PERFORMANCE WORK STATEMENT

Hood and Duct Systems Cleaning Services At Little Rock AFB, AR

1. SCOPE OF WORK

1.1 Contractor to furnish all labor, parts, tools, material, facilities, and transportation necessary to conduct cleaning and degreasing of hood and duct systems across 11 buildings on Little Rock AFB. The contractor shall accomplish all services below to meet the requirements of this Performance Work Statement (PWS). All cleanings shall be accomplished in accordance with the delivery schedule provided below (See figure 1). All work to be accomplished in compliance with the National Fire Protection Association (NFPA) Standard 96, Code of Federal Regulations (CFR) 29, UL 300, Occupational Safety and Health Association (OSHA) regulations, and applicable federal, state, and local laws and regulations.

1.2 The requirements official has determined that none of the services to be performed are inherently governmental.

1.3 PERSONNEL TO CONTACT: All contact made in regards to this statement of work shall be directed to Little Rock AFB Fire &Emergency Services, Fire Prevention Chief, Mr. James Farrell at (501) 987-6529 or by email at james.farrell.1@us.af.mil.

2. WORK REQUIREMENTS

2.1 The Contractor shall conduct the following:

· Inspect and clean hoods/ducts by covering all surrounding equipment with plastic

· Clean hoods with EPA approved degreaser to include roof vents systems and filters.

· Flush and rinse areas with clean water, wipe down hoods to remove stains and dry surface areas.

· If there is a floor drain, ensure drain is plugged and the wash water with degreaser is disposed of properly

· Remove plastic; mop and clean all working areas, wipe down hoods inside and out.

· Maintain hoods/ducts cleanliness on a scheduled basis as identified in Figure 1.

The Contractor and the Government Representative shall review the current cleanliness status of all hood and duct systems included in this contract. The Contractor shall submit any discrepancies that they may find on all hood and duct systems during their cleaning to the government representative. If work is unsatisfactory per the government representative, the contractor shall fix the discrepancy until satisfactory.

2.2 Initial Status Report: The Contractor shall review the hood and duct systems to identify the current status of cleanliness. This initial status update shall be reported by the contractor to the government representative.

2.3 Reporting Instructions: The contractor shall report to the building manager prior to commencement of any service described herein, and before leaving the service area. After completion of work, the contactor shall obtain a signature on their service ticket from that individual authorized to accept services. Service ticket shall also state order number, services performed, date/time commenced, date/time completed, equipment serviced, and signature of the contractor representative who performed the work. One copy of the completed and signed ticket shall be provided to the individual accepting the services. The original service ticket shall be attached to the invoice. A report of work performance shall be forwarded to the Government Representative no later than 10 calendar days after work completion each month. All residue collected during the cleaning shall be removed by the contractor from the base and disposed of in accordance with Arkansas State regulations, OSHA, and EPA guidelines.

2.4 Contractor Qualifications: The contractor shall have only competent and qualified personnel perform the work. The personnel shall be certified to perform hood and duct cleaning services.

2.5 Quality Assurance: The Government will evaluate the Contractor’s performance under this contract. The Government Representative will participate in the observation of this contract. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer. All services to be performed by the Contractor shall be subject to review by the CO or Government Representative.

2.6 Continuation of Essential Services during Crisis: This requirement has been determined to be essential, and does require continued support during a crisis as defined in DFARS Clause 252.237-7023 and DFARS 252.237-7024.

2.7 Hours of Operation: The Contractor shall perform work Monday through Friday under this contract during the following hours: 06:30 a.m. until 4:00 p.m., excluding Federal Holidays. Work required to be performed outside of these hours shall only be done after approval from the Contracting Officer.

Federal Holidays:

New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January Washington’s Birthday - 3rd Monday in February Memorial Day - last Monday in May Independence Day - 4 July Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day - 11 November Thanksgiving Day - 4th Thursday in November Christmas Day - 25 December

If these holidays fall on Saturday, the preceding Friday shall be observed. If these holidays fall on Sunday, the following Monday shall be observed. If a holiday falls on a scheduled service day, the Contractor shall be responsible for rescheduling services for the first day following the holiday observance.

2.8 Contractor Personnel: The Contractor shall not employ persons on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security and general well-being or operational mission of the installation and its population. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities. The Contractor shall be prohibited from employing off-duty COR’s who are surveilling any con-tracts/subcontracts awarded to the Contractor. All contract employees depending on particular condition, occasion or place, or job assignment shall require professional, suitable attire. All Contractor employees shall conduct themselves in a professional, courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor employees in performance of this PWS shall be the responsibility and prerogative of the contractor; however, the Contractor shall comply with the requirements set forth in this PWS and Little Rock AFB regulations concerning conduct of employees as referenced herein. The Contractor shall employ and maintain a technically trained and experienced work force.

2.9 Environmental Protection and Safety: The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and requirements regarding environmental protection and occupational safety and health. The Contractor is solely responsible for compliance with the Occupational Safety and Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days. The Contractor shall comply with any laws or regulation changes during the term of this contract. The Contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling (including hazardous materials).

2.10 Hazardous Material (HAZMAT): The Contractor shall not store chemicals/HAZMAT on Little Rock AFB. All chemicals/HAZMAT shall be approved prior to being transported on base for use by any contractor. The Contractor shall prepare and submit to the Government Representative, ten (10) calendar days after contract award date, one (1) each, Chemical/Hazardous Material Request Authorization Form (Air Force Form 3952) and Safety Data Sheets (SDS) for each chemical, material and substance proposed to be used in performance of this PWS. The Government Representative will forward the completed AF Form 3952(s) and SDS to 19 CES/CEIE for approval (POC: Mr. Weston Lee (501)-987-8135 and Mr. Ronnie Shaw (501)-987-. The Contractor shall not transport or use any chemical, material or substance on Little Rock AFB before written approval is provided by 19 CES/CEIE. Contractor shall maintain copies of SDS(s) for all chemicals. The SDS(s) shall correspond with the manufacturers of the chemicals on hand. Manufacturer, material, and SDS shall match. NOTE: SDS refers to manufacturer SDS. Once material is approved for use, contractor shall contact base Environmental Management (EM) (19 CES/CEIE) and provide a list of all material. 19 CES/CEIE shall determine any additional material requirements based on the hazard code of the material. . Compliance required in accordance with Air Force Manual 32-7002 (AFMAN 32-7002).

2.11 Health and Safety on Government Installation: In performing work under this contract on a Government installation, the Contractor shall comply with federal, state and local regulations regarding occupational safety and health and the specific health and safety requirements established by this contract. The Contractor shall notify the QAP of any injuries that occur to Contractor personnel while on the installation. The Contractor shall notify the QAP of damage to Government property or equipment during the execution of the contract.

2.12 Emergency Procedures: Contractor employees shall follow the direction of Government employees in regards to emergency evacuation procedures, i.e. fire, tornado, active shooter and bomb threats. It is the Contractors’ responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with government personnel in all fire and tornado drills, and at no additional cost to the government.

3. SCHEDULING AND COORDINATION

3.1 Work shall be scheduled with each building manager, shift supervisor, or user and coordinated, as needed, with the QAP, Mr. James Farrell at (501) 987-6529 or by email at james.farrell.1@us.af.mil. Upon notice to proceed, the contractor shall establish a schedule of work for the initial cleaning by making the necessary arrangements with each building manager. The schedule of work shall be forwarded to the QAP. Subsequent cleanings shall follow the delivery schedule listed in Figure 1. The contractor shall schedule each month’s service with the building managers and forward the schedule to the QAP at least one week prior to work performance.

4. GENERAL REQUIREMENTS

4.1. Contractor personnel and vehicles must possess appropriate passes. These shall be obtained from Installation Security Forces Pass and ID at the Vehicle Inspection Center.

4.2. The Contractor is subject to all installation fire, safety, security, and traffic regulations.

4.3. The Government contracts on the understanding that the Contractor is complying with applicable laws concerning workplace safety.

4.4. The Contractor is subject to all citations and penalties for violation of applicable workplace safety laws. The burden of compliance rests with the Contractor. Enforcement of workplace safety laws is the responsibility of the agency specified in the applicable law.

4.5. The Contracting Officer may notify the Contractor of hazards in Contractor operations that endanger Government personnel or property and require appropriate action. This provision does not constitute assumption of liability for Occupational Safety and Health compliance on the part of the Government or negate other agency responsibility for enforcement of applicable laws.

4.6. Accident prevention principles must be thoroughly integrated into all operations. Unsafe conditions and practices, as identified by the designated representative, must be corrected immediately.
