ADDENDA TO FAR 52.212-1 - PROPOSAL PREPARATION INSTRUCTIONS

The offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers.

A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of three (3) separate parts; Part I – Price

Proposal, and Part II – Technical Proposal and Part III- Past Performance Information.

B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.

C. Specific Instructions:

1. Requirements of the RFP (CLINs & PWS) and government standards and regulations pertaining to the PWS.

2. Evaluation Factors for Award in section, 52.212-2 EVALUATION – COMMERCIAL ITEMS of this RFP.

3. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in this section, FAR 52.212-1 INSTRUCTION TO OFFERORS – COMMERCIAL ITEMS, will not be read or evaluated, and will be removed from the proposal.

Site Visit Information

A pre-proposal conference and site visit will be conducted at Building 2402, 87th Contracting Squadron conference room, JB MDL, NJ 08641 at 9:00 am 30 Apr 15. Attendees are urged to arrive at least 15 minutes prior to the scheduled start time. The site visit will last until approximately three hours but may last longer. Submit names of attendees, social security numbers, driver license numbers, date of birth and state of issue to the contracting office via email by close of business on 28 Apr 15 to karin.bogner@us.af.mil and jeremy.botkin@us.af.mil . Please provide a contact number of the individual attending the site visit in the event of a change in meeting location or time. Contractors are limited to two (2) attendees per company. All prospective offerors are urged to attend this conference.

1. PART I – PRICE PROPOSAL - Submit original .

(a) Complete blocks 12, 17a, and 30a, b, and c of the SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The

SOLICITATION constitutes the model contract.

(b) Insert proposed unit and extended prices in the Pricing Schedule. The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal(s) must be submitted for a six month bridge effort.

(c) Complete the necessary fill-ins and certifications in provisions. The provisions FAR 52.212-3, Offeror Representations and Certifications – Commercial Items, shall be returned along with the proposal.

2. PART II – TECHNICAL Proposal –Limited to no more than 50 pages. Submit one.

The Technical proposal is comprised of five separate factors. Each factor shall clearly demonstrate an understanding of all tasks specified in the PWS and describe the approach to perform the requirements of the factor. Each factor for technical rating will receive an individual rating of “Pass” or “Fail.” There will not be an overall rating for Technical and Technical Risk. The offeror shall provide the following:

1.) Subfactor – Staffing Approach: The offeror shall submit a realistic manpower breakdown that depicts sufficient numbers of personnel by labor classifications/skills to fulfill the proposed Performance Work Statement

(PWS). Sufficient details shall be provided to support manpower requirements for each specific area in the PWS, including full-time, part-time, temporaries, and cross-utilization. The offeror’s personnel will be evaluated based upon their qualifications and experience in relation to the tasks to be performed under the PWS.

Measure of Merit: This requirement is met when the offeror’s staffing plan provides suitable labor mix for all tasks described in the PWS to include appropriate qualifications, certifications, and licenses for personnel and appropriate emergency contact information.

2.) Subfactor - Quality Control Approach: The offeror shall submit an effective and complete quality control approach that addresses all requirements of the PWS. The plan must address methods to be used for identifying and preventing defects before the performance becomes unacceptable. The plan shall also address the procedures to identify unfavorable performance trends and problem areas and the corrective action that will be taken. The quality control approach will be evaluated to ensure internal procedures are in place for random maintenance inspections and to ensure compliance with technical orders, safety regulations and other directives outlined in the PWS.

Measure of Merit: This requirement is met when the contractor’s proposal addresses all elements of the PWS in a quality control approach designed for continual use by supervisors/management.

3.) Subfactor - Foreign Object Damage (FOD) Prevention Program: Offeror shall submit an FOD Prevention

Program. Offeror should state plans to control FOD generated in the course of servicing/maintenance, and found during scheduled FOD inspections.

Measure of Merit: This requirement is met when the contractor’s proposal demonstrates a plan for the prevention of

FOD, foreign object elimination, frequency of FOD inspections and reporting procedures where required.

4.) Subfactor - Tool/Equipment Control Program: Offeror shall submit an effective Tool/Equipment Control

Program for contractor-furnished tools and equipment. Plan shall show methods of tool/equipment identification

(marking), control, security, and plan for reporting tools/equipment lost on the flight line or aircraft hangars to proper authorities.

Measure of Merit: Tool/Equipment Control Program will be evaluated based upon the ability to readily identify tools and equipment a belonging to the contractor, method of security, and means of constant accountability.

5.) Subfactor – Notice of Continuation of Essential Services: In accordance with DFAR 252.237-7024, “Notice of Continuation of Essential Contractor Services,” the offeror shall provide a Continuation of Essential Contract

Services Plan which shall describe how it will continue to perform the essential contractor services of this PWS. The plan shall identify provisions made for the acquisition of essential personnel and resources, if necessary for continuity of operations for up to 30 days or until normal operations can be resumed.

Measure of Merit: The requirement is met when the contractor’s proposal addresses all elements under DFARS

252.237-7024, “Notice of Continuation of essential Contractor Services”

3. PART III – PAST PERFORMANCE INFORMATION - Only references for relevant contracts are desired. Submit one (1) copy.

(a) Quality and Satisfaction Rating for Contracts Completed in the Past Five (5) Years:

Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.

(b) Performance Surveys: The government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance. Provide a list of no more than ten (10), of the most relevant contracts performed for

Federal agencies and commercial customers within the last five (5) years. Relevant contracts include

Aircraft Services that are similar to or greater in scope, magnitude, and complexity than the effort described in the PWS. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed:

(1) Company/Division name

(2) Product/Service

(3) Contracting Agency/Customer

(4) Contract Number

(5) Contract Dollar Value

(6) Period of Performance

(7) Verified, up-to-date name, address, FAX & telephone number of the contracting officer

(8) Comments regarding compliance with contract terms and conditions

(9) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions

If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past relevant contracts.

(c) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.

(d) Documents submitted in response to this solicitation must be fully responsive to and consistent with the following:

1. Requirements of the solicitation (Item Numbers) and Performance Work Statement (PWS), and government standards and regulations pertaining to the PWS.

General Information

INFORMATION REGARDING SUBMISSION OF PROPOSAL: Hand carried proposals are to be brought to the 87th Contracting Squadron, 2402 Vandenberg Avenue, McGuire, Joint Base McGuire-

Dix-Lakehurst NJ. The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the solicitation number, and the name and address of the offeror.

Offerors are cautioned that JB MDL, NJ has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. OFFERORS SHOULD EXPECT

TO BE DELAY WHEN ACCESING THE INSTALLATION. Offerors should allow sufficient time to obtain a visitor pass and arrive at the bid depository PRIOR to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.212-1(f) “Late submission, modifications, revisions, and withdrawals of offers.”

Reference far clause entitled “Insurance Work on Government Installation”. The contractor shall, at their own expense, procure and thereafter maintain the following kinds of insurance with respect to performance under the contract.

1. Workmen’s compensation and employers liability insurance is required by law except that if this contract is to be performed in a state which does not require or permit private insurance, the compliance with the statutory or administrative requirements in any such state will be satisfactory. The required workmen’s compensation insurance shall extend to cover employer’s liability for accidental bodily injury or death and for occupational disease with a minimum liability of $100,000.

2. General liability insurance shall be required on the comprehensive form of policy and shall provide bodily injury liability and property damage liability covering the operation of all automobiles used in connection with the performance of the contract. At least the minimum limits of $200,000 per occurrence for property damage shall be required.

52.204-3 TAXPAYER IDENTIFICATION (OCT 1998)

(a) Definitions.

Common parent, as used in this provision, means that corporate entity that owns or controls an affiliated group of corporations that files its Federal income tax returns on a consolidated basis, and of which the offeror is a member.

Taxpayer Identification Number (TIN), as used in this provision, means the number required by the Internal Revenue

Service (IRS) to be used by the offeror in reporting income tax and other returns. The TIN may be either a Social

Security Number or an Employer Identification Number.

(b) All offerors must submit the information required in paragraphs (d) through (f) of this provision to comply with debt collection requirements of 31 U.S.C. 7701(c) and 3325(d), reporting requirements of 26 U.S.C. 6041, 6041A, and 6050M, and implementing regulations issued by the IRS. If the resulting contract is subject to the payment reporting requirements described in Federal Acquisition Regulation (FAR) 4.904, the failure or refusal by the offeror to furnish the information may result in a 31 percent reduction of payments otherwise due under the contract.

(c) The TIN may be used by the Government to collect and report on any delinquent amounts arising out of the offeror's relationship with the Government (31 U.S.C. 7701(c)(3)). If the resulting contract is subject to the payment reporting requirements described in FAR 4.904, the TIN provided hereunder may be matched with IRS records to verify the accuracy of the offeror's TIN.
