PAST PERFORMANCE QUESTIONNAIRE

THIS DOCUMENT IS SOURCE SELECTION SENSITIVE INFORMATION IAW FAR 3.104
Solicitation : FA4486-10-R-0016
When addressing Present & Past Performance evaluation criteria set forth in Section M of the above referenced solicitation, the offeror shall forward a copy of the questionnaire to all references for whom the offeror has contracted similar services with the Federal, State and Local Government during the past 5 years. If the offeror does not have government references, the offeror shall provide references from private industry sources that are not prohibited from furnishing information to the government. The questionnaires must be forwarded to a verified point of contact at each of these agencies or business organizations for completion. Prior to forwarding the questionnaire to each reference, the offeror shall complete Section A, Contractor Information. Offerors may include with their proposal, information on problems encountered on the identified contracts and the offeror’s corrective actions. The Government will consider this information, as well as information obtained from any other sources, when evaluating the offeror’s past performance. Offerors are responsible for ensuring that their reference sources receive the questionnaires in time to complete and return the questionnaire to the 65 CONS/LGCC not later than 10 calendar days prior to the proposal due date.

Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:
CODE PERFORMANCE LEVEL
E EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
VG VERY GOOD- The contractor’s performance meets contractual requirements and exceeds some
(requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
M MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
U UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
