STATEMENT OF WORK

HULBURT FIELD FIRE DEPARTMENT

3,000 GALLON ARFF VEHICLE LEASE

27 Aug 18

SCOPE OF WORK. The contractor shall provide the Government with one (1) 3,000 gallon Aircraft Rescue Fire Fighting (ARFF) Crash Vehicle, Non-Ultra High Pressure (UHP), model year 1995 or newer. Site of delivery is located at Bldg 90734, 210 Bennett Ave, Hurlburt Field, Florida, 32544.

1. SPECIFICATION REQUIREMENTS.

1.1 The contractor shall furnish one ARFF vehicle. This vehicle shall be outfitted to the original manufacturer’s standards of operational condition, and shall meet all Original Equipment Manufacturers (OEM) standards. All work and documentation must be compliant with all standard Manufacturers Specifications and The National Fire Protection Association (NFPA) 414, Standard for aircraft Rescue Firefighting Vehicles (2017 edition). All repairs, if needed, shall be performed by the contractor or an Emergency Vehicle Technician (EVT) certified person/agency designated by the contractor. The contractor shall provide a technically qualified representative and a Government representative will do a joint Technical Inspection (TI) of vehicle to determine the maintenance requirements, to include all labor and part cost, to ensure compliance with NFPA 414, chapter 4 requirements. Both will agree upon the necessary maintenance to be performed. All results must be documented on the inspection sheet and the forms will be signed by all parties.

1.2 The contractor shall provide all personnel, equipment and materials necessary to perform the required services and dispose of any residue generated as a result of the work performed. Contractors shall have an ARFF Technician on the phone with department within 1 hour of being notified of a problem. Contractors shall have an ARFF technician on site (if required) within 48 hours due to the distance to Hurlburt Field. Providing vehicle goes out of service, contractors shall make all provision to get vehicle back in service within 72 hours. If vehicle cannot be repaired within the 72 hours, provide the same like vehicle upon availability. The contractor shall deduct from their invoice at a pro-rated amount for each day the vehicle is unserviceable after 72 hours.

1.3 Vehicle Specifications. Any additional vehicle specifications not described below shall meet NFPA 414.

1.3.1 Seating Capacity: The seating capacity in the cab shall be a minimum of three (3).

1.3.2 Transmission: Truck shall be an automatic with pump and roll capability.

1.3.3 Tires: To include (1) serviceable spare tire, shall be in accordance with NFPA

414, Chapter 4, section 4.8 (2017 edition).

1.3.4 Wheel Chocks: There shall be one (1) pair of folding, wheel chocks provided and wheel chock brackets.

1.3.5 Mirrors: A mirror shall be mounted on each side of the front cab door.

1.3.6 Air Conditioning: A high performance, air conditioning system shall be furnished inside the cab and crew cab. The air conditioning system shall be capable of cooling the average cab temperature from 100 degrees Fahrenheit to 75 degrees Fahrenheit within 30 minutes at 50 percent relative humidity.

1.3.7 Exterior Lighting: Exterior lighting shall meet or exceed Federal Department of

Transportation, Federal Motor Vehicle Safety Standards and NFPA 414, Chapter

4.25. Lighting and Electrical Equipment requirements (2017 edition).

1.3.8 Water Tank. Shall have a capacity of 3000 - 3300 gallons and meet the Water tank requirements of NFPA 414, chapter 4, section 4.16.

1.3.9 Foam Tank. Shall have capacity of at least 400 gallons, provide a 6% concentrate foam system and meet requirements of NFPA 414 chapter 4, section 4.17 (2017 edition).

1.3.10 Pump. Shall provide at least 1,500 GPM’s and meet minimum requirements of

NFPA 414 chapter 4, section 4.2 (2017 edition).

1.3.11 Roof Turret. Shall be capable of delivering at least 1250 GPM PSI and meet minimum requirements of NFPA 414 chapter 4, section 4.2 (2017 edition).

1.3.12 Bumper Turret. Shall be capable of at least 250 GPM and meet minimum requirements of NFPA 414 chapter 4, section 4.2 (2017 edition).

1.3.13 Handline. Provide (2) handlines of 95 GPM with minimum 150 foot length and meet minimum requirements of NFPA 414 chapter 4, section 4.2 (2017 edition).

1.3.14 Secondary firefighting system. Dry chemical handline with at least 100 lb.

capacity and hose requirements as outlined in NFPA 414, Chapter 4, table 4.1.1, section 4. Complimentary Agent for Vehicle Water tank Capacity greater than 1585 gallons. Must meet minimum requirements of NFPA 414, chapter 4, section 4.22 Complimentary Agent System (2017 edition).

1.3.15 Emergency Lighting. Trucks shall have appropriate emergency lighting in compliance with NFPA 414, Chapter 4, section 4.25 Lighting and Electrical Equipment (2017 Edition).

1.3.16 Siren. Trucks shall have electronic siren as required in NFPA 414, chapter 5, section 5.2.2 (2017 edition).

1.4 Delivery. The vehicle shall be delivered to the Hurlburt Field Fire Department. At time of delivery, complete electronic or paperbound operation and maintenance technical order, owner’s manual or equivalent, covering the apparatus shall be provided. A permanent plate with specifications depicting vehicles capabilities as required according to NFPA 414, chapter 4 (2017 edition), shall be mounted in the driver's compartment specifying the quantity and type of fluids required including engine oil, engine coolant, transmission, pump transmission lubrication, pump primer and drive axle.

1.5 Documentation. Provided at the time of delivery shall also include apparatus safety information in accordance with NFPA 414, chapter 4, section 4.2 (2017 edition) that shall address key safety considerations for personnel to follow when they are driving, operating, and maintaining the apparatus.

1.6 Performance Test. A road test shall be conducted with the apparatus fully loaded and a continuous run of no less than four (4) miles. During that time the apparatus will show no loss of power nor will it overheat. The transmission drive shaft or shafts and the axles will run quietly and be free of abnormal vibration or noise. The apparatus shall meet NFPA 414, chapter 6 (2017 edition).

1.7 Pump Test. The pump shall be tested, approved, and certified by an independent third party testing agency at the manufacturer's expense. The test results, along with the pump manufacturer's certification of hydrostatic test, the engine manufacturer's certified brake horsepower curve shall be forwarded to the Fire Department. The date shall not expire during the lease agreement. The apparatus shall meet NFPA 414, chapter 6 requirements.

1.8 Warranty. The warranty shall be bumper to bumper including the chassis, pump, body, firefighting system, electrical system for the duration of the lease agreement and ensure continued compliance with NFPA 414 chapter 6 (2017 edition).

1.9 Technical Inspection (T.I.). The Government will inspect the ARFF vehicle to assure all requirements and vehicle specifications are met.

1.10 Training. The contractor shall provide two (2) consecutive days of training (one day for each shift) per location to approximately 12-16 fire department personnel each day covering operation, inspection, firefighting systems operation and servicing.

1.11 Period of Performance. The lease period shall be 12 months with an option to extend for an additional 12 months on a month by month basis.

2. GOVERNMENT PROVIDED EQUIPMENT AND SERVICES.

2.1 The Government will furnish, without cost to the Contractor all utilities deemed necessary to accomplish the needed maintenance that are conducted on Hurlburt Field.

3. GENERAL INFORMATION

3.1 Security Requirements. The Contractor and his employees shall be required to comply with pertinent Installation Regulations on security, which are in effect or may be put into effect during the course of performance of work/maintenance under the contract.

3.2 Contractor/Employee Base Pass and Identification, Special Clearances, and Vehicle Passes. The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations and other requirements stated herein. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.

When reporting to the registration office, the authorized contractor individual(s) shall provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. Hurlburt Field adheres to the guidelines of the REAL ID Act outlined in Air Force Guidance Memorandum to AFMAN 31-113.

Driver’s Licenses from the following non-compliant states are unauthorized IAW the REAL ID Act will not be accepted for entry into Hurlburt Field:

Current Identification which may be used in place of an unauthorized State Driver's License are the following:

US Passport Foreign Passport Veterans Identification Card Permanent Resident card Federal personal identification Cards (Contractor ID)

3.3 Contractor Employee List. This form shall list every employee requiring base access.

Every time employees change, it shall be updated to reflect additions and/or deletions.

Identify those individuals you wish to have escort authority by including the word “escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center.

The contractor shall submit the employee listing to the Contracting Officer no less than 3 days prior to the performance start date and at least one day prior as changes occur. Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification IAW paragraph 4.1.2.1. above.

If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.

During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.
