Statement of Work Youth Center Hurlburt Field, FL

Installation of Vulcanized Rubber Flooring

DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide and install vulcanized rubber flooring on the Youth Center gymnasium floor 117 McMillan St., (Bldg. 90304) at Hurlburt Field, FL 32544.

1. SPECIFICATION REQUIREMENTS.

1.1. The contractor shall provide and install Mondo Advance or equivalent vulcanized rubber flooring with reinforced underlayment over the existing poured urethane floor. The Mondo Advance shall consist of three layers; Performance layer, Load Disbursement Layer, and Comfort Layer. The Youth Center gymnasium floor is approximately 6,700 sq. feet.

1.2. The contractor shall provide vulcanized rubber flooring that has a protective slip sheet that is fiberglass reinforced to ensure no moisture or delamination issues occur.

1.2.1. The contractor shall inform the Government POC 72 hours in advance prior to laying down any type of chemicals, such as, urethane, coatings, and/or paints so that HVAC settings may be changed to ensure potentially dangerous fumes are not distributed throughout the facility.

1.3. The contractor shall provide Mondo Advance Performance or equivalent vulcanized rubber flooring. The flooring shall be a minimum of 6mm thick with two (2) layers vulcanized together and shall be fully glued to the reinforced underlayment.

1.4. The contractor shall provide the following game markings on top of the vulcanized rubber flooring;

1.4.1. National Federation of State High School Associations (NFHS) basketball, badminton and volleyball court game markings.

1.4.1.1. The contractor shall use the following colors for the game markings;

1.4.1.1.1. Basketball lines in white, Badminton in royal blue, and Volleyball in black.

1.4.1.1.2. The contractor shall provide paint brand, paint lines, and paint codes with their quotes. The game markings shall be painted onto the vulcanized rubber flooring.

1.4.2. The contractor shall apply one full and half basketball court and one full and half volleyball court.

1.4.3. The basketball court, volleyball court, and the badminton court shall be applied with NFHS standard game markings.

1.4.4. LOGO: The contractor shall provide a 2-part polyurethane painted logo on top of the vulcanized rubber flooring. A pdf or jpeg file shall be provided at the time of award.

1.5. The contractor shall level the vulcanized rubber flooring with the existing volleyball metal plates and volleyball sleeves.

1.6. The vulcanized rubber flooring shall be vulcanized, nonporous, and antimicrobial and antibacterial, and slip resistant.

1.7. The vulcanized rubber flooring shall be class one (1) fire rated code rating and smoke density code rating.

1.8. The Youth Center gymnasium currently has existing manual telescopic bleachers. The contractor shall remove the existing manual telescopic bleachers without any damage from the gymnasium wall. The contractor shall move the existing manual telescopic bleachers from one side of the gymnasium to the other side of the gymnasium floor during installation of the vulcanized rubber flooring. The contractor shall replace the existing manual telescopic bleachers to their existing place on the gymnasium wall after the installation of the vulcanized rubber flooring. The existing manual telescopic bleachers will remain in their existing location during the game marking process.

1.9. The contractor shall remove and replace all existing grey vinyl baseboards around the Youth Center gymnasium wall. The replacement product shall be of the same color, shade quality and appearance when installed as the product being replaced. The contractor shall submit a sample of the product to the Government for approval prior to installation to confirm the product is the same.

1.10. The contractor shall remove and replace all existing transitional metal pieces in the double-door entry areas. The replacement products shall be of the same color, shade, quality and appearance when installed as the product being replaced.

1.11. PERIOD OF PERFORMANCE: The period of performance shall be 14 days after start of on site work.

1.12. SITE AREA PERFORMANCE: The contractor shall utilize the east gate entry of the Youth Center for material delivery and as a small scale laydown area.

1.13. The contractor shall provide barricades, caution tape, signage or any other material to block off the east gate entry during the period of performance. The east gate entry shall be safely cleared from the Youth Center children, public, and personnel. The east gate area shall be clear of debris and trash on a daily basis.

1.14. DEBRIS: All trash and debris left by contractor personnel during the performance of work shall be removed from the work location daily and transported off Hurlburt Field, FL to an official debris/trash location of the contractor’s choice.

1.15. PRODUCT WARRANTY.

1.15.1. The contractor shall warranty all materials and installation/workmanship for ten (10) years guarantee after acceptance.

1.15.2. The contractor shall replace any defective flooring, baseboards, logo, and/or any other materials used during the period of performance to complete the requirement for the period of ten years from the date of acceptance.

1.15.3. The replacement of parts under warranty shall not extend the warranty period beyond the original ten year period. In addition, the contractor shall perform the required labor to repair or install the components for the period of one year from the original installation date.

1.15.4. All costs for removal and reinstallation of the defective product, shipping, and re-installation shall be the responsibility of the contractor.

1.15.5. The contractor shall identify all items being installed which are covered by a manufacturer’s warranty.

1.16. ON-SITE SURVEY: The contractor is encouraged to conduct an on-site walk through at the Hurlburt Youth Center gymnasium prior to the award to confirm exact dimensions, specifications, and requirements that may arise. The pricing provided in the contractor’s quote shall be Firm-Fixed Priced (FFP). During the site visit, the contractor will be allowed to make accurate measurements of the existing flooring.

2. GOVERNMENT PROVIDED EQUIPMENT AND SERVICES:

2.1. The site diagrams of the gymnasium to install the vulcanized rubber flooring and present existing game markings are provided in Appendix A.

2.2. The Government will provide the contractor with electricity at the work location for performance of installing the vulcanized rubber flooring. The use of this utility shall be monitored by Government personnel and shall be conserved by the contractor.

3. GENERAL INFORMATION.

3.1. Contractor Personnel.

3.1.1. CONTRACT MANAGER. The contractor shall provide a contract manager who shall be responsible for the performance of work. The contract manager’s contact information shall be provided to the Government point of contact within three (3) business days after contract award.

3.1.2. PHYSICAL SECURITY. The contractor shall ensure no Government owned or controlled real or personal property is damaged during any service provided under the contract. The contractor shall be responsible for the repair or replacement of any governmental property damaged by the contractor at no expense to the government. At the end of each work period, all Government facilities, equipment, and materials shall be secured and safeguarded.

3.1.3. The contractor personnel are prohibited from possessing weapons, firearms or ammunition on Hurlburt Field, FL.

3.1.4. The contractor shall maintain OSHA compliance during the performance period, and all personnel shall maintain Personal Protective Equipment (PPE).

3.1.5. The contractor shall report to the Government point of contact any information that would pose a threat to the security and safety of others during the work performance.

3.1.6. The contractor personnel shall leave the location during work performance, and smoke, or eat, only in designated locations.

3.1.7. The contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. All articles of clothing shall be reasonably clean, in good repair, and shall not display any obscene or objectionable words, phrases, sketches or pictures.

4. HOURS OF OPERATION.

4.1. DUTY HOURS: The contractor shall perform required services between 6:00 a.m. and 7:00 p.m., Monday through Thursday, and Friday 6:00 a.m. to 9:00 p.m. except Federal holidays or when alternate duty days or hours are agreed upon in advance by the Government and contractor. Alternate duty days may include Saturday and Sunday from 9:00 a.m. to 6:00 p.m. No additional fees shall be charged for work performed on mutually agreed days/times.

4.2. FEDERAL HOLIDAYS: The contractor shall not work on Federal holidays unless the dates and times for performance are coordinated in advance with the Government. The following is a list of Federal holidays observed by this installation. Repairs should not be coordinated on these dates unless coordinated with the Government Point of Contact in advance.

4.2.1. CONTRACTOR EMPLOYEE BASE PASS, IDENTIFICATION, AND Vehicle PASSES: The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations and other requirements stated herein. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.

4.2.2. The REAL ID Act of 2005 established minimum standards for the production and issuance of state-issued driver’s licenses and ID cards. Hurlburt Field will comply with the guidelines of the REAL ID Act as outlined in the Air Force Guidance Memorandum to AFMAN 31-113. State-issued driver’s licenses and ID cards from states not meeting the standards can no longer be used for accessing Federal facilities to include Air Force installations.

4.2.3. Driver’s Licenses from the following non-compliant states are unauthorized IAW the REAL ID Act will not be accepted for entry into Hurlburt Field. As of 30 January 2017, the following states are non-compliant with the REAL ID driver’s license standards:

4.2.4. Current Identification which may be used in place of an unauthorized State Driver's License are the following:

US Passport Foreign Passport Veterans Identification Card Permanent Resident card Federal personal identification Cards (Contractor ID)

4.2.5. THE CONTRACTOR EMPLOYEE LIST: This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office

FA4417-18-Q-0014 Attachment 3 representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center.

4.2.6. The contractor shall submit the employee listing the Contracting Officer no less than three (3) days prior to the contract performance start date and at least one (1) day prior as changes occur. Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification IAW paragraph 4.1.2.1. above.

4.2.7. If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.

4.2.8. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

4.2.9. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.
