submitted for used in conjunction with the contractor’s EAL.

b) Date of birth (DOB).

c) Full Social Security Account Number (SSAN).

d) Citizenship.

e) The Government issuing agency name and the control number (exactly as it appears on the credentialed photo ID) of the Government-issued credentialed photo ID submitted for used in conjunction with the contractor’s EAL.

f) U.S. Government-issued credentialed photo IDs must have a picture of the individual, individual’s DOB and a control number. The following are acceptable government issued credentialed photo IDs: U.S. Federal (i.e., U.S. passport or U.S. passport card), or state, commonwealth, territory, protectorate, the District of Columbia, US sovereign insular area or a tribal organization recognized by the US Department of the Interior (i.e., state- issued driver’s license, state-issued photo identification, or Tribal ID card).

5.5.2.2. The Contractor’s summarized list of vehicle information must include:

a) Vehicle Make b) Vehicle Model c) Vehicle Year d) Vehicle Primary Color e) Vehicle License Plate # (State of Issue and Number) (If no license plate, provide the vehicle identification number (VIN) or Serial Number).

f) Vehicles include: Company owed/leased/rented vehicles driven out to the MAFs and brought onsite for use (i.e., pick-up trucks, cars, vans, lineman's trucks, dump trucks, and all trailers being taken out to MAFs and brought onsite).

g) All equipment (i.e., skid-steers, bobcats, forklifts, loaders/backhoes, lifts, trenchers, air compressors, welders, etc.), materials, and hand-carried items (i.e., power tools and hand tools) brought out to the sites on a trailer are not required to be listed on the EAL. But if you drive a motor driven piece of equipment (i.e., a loader or forklift) out to the site by itself (not on a trailer), then this equipment will be considered a vehicle and information on it will need to be listed on the EAL.

h) No privately owned vehicles (POVs) will be permitted on MAFs.

5.5.2.3. Each employee must have their identity proofed and their access vetted through the National Crime Information Center database and the Security Forces Information Management System using their name, DOB, and full SSAN. The employees’ DOBs and full SSANs will only be used for this proofing and vetting process and are not used on the EAL used in the field for access.

5.5.2.4. Deletions to remove employees or vehicles from the EAL may be made without preparing a new EAL employee or vehicle summarized list, but must be requested by the Contractor in writing. However, for additions/changes of employee or vehicle information on the EAL, a new EAL employee or vehicle summarized list and a legible photocopy of each employee's credentialed photo ID must be submitted by the Contractor, in writing, fifteen (15) working days prior to any additions or changes to the EAL.

5.5.3. Upon arrival at a MAF, Contractor personnel will contact the Flight Security Controller (FSC) using the gate intercom telephone and inform the FSC of their presence at the security gate. Contractor personnel must remain outside of the MAF security fence and in or near their vehicle until the arrival of the Government inspector/escort unless prior permission from the site FSC has been granted to accomplish work while remaining off-site.

5.5.4. The Government inspector/escort will identify and search all Contractor personnel upon arrival. All Contractor personnel will provide the Government inspector/escort with the same government-issued credentialed photo ID listed on the EAL for entry access to the MAF. The Contractor's vehicles will be checked/searched prior to site entry. Only Contractor-owned/ leased/ rented vehicles listed on the EAL will be allowed site entry.

5.5.5. If Contractor personnel are not listed on the EAL, or those who are listed cannot provide proper identification, they will not be permitted on the site. Delays caused by lack of proper EAL data or proper identification shall be at no cost to the Government.

5.5.6. Restricted Area. The work to be performed is in a restricted area. Therefore, the Contractor shall conform to all requirements of AFMAN 31-108, 20 AFI 31-133, and the Minot AFB IDP 31 Vol. 1. Workers will be required to have Government inspector/escort with them at all times. The Contractor shall anticipate delays of approximately 1 hour on and 30 minutes off site each day.

5.6. HAZARDOUS MATERIAL IDENTIFICATION. Material Safety Data Sheets (MSDSs) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. MSDSs, container sizes, and material quantities must be submitted by the Contractor upon contract award at least 15 working days prior to bringing any materials onto government property. Failure to provide MSDSs or certificate when requested could result in the Contractor being considered non-responsive and result in termination of the contract. The Contractor is responsible to provide 5 CES/CEIE with monthly usage reports tallying the amount of each approved chemical used during the previous calendar month.

The Contractor shall not use products that are or contain Toxic 17 chemicals, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any HM containing one of these banned substances will not be allowed on the MAF.

5.7. HAZMAT APPROVALS. The Contractor will be responsible for purchase of all approved hazardous materials. No materials may be used without prior approval through the CO to the base Environmental Office (5 CES/CEIE). The government (5 CES/CEIE, Environmental Office) will review and approve or deny use of all chemicals that the Contractor requests to bring on to Minot AFB or any property under its control. No materials can be stored or left at Government facilities between visits.

5.7.1. SPILL RESPONSE. The Contractor will be briefed on the site-specific spill response procedures prior to start of work. The Contractor is responsible to report to workplace Government inspector or facility manager and promptly cleanup all spills and dispose of all associated waste in a manner consistent with current environmental regulations. Contractor shall have a mobile spill kit large enough to contain a spill at least 10% bigger than the largest container of cleaning product. In the event that it is necessary to utilize Government material, equipment or personnel to clean up a Contractor-caused spill, the Contractor shall be required to reimburse the Government for all associated costs.

5.7.2. HAZARDOUS MATERIAL/WASTE MANAGEMENT. Contractor is responsible for all wastes and waste disposal costs associated with PWS and must remove all waste from site when they leave. All wastes will be properly disposed of according to local, state and federal environmental regulations. Criteria for identifying hazardous waste is contained in Subpart C of 40 CFR, Part 261.

5.8. HARNESS/LANYARD. Contractor shall adhere to OSHA requirements found in CFR 1926.104. Harness/Lanyard shall be provided by contractor.

5.8.1. HARD HATS. Contractor shall adhere to OSHA requirements found in 29 CFR 1910 and 1926. Hard hat inspection criteria are found in 29 CFR 1910.135 and 1926.100. Industry and manufacturer standards shall be met on hard hats in order to meet ANSI Z89.1 standards. Hard hats shall be provided by Contractor.

5.8.2. Personnel shall not wear any objects such as rings, metal wrist bands, metal watch chains, etc., when climbing on or around the Elevator.

5.9. WEAPONS, FIREARMS, AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their Contractor-owned or privately owned vehicle while on Minot AFB.

5.10. CONTROLLED UNCLASSIFIED INFORMATION (CUI). The contractor does not require access to any CUI or other sensitive information and is not authorized any access in the performance of these duties.

5.11. FOR OFFICIAL USE ONLY (FOUO). The Contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

5.12. PERSONNEL. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The Government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

5.12.1. CRAFTSMEN. Only skilled craftsmen in the trades required for the job shall be performing the work. Elevator mechanics must be licensed in their trade and apprentices shall be actively enrolled in training. All work shall result in the highest standards of workmanship, safety, and quality. The contracting officer may, in writing, require the Contractor to remove from the project any employee deemed to be incompetent, careless, or otherwise objectionable to personnel safety, equipment, or the quality of workmanship.

5.12.1.2. WELDING, CUTTING, BRAZING, ETC. Before "Hot Work" (welding, cutting, brazing, etc.) is going to take place, the Contractor shall obtain a welding permit (AF Form 592) from Base Fire Department, within ten working days after award, before any open flame or welding operation is conducted. The Contractor shall allow five workdays for processing the permit. No welding actions shall occur without the welding permit in hand and on site. The welding permit shall be renewed every 30 days. The Contractor shall conduct his work such that, after any welding operation, a one-hour (minimum) fire watch in the area in which welding has occurred is conducted.

5.13. REPORTING REQUIREMENTS. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor personnel shall report any sensitive information (classified or CUI) discovered or accessed IAW the procedures outlined in the Initial Information Protection training provided at each MAF site prior to start of work.

5.14. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.

5.15. DAILY CLEANUP AND DISPOSAL OF CONSTRUCTION DEBRIS

5.15.1. The Contractor shall at all times keep the work area clean of all foreign matter not essential to the project completion. The Contractor shall clean up at the end of each day (within the working hours defined in paragraph 5.1) all foreign matter not essential to the project completion. The Contractor shall return all areas to the condition of cleanliness that existed before work commenced. Contract completion time shall include final cleanup of the premises in accordance with (IAW) the terms and clauses contained in this contract. It is essential that all equipment and materials clear (by a minimum of 25 feet) the perimeter fence and/or perimeter fence line during all non-working hours.

5.15.2. DISPOSAL. All debris, excess material, and salvaged material which become the property of the Contractor shall be disposed of off Government property in North Dakota State-approved disposal facilities using North Dakota State-approved procedures by the Contractor at no additional cost to the Government.

5.15.3. SITE DAMAGE. Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the Government. Any damage done will be the responsibility of the Contractor to repair or replace to acceptable standards.

5.15.4. DRAWINGS – Drawings shall be provided to the CO and 5 CES Missile Engineering Inspector. Drawings shall illustrate how the specified materials are integrated into the work as a whole. Adjacent materials that affect the complete installation of the specified products shall be shown and identified. Deviations from contract drawings shall be clearly identified and the reason for deviation clearly explained.

6.0. APPENDICES. The government will make all publications, forms, references and report formats listed available. Publications can be accessed on-line at http://www.e-publishing.af.mil/. Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract.
