FAR 52.215-1 INSTRUCTIONS, CONDITIONS AND NOTICES TO OFFERORS
1. GENERAL PROPOSAL PREPARATION INSTRUCTIONS
1.1. To ensure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient and respond directly to the requirements of this solicitation. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications and all identified evaluation factors. Failure to meet a requirement may result in an offer being ineligible for award. The response shall consist of three (3) separate parts:
Volume I - Construction Permit
Volume II - Price and Contract Forms
Volume III - Past Performance
Offerors shall submit one (1) electronic proposal of Volumes I, II and III. A paper hardcopy is optional.
The offeror shall prepare the proposal as set forth in the volume submission requirements Table below. The titles and contents of the volumes shall be as defined in this table, all which shall be within the required page limitation and with the number of copies as specified. Pages exceeding the page limitations set forth in the section will be removed from the proposal and will not be read or evaluated.

1.2. This is an all-electronic solicitation release. Hard copies are not available. The Government will post amendments to the solicitation to the Electronic Posting System (EPS) at https://www.fbo.gov.
If an amendment is issued, normal procedures relating to the acknowledgment and receipt of any such amendment as described in contract clause 52.215-1, Instructions to Offerors-Competitive Acquisition, of this Request for Proposal (RFP) shall apply. It is the responsibility of each offeror to review the web page for notice of amendments, updates or changes to current information.
1.3. Offers may be submitted by hand delivery, mail or email in response to this solicitation.
The content and page size of electronic copies must be identical to the hard copies. Facsimile submissions will not be accepted. If the offeror chooses to mail the offer via postal services, the Government will not be responsible for any non-receipt of the offer, or any failure of the offeror to verify receipt of the mailed offer.

The proposal may also be hand delivered to the 65th Contracting Flight, Building T-615, room 203.
Late proposals will be processed in accordance with FAR 15.208, "Late Submission, Modifications, and Withdrawals of Proposals". If installation access is not granted, please contact the contract administrator immediately.
NOTE: Offerors are cautioned that Lajes Field, Terceira Island, Azores, Portugal has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor's pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING PROPOSALS.
Offerors should allow sufficient time to obtain a visitor pass and arrive at Bldg. T-615 PRIOR to the time specified for receipt of offers. Offerors should verify receipt of any mailed, delivered or e-mailed proposal with the contracting point of contact.
1.4. In compliance with FAR Subpart 4.8, Government Contract Files, the Government will retain one copy of all unsuccessful proposals and will destroy all extra copies. No destruction certificate will be provided.
2. SPECIFIC PROPOSAL PREPARATION INSTRUCTIONS:
2.1. VOLUME I - CONSTRUCTION PERMIT
(a) The offeror is required to provide proof that they possess a valid Construction Permit/Alvara, minimum Class 4, issued by the Portuguese Institute of Public Market, Real Estate and Construction (IMPIC, I.P.), in accordance with Portuguese Law 41/2015 of 3 June 2015. Also contractor shall possess, or present the subcontractor(s) that will execute the work that have, Alvara categories and subcategories for:
- "1a Categoria" - Buildings and Built Heritage
- "1a Subcategoria" - Concrete structures and elements - 1st class
- "2a Categoria" - Communication Routes, Urbanization Works and other Infrastructures
- "1a Subcategoria" - Roadways and aerodromes - 1st class
- "4a Categoria" - Electrical and Mechanical Installations
- "4a Subcategoria" - Electrical and Mechanical Installations of service voltage up to 30 kV - 1st class
- "17a Subcategoria" - Support installations and signaling in transport of systems - 2nd class
- "5a Categoria" - Other jobs
- "8a SubCategoria" - Armor for reinforced concrete - 1st Class
- "10a Subcategoria" - Formwork - 1st class
Failure to provide a valid Construction Permit will result in the proposal being excluded for further evaluation.
2.2. VOLUME II - PRICE AND CONTRACT FORMS
(a) Provide an introductory page to include NCAGE Code, DUNS number, points of contact, and contact information. Complete blocks from 14 thru 20c. of Section A in the Standard Form (SF) 1442.
In doing so, the offeror assents to the contract's terms and conditions as written in the Sections A through K. An official having the authority to contractually bind your company must sign the SF 1442 in accordance with FAR 4.102, Contractor's Signature.
(b) Insert the proposed total price in page 4 for the Contract Line Item Number (CLIN).
(c) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections F through J, the offeror shall submit only those pages that require a fill-in.
(d) A letter of authorization for the contracting officer to verify the offeror's financial responsibility.
Include name, address and phone numbers for financial institutions to include points of contact.
(e) Acknowledgement of all amendments to the solicitation in accordance with the instructions on the
SF 30.
(f) All proposed prices shall be submitted in Euro, offers received in any other currency will not be considered valid.
2.3 VOLUME III - PAST PERFORMANCE
Past Performance includes current/on-going performance that is complete by 70% or greater.
(a) Offeror's shall provide a Performance Relevancy Questionnaire (PRQ), Exhibit A.
Exhibit A shall be used to provide information that demonstrates a history of performance on contracts that are similar in scope, complexity, and cost magnitude to the present project drawings and specifications. Contractor must present one (1) or more projects for the purpose of evaluating Past Performance.
Offerors should consider the recency, complexity and relevancy of the past performance information.
Recency is defined as projects that occurred within the past five (5) years, which the period begins five (5) years prior to the solicitation release date and includes current performance. The performance must have been completed within the past five (5) years to be considered for award. More Current performance may have greater impact than older performance.
Explain what aspects of the contract the offeror finds relevant to the proposed effort and identify how each contract ranks, in terms of relevancy, with respect to the other contracts addressed in the proposal.
In determining relevancy to the solicitation requirement, offerors should consider the similarity of construction methods employed, size, scope, cost magnitude, location, client type (e.g. public or private) and complexity of work performed to those required for the proposed effort. Examples of similar projects include installation of airfield or industrial light fixtures (NOTE: installation of residential light fixtures are not similar in nature). Similar cost magnitude is defined as a construction project with a value between $500,000.00 and $1,000,000.00.
(b) Offerors shall prepare and submit to prior customers (of all the projects listed on your Exhibit A submission) a Past Performance Questionnaire (PPQ), Exhibit B.
Offerors shall instruct their references to email all questionnaires directly to the contract administrator at letray.womble@us.af.mil and 65CONF.LGC.ALL@us.af.mil. The Government will use the PPQ to evaluate the recency and relevancy of the offeror's past performance.
Offerors are advised that the Government may use all data provided by the offeror in this volume and data obtained from any other sources, to include, but not limited to, data independently obtained from other Government or commercial sources; from any other sources available to the Government, to include, but not limited to, the Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), or other databases; interviews with Program/Project Managers, Contracting Officers or rely upon personal business experience with the offeror. The Government reserves the right to obtain and evaluate past performance information from any source it deems appropriate. While the Government may elect to consider data obtained from other sources, the burden of providing current, accurate, and complete past performance information rests upon the offeror.
(c) A listing of the PPQ references shall be included in the Past Performance Volume III.
3. FORMAT
Format for Volumes I, II and III shall be as follows:
(a) The proposals will be 8 1/2" x 11" paper or standard A4 paper, except for fold-outs used for charts, tables, or diagrams, which may not exceed 11" x 17".
(b) A page is defined as one face of a sheet of paper containing information.
(c) Typing shall not be less than 12 point.
4. AVAILABILITY OF FUNDS
Notice to Offerors: Funds are not presently available for this effort. Therefore, offerors must declare their proposal is valid until 31st December 2019. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any preparation costs.
