PERFORMANCE WORK

STATEMENT

FOR

Water Line Leak Detection Survey

PREPARED BY:

1. DESCRIPTION OF SERVICES:

1.1. Scope: The Contractor shall provide all labor, tools, equipment, hoses, gauges, test equip-ment, hearing protection, eye protection including prescription safety glasses, materials, sup-plies and parts, transportation and other incidentals necessary to travel to Hanscom AFB, Bed-ford, MA, & 4th Cliff Recreation Area, Humarock, MA to provide a a water line leak detection survey for Hanscom AFB & 4th Cliff Recreation Area, in accordance with all applicable Na-tional, Air Force, State and local codes and regulations. The Contractor shall be responsible for ensuring all services and parts provided under this contract are in accordance with the terms and conditions outlined herein and conform to the manufacturers’ equipment specifications and this Performance Work Statement (PWS).

1.1.1 The contractor shall provide supporting documentation showing a minimum of 5 years experience, within the past 10 years, proving past technical capability as a Water Leak Detec-tion Contractor who has provided and coordinated work in the course of a project of this mag-nitude.

1.2. Requested Service: The Contractor shall conduct the following:

1.2.1 Kick off meeting to review scope, procedures, schedule and Draft Report. The contractor shall explain their procedures and equipment, provide a proposed schedule and a proposed Draft

Report.

1.2.1 Water Line Leak Detection Survey.

1.2.1.1 1. Survey the potable water distribution system for leaks, on Hanscom AFB, as out-lined in Attachment #1, to include all fire and building laterals and all valves. Leak survey to also include FamCamp, shown on Attachment #1 (upper page) in quadrants BB to DD and 28 to 32. The Base Housing area, on Attachment #1, shown at the bottom of the page as tightly clustered buildings is not part of the scope of this survey. The Hanscom AFB leak survey to stop at the four water meters that isolate Base Housing.

1.2.1.2. Survey the potable water distribution system for leaks, on 4th Cliff Recreational Area, as outlined in Attachment #2, to include all fire and building laterals and all valves.

1.2.1.3 All leaks detected to be reported to Utilities Systems Repair Operations Supervisor, Joe

Philbrick, Tel. 781-656-3803, at the end of each survey day. The Contractor shall physically mark the location of all leaks, at the time the leak is identified, and record the GPS coordinates.

In grass areas the Contractor shall drive a 24” wooden stake, painted pink, at the location of the leak. On hard surfaces, the Contractor shall paint a pink “X” at the leak location.

1.2.2 In the event where specific parts of the water distribution system (pipe lines, valves, hydrants, etc.) cannot be located, the contractor shall contact the Utilities Systems Repair

Operations Supervisor Joe Philbrick, at 781-656-3803, for assistance in locating. If the Utilities

Shop is able to locate the item, it shall be included in the survey. Annotated reports/lists indicating an item could not be located will only be acceptable if the Utility Shop concurs.

1.2.3. Water Line Leak Detection Report.

1.2.3.1. Provide a narrative report that outlines the process, procedure, and equipment used to perform the survey. As an appendix to the report include calibration certificates for all equip-ment used to detect leaks. The report shall include:

a. Summary table that lists the location and magnitude of any leaks detected.

b. Provide summary of all sustainment and/or maintenance issues discovered dur-ing the survey and provide suggested improvements and/or repairs to bring the system up to optimal operating condition.

c. Perform a final on-site meeting with Hanscom personnel to explain the results of the survey report.

1.2.3.2. Provide a system distribution map that graphically depicts the location and approxi-mate magnitude of any leaks detected. Submit one hard copy and a writeable CD of the drawing. Base drawings provided in pdf for contractor modification, as Attachment #1 Wa-ter Supply System HAFB (Drawing, July 2017) and Attachment #2 Fourth Cliff Water Util-ity (Drawing, 30 April 2015). All leaks found by this survey shall be accurately located (on the X,Y Coordinate grid) and identified on the Attachment #1 & 2 drawings; submitted as part of the Final Report package. Modify drawings to show as found and not found deviations.

1.2.4 Provide meeting minutes for all project meetings within 72-hours of meeting ajourn-ment.

1.3. DRAWINGS & SPECIFICATIONS:

1.3.1 Specifications are not required as part of this project. Drawings/map requirements see par. 1.2.3.2.

1.4. Quality Assurance: According to the contract’s inspection clause, the Government will evaluate the Contractor’s performance under this contract. The COR is a representative of the

Contracting Officer (CO) and will participate in the administration of this contract. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO. All services to be performed by the Contractor shall be subject to review by the CO or COR. The Contractor shall provide an answer to a submitted Corrective Action Report

(CAR) and Customer Complaint Form from the COR within one (1) working day from receipt.

The Contractor shall provide written response addressing the root cause, corrective action and preventive actions (s).

1.5. Continuation of Essential Services during Crisis: This requirement has been deter-mined not to be essential, and does not require continued support during a crisis as defined in

DFARS Clause 252.237-7023.

1.6. Hours of Operation: The Contractor shall perform work Monday through Friday under this contract during the following hours: 07:00 a.m. until 4:00 p.m., excluding Federal Holi-days. Work required to be performed outside of these hours shall only be done after approval from the Contracting Officer.

1.6.1 Federal Holidays:

New Year’s Day - 1 January

Martin Luther King Day - 3rd Monday in January

Washington’s Birthday - 3rd Monday in February

Memorial Day - last Monday in May

Independence Day - 4 July

Labor Day - 1st Monday in September

Columbus Day - 2nd Monday in October

Veteran’s Day - 11 November

Thanksgiving Day - 4th Thursday in November

Christmas Day - 25 December

If these holidays fall on Saturday, the preceding Friday shall be observed. If these holidays fall on Sunday, the following Monday shall be observed. If a holiday falls on a scheduled service day, the Contractor shall be responsible for rescheduling services for the first day followingthe holiday observance.

1.7. Contractor Personnel: The Contractor shall not employ persons on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security and general well-being or operational mission of the installation and its population. The Contractor shall not employ any person who is an employee of the United States Government if the em-ployment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regula-tions (JER). In addition, the Contractor shall not employ any person who is an employee of the

Department of the Air Force if such employment would be contrary to the policies contained in

AFI 64-106, Air Force Industrial Labor Relations Activities. The Contractor shall be prohibited from employing off-duty COR’s who are surveilling any contracts/subcontracts awarded to the

Contractor. All contract employees depending on particular condition, occasion or place, or job assignment shall require professional, suitable attire. All Contractor employees shall con-duct themselves in a professional, courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor employees in performance of this

PWS shall be the responsibility and prerogative of the contractor; however, the Contractor shall comply with the requirements set forth in this PWS and Hanscom AFB regulations concerning conduct of employees as referenced herein. The Contractor shall employ and maintain a tech-nically trained and experienced work force.

1.8. Environmental Protection Health and Safety on a Government Installation:

1.8.1. In performing work under this contract on a Government installation, the contractor shall comply with federal, state and local regulations regarding environmental, occupational safety and health and the specific health and safety requirements established by this contract.

The contractor shall notify the COR of any injuries that occur to contractor personnel while on the installation. The contractor shall notify the COR of damage to government property or equipment during the execution of the contract.

1.8.2. The contractor shall be knowledgeable of all applicable federal, state and local laws, regulations and requirements regarding environmental protection and occupational safety and health.

1.9. Hazardous Material (HAZMAT): The Contractor shall not store chemicals/HAZMAT on Hanscom AFB. All chemicals/HAZMAT shall be approved prior to being transported on base for use by any contractor. The Contractor shall prepare and submit to the COR, ten (10) calendar days after contract award date, one (1) each, Chemical/Hazardous Material Request

Authorization Form (Air Force Form 3952) and Safety Data Sheets (SDS) for each chemical, material and substance proposed to be used in performance of this PWS. The COR will forward the completed AF Form 3952(s) and SDS to 66 ABW/CEIE for approval. The Contractor shall not transport or use any chemical, material or substance on Hanscom AFB before written ap-proval is provided by 66 ABW/CEIE. Contractor shall maintain copies of SDS(s) for all chem-icals. The SDS(s) shall correspond with the manufacturers of the chemicals on hand. Manu-facturer, material, and SDS shall match. NOTE: SDS refers to manufacturer SDS. Once ma-terial is approved for use, contractor shall contact base Environmental Management (EM) (66

ABW/CEIE) and provide a list of all material. 66 ABW/CEIE shall determine any additional material requirements based on the hazard code of the material.

1.10. Hazardous Waste: Contractor shall dispose of any spent/processed chemical solutions and used rags in waste drums located in the shop/area where the work is being performed.

1.11 RESERVED

1.12. Spills: Upon occurrence of any spills that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident.

When possible, the contractor shall use a base telephone to receive help from Hanscom AFB

911. If using a cell phone, Contractor shall request Hanscom AFB 911. The Contractor shall report all environmental violations to the COR. The Contractor shall submit a typed or written report about the incident within four (4) clock hours of the occurrence of said incident. The

Contractor shall provide five (5) copies of the written report. The Contractor shall provide one copy to the CO, two copies to the COR, a copy for the Unit Environment Coordinator (UEC) and a copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information.

1.) Name and contact number of person reporting the spill

2.) Date and time of incident

3.) Location and source of spill

4.) Substance or pollutant spilled

5.) Amount spilled and the rate of discharge

6.) Any damages or injuries involved

7.) Extent of the area impacted

8.) Potential hazards

9.) Action taken

10.) Organizations contacted

11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equipment) and whether 911 was called and which agencies responded.

1.13 Housekeeping: The Contractor shall keep the work areas clean and neat in accordance with Industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this PWS.

1.14. Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Hanscom AFB per AFI 31-204 (in its entirety), Motor Vehicle Traffic supervision.

1.15. Visitor Pass: The Contractor shall comply with requirement and procedures IAW FAR

5353.242-9000, Contractor Access to Air Force Installations. Prior to coming to Hanscom AFB to perform any work, Contractor shall contact the Civil Engineering Government Representa-tive to coordinate access to the base as well as to the building (s) where the work shall be performed. A list of the names of the workers shall be required to gain access, and passes shall be distributed at the visitor’s building at Vandenberg Gate. Workers shall have proof of identi-fication as well as a valid driver’s license. If bringing a vehicle on base, the Contractor will need proof of registration and insurance of that vehicle. Contractor employees must have iden-tification that readily identifies them as contractors. During the course of the contract, certain areas/sites may be restricted or closed to Contractor personnel. The Contractor shall comply with all requirements for entry. In addition, unscheduled gate closures may occur at any time and personnel entering or exiting the installation may experience delays.

In order to maintain accountability once gaining access onto Hanscom AFB, Contractor shall immediately sign the logs at Building 1810, indicating time and date of arrival. Before depar-ture, Contractor shall again sign the logs indicating the departure time, as well as the work performed and the buildings worked in. A copy of a signed work receipt on Contractor’s letter-head detailing work performed shall also be given to the Civil Engineering Government Rep-resentative or left in the log book prior to departure

1.16. Foreign National(s): The Contractor shall notify the COR before sending a Foreign

National representative to perform services. Special rules apply for foreign national(s) visiting a US Government facility.

1.17. Health and Safety on Government Installation: In performing work under this con-tract on a Government installation, the Contractor shall comply with federal, state and local regulations regarding occupational safety and health and the specific health and safety require-ments established by this contract. The Contractor shall notify the COR of any injuries that occur to Contractor personnel while on the installation. The Contractor shall notify the COR of damage to Government property or equipment during the execution of the contract.

1.18. Emergency Procedures: Contractor employees shall follow the direction of Govern-ment employees in regards to emergency evacuation procedures, i.e. fire, tornado, active shooter and bomb threats. It is the Contractors’ responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with government personnel in all fire and tornado drills, and at no additional cost to the government.

1.19. Partnering Agreement: The Contracting Officer may require a partnering agreement between the Government and the Contractor to ensure joint cooperation and a sound partnership of all parties involved in the execution of this contract. Partnering is the creation of a Govern-ment-Contractor relationship that promotes achievement of mutually beneficial goals. It in-volves an agreement in principle to share the risks involved in completing the project and to establish and promote a nurturing partnership environment. Representatives from each organ-ization are encouraged to participate in developing the partnering agreement. Suggested repre-sentation is the Civil Engineer Manager, the Government Inspector, the Government Contract

Administrator, the Contractor’s manager and the Contractor’s quality control person. All costs for the partnership agreement should be shared equally between the Government and Contrac-tor. This group is responsible for developing a formal partnering agreement that should be signed by all parties involved. The agreement should contain as a minimum: specific goals to be reached and a list of objectives to reach the goals, a set of metrics to evaluate the objectives, a frequency for meetings to review the metrics, and a statement of cooperation to execute the terms of the agreement.

SECTION 2

Performance Assessment

The Contractor shall be aware that the absence of any contract requirement from the Service

Summary does not detract from its enforceability nor limit the rights or remedies of the Gov-ernment under any other provision of the contract. The Contractor shall ensure the following service summary requirements are accomplished.

SECTION 3

GOVERNMENT-FURNISHED PROPERTY AND SERVICES

3.1. Government Furnished Property (GFP): Government Furnished Property is not ap-plicable to the in the performance of this contract.

3.2. Reserved.

3.3. Government Furnished Services: Government Furnished Services are applicable dur-ing the performance of this contract.

3.3.1. Security Police: The Government will provide general on-base Security Forces service.

The Security Forces’ telephone number is 911 for emergencies, for crime in progress 781-225-

5613 and 781-225-5613 for non-emergency calls.

3.3.2. Fire and Emergency Services: The Government will provide general on-base fire pro-tection services. The Contractor shall comply with fire prevention rules and regulations en-forced by the Hanscom AFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters and/or calls to dispatch, call 781-225-3330.

Note: Calling 911 from a cell phone will be received by off base emergency 911 systems, causing a delay in response time by emergency personnel. When possible use a base telephone to receive on base help.

3.3.3. Utilities: The Government will furnish utility services. These utilities include natural gas, electricity, water and sewer for the sole purpose of accomplishing this contract. If the

Contractor requires additional utility services, it shall be the Contractor’s responsibility to pro-vide such services. Contractor shall use due care and diligence in efforts to conserve utilities in order to reduce utility costs.

3.3.4. Refuse Collection: The Government will provide refuse collection services limited to the extent provided in the Base Refuse Collection Contract for Government provided facilities.

The Government will only allow non-hazardous refuse to be emptied into the nearest authorized refuse dumpster for trash collection. Government dumpsters shall not be used for collection of

Contractor debris.

3.3.5. Workspace: The Government will provide adequate workspace around the equipment area.
