A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, and representations and certifications. Failure to meet a requirement may result in an offer being ineligible for award.

Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of a Price Proposal (on the SF 1449 Schedule of

Supplies/Services and all applicable amendments) and a signed cover letter at the vice-president level or equivalent or higher including the below information:

1. Identify the individual(s) with the authority to obligate or otherwise bind your company to the resulting contract and, if necessary, future contract modifications; and

2. A statement of Compliance with the Collective Bargaining Agreement (CBA) provided at Attachment

2 of the RFP acknowledging that:

(i) your company is in compliance with the requirements of the CBA,

(ii) the prices proposed for labor covered by the CBA were formulated using the minimum wage and benefit rates specified in those documents as required for covered contractor personnel;

(iii) the minimum wage and benefit rates specified in the CBA are not escalated beyond the basic period of performance (FAR 52.222-43(a) and (b)).

(iv) Indicate what steps will be taken to ensure that, in the event a follow-on CBA is entered into, wages, fringe benefits, and periodic increases agreed to will not vary substantially from those prevailing for similar services in the same locality (FAR 22.1013).

B. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:

1. Requirements of the RFP (CLINSs & PWS) and government standards and regulations pertaining to the

PWS.

2. Evaluation Factors for Award in section, 52.212-2 EVALUATION – COMMERCIAL ITEMS of this

RFP.

C. Please submit one (1) hard copy, paper proposal and one (1) electronic copy proposal in .pdf, Microsoft Word

2007 format or earlier on a non-rewriteable Compact Disc (CDR).

1. Complete Blocks 17a, (including adding the CAGE code to the “Code” section); 17b and 26 (which will contain the total dollar value proposed offer); and 30a, b, and c of the SF 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation, with attachments. The solicitation constitutes the model contract.

2. Insert proposed unit and extended prices (rounded to 2 decimal places) in the SF1449 Schedule of

Supplies/Services for all FFP CLINs (X001-X006) in the Base Period plus all Option Periods. The extended amounts must equal the unit price multiplied by the number of units.

3. Complete the necessary fill-ins and certifications in clauses/provisions within the solicitation, to include but not limited to FAR 52.209-7, 52.212-3 and DFARS 252.212-7000 and return along with your proposal. Ensure that your company’s data is current in both the Central Contractor Registration (CCR) and Online Representation and Certifications systems. The Internet addresses for those two systems are www.ccr.gov and https://orca.bpn.gov respectively.

4. Return any solicitation amendments to the original solicitation documents with the signature from the same official authorized to sign the SF 1449.

D. Past Performance Questionnaire - The offeror shall send out and track the completion of the Past Performance

Questionnaire in Attachment 3 of the RFP for recent and relevant contracts. The responsibility to timely send out and track the completion of the Past Performance Questionnaire rests solely with the offeror. The offeror shall exert its best efforts to ensure the points of contact submit completed Past Performance Questionnaires directly to the

Contracting Officer specified on the second page of the Past Performance Questionnaire by the closing date of the solicitation.

E. General Information. Mail or hand-deliver the proposal by the date/time specified in SF 1449, Block 8 to the

Contracting Office at 81 CONS, 310 M Street Bldg 4605, Rm 102, Keesler AFB MS 39531, Attn: Ms. Carlet Jones.

The Contracting Squadron, Bldg 4605, has a contractor entrance south of the Keesler AFB White Avenue Entry

Control Point. To prevent any delay in a hand delivered proposal, delivery can be made through the contractor entrance during normal business hours (0730-1630). Late proposals will be processed in accordance with FAR

52.212-1(f) “Late submission, modifications, revisions, and withdrawals of offers.” Electronic transmissions by fax or email, of proposals are not authorized.

F. The offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipt of offers.

ADDENDUM TO 52.212-4 CONTRACT TERMS AND CONDITIONS—COMMERCIAL ITEMS

The following paragraphs of this clause are tailored as follows:

(a) Inspection/Acceptance. Inspection and acceptance of the services will be performed at Keesler Air Force Base, MS, by 81 FSS.

(c) Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.

SUBMISSION OF INVOICES: The contractor will be paid monthly on the submission of proper invoices for the services performed or supplies furnished during the preceding month. Invoices shall be submitted on a monthly basis covering the items furnished the preceding month.

ADDENDUM TO 52.228-5 INSURANCE - WORK ON A GOVERNMENT INSTALLATION

The Contractor shall, at its own expense, procure and thereafter maintain the following kinds of insurance with respect to performance under the contract.

(a) Workman’s Compensation and Employers Liability Insurance as required by law except that if this contract is to be performed in a State which does not require or permit private insurance, then compliance with the statutory or administrative requirements in any such State will be satisfactory. The required Workmen’s Compensation insurance shall extend to cover employer’s liability for accidental bodily injury or death and for occupational disease with a minimum liability limit of $100,000.

(b) General Liability Insurance. Bodily injury liability insurance, in the minimum limits of $500,000 per occurrence shall be required on the comprehensive form of policy.

(c) Automobile Liability Insurance. This insurance shall be required on the comprehensive form of policy and shall provide bodily injury liability and property damage liability covering the operation of all automobiles used in connection with the performance of the contract. At least the minimum limits of $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage shall be required.

ADDENDUM TO 52.237-1 SITE VISIT

A site visit will be held on 22 May 2012 at 0900 Central Time. All parties will meet at the 81st Contracting

Squadron, 310 M Street, Bldg 4605, Keesler AFB, MS prior to departing to the facilities. All parties must contact

Mr. Reggie Temple, reggie.temple@us.af.mil, prior to 0900 CST on 21 May 2012 to confirm attendance. Each vendor is limited to two attendees. The number of attendees should be included in the confirmation. In order to gain access to the installation, you must have proper base authorization. To obtain a base pass, a valid driver’s license, current registration, and proof of insurance must be provided for each vehicle. All individuals entering the installation must have picture identification. Any questions in reference to the site visit should be directed to Mr.

Reggie Temple at the contact information provided above.

52.219-28 POST-AWARD SMALL BUSINESS PROGRAM REREPRESENTATION (APR 2012)

(a) Definitions. As used in this clause--

Long-term contract means a contract of more than five years in duration, including options. However, the term does not include contracts that exceed five years in duration because the period of performance has been extended for a cumulative period not to exceed six months under the clause at 52.217-8, Option to Extend Services, or other appropriate authority.

Small business concern means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR part 121 and the size standard in paragraph (c) of this clause. Such a concern is ``not dominant in its field of operation'' when it does not exercise a controlling or major influence on a national basis in a kind of business activity in which a number of business concerns are primarily engaged.
