GROUND MINUTEMAN AUTOMATED TEST SYSTEM
1. Description of Services This Performance-based Work Statement (PWS) is to provide removal, de-installing, moving, and installation support for one customized S-9160 test station in support of the AN/GSM-315A Ground Minuteman Automated Test System (GMATS) program. The instruments in the GMATS test station are obsolete and fragile. The contractor shall be required to move the test station (quantity of 1), interface test adapters (ITAs – estimated quantity, not to exceed 17), mobile work surface (MWS - quantity of 1) and all support equipment from Hill AFB to Offutt AFB, Nebraska. At Offutt, the contractor shall reinstall test station to working order. The contractor shall provide personnel, equipment, tools, materials, supervision, and other items necessary to complete the tasks as defined in the PWS, when not provided by the requesting organization.
2. Basic Services The contractor shall conduct a pre-inspection and acceptance of equipment at Hill AFB and provide the government a copy of the acceptance.
2.1 The contractor shall divide the 4 bay GMATS test station into 2 double bays. The contractor shall package, move, and load the test station, ITAs, MWS and all support equipment from Hill AFB to Offutt AFB, Nebraska and provide mode of transportation. The contractor is responsible for the integrity and any damages of equipment. Two crates for S9100 double bay, plus accessories and air ride truck are required to transfer a $1.5 million test station.
2.1.1 The contractor shall provide all packing, loading, transportation and manpower required for relocating the test station, ITA’s, MWS, and all support equipment from Hill AFB to Offutt AFB, Nebraska
2.2 The contractor shall, with a 48 hour notice for delivery to receiving office, unload and reinstall the test station, ITAs, and all support equipment.
2.2.1 The contractor will perform and must demonstrate the ability to run the Commercial Software Items, P/N 25-93284-6, 25-93284-7, 25-93284-8, 25-93284-9, 25-93284-10, and Teradyne Ghost Image, CPIN 85Q-GSM315A-S001-00A to the relocated test station. (Appendix B)
2.3 The contractor shall conduct a Factory Acceptance Test (FAT) of a fully operational test station, at Offutt AFB, Nebraska and provide the government a copy of the report. This would include as a minimum:

2.4 The contractor shall coordinate with the calibration team, with a 24 hour notice for station calibration when the contractor completed reinstall.
2.5 To verify the quality of a successful installation of the test stand and software, the Government will run a government acceptance test that will consist of successfully running / verifying a unit under test, (UUT).
2.6 During performance of the contract, the contractor shall maintain a clean and orderly work area and shall properly remove and dispose of resultant materials and debris on a continuous basis during the execution of work.
2.7 Upon completion of the work, the contractor shall leave the area neat and clean to the satisfaction of the government representative.
2.7.1 IAW AFI 63-124, para 2.7.1, and DODI 3020.37 this requirement is not considered mission essential.

4. Government Furnished Property and Services
4.1 The contractor is responsible for the S-9160 test station in support of the AN/GSM-315A Ground Minuteman Automated Test System (GMATS) program, interface test adapters (ITAs – estimated quantity, not to exceed 17), mobile work surface (MWS - quantity of 1) and all support equipment from acceptance at Hill AFB, Utah until Governmental acceptance at Offutt AFB, NE.
4.1.1 The contractor shall create an inventory of all actual government property, (Software, Test Stands, ITA’s, MWS, etc), that will be shipped from HAFB to Offutt AFB, and provide a copy to the Contracting Officer (CO) and the Program Manger, (PM) at both HAFB and Offutt AFB. As a minimum, this inventory will include Part number, nomenclature, serial number, and packing box number.
4.2 The contractor shall have access to the necessary facilities, equipment, and property as required for the contract, and any property or equipment provided to the contractor will be returned to the using organization in the same condition as received, other than normal wear and tear after the completion of the contract.
4.3 Government telephones shall be used for official business only
4.4 The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, property, and materials shall be secured.
5. General Information
5.1 Security Requirements
5.1.1 The contractor will not be required to obtain a Controlled Area Badge. The contractor will be escorted at all times within a controlled area.
5.2 Quality Control
5.2.1 The Government Representative (GR) is the authorized individual who will perform assessments of the contractor’s performance.
5.2.2 The GR or alternates will inform the contract manager in person when discrepancies occur and will request corrective action. The GR or alternates will make a notation of the discrepancy on their assessment checklist with the date and time the discrepancy was noted and will request the Contract Manager (CM) or authorized representative, to initial the entry on the checklist.
5.2.3 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO.
5.2.4 The services to be performed by the contractor during the period of this contract shall at all times and places are subject to review by the CO and or authorized GR.
5.3 Quality Control Program
5.3.1 In compliance with the clause entitled “Inspection of Services” FAR 52.246-4, the contractor shall establish a complete Quality Control Program to ensure the requirements of this contract are provided as specified. The contracting officer will notify the contractor of acceptance or required modifications to the plan before the contract start date. The contractor shall make appropriate modifications (at no additional costs to the government) and obtain acceptance of the plan by the contracting officer before the start of the first operational performance period.
5.3.2 The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor's quality control program and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints.
5.4 Hours of Operation
5.4.1 The contractor is responsible for designating work hours for their employees but at a minimum shall be available to support the Government during the following hours of operation:
5.4.1.1 Work performed at Hill AFB will be performed during normal duty hours, 0700 to 1630 Mountain Standard Time (MST), Monday through Friday, except federal holidays.
5.4.1.2 Work performed at Offutt AFB will be performed during normal duty hours, 0700 to 1630 Central Standard Time (CST), Monday through Friday, except federal holidays.
5.4.2 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities on federal holidays. If the holiday falls on Sunday, it is observed on Monday. If the holiday falls on Saturday, it is observed on Friday.
5.5 Conservation of Utilities
5.5.1 The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that prevent the waste of utilities.
5.6 Records
5.6.1 The contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in this PWS. If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request.
5.7 Environmental Controls
5.7.1 Compliance with Laws and Regulations
5.7.1.1 The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the CO pursuant to notice requirements (see FAR 52.243-7, 43.104, 43.106) and negotiate a modification to the contract.
5.7.2 Notification of Environmental Spills
5.7.2.1 If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately report the incident to Hill AFB Fire Dept at 777-1911. The liability for the spill or release of such substances rests solely with the contractor and its agent.
5.7.3 Material Storage and Use
5.7.3.1 The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.
5.8 Government Observations
5.8.1 Government personnel, other than CO and Quality Assurance Evaluators, (QAE), may from time to time, with CO coordination, observe contractor operations. However, these personnel may not interfere with contractor performance.

APPENDIX B
APPLICABLE REGULATIONS, MANUALS AND SPECIFICATIONS
The contractor shall perform in accordance with all such referenced directives, regulations, manuals, pamphlets, instructions, and other guidance as may be revised, supplemented, or amended at the government’s discretion. The contractor shall implement all revisions, supplements or amendments that result in no change in contract price. Any identified increase or decrease in cost performance shall be forwarded to the ACO for approval.

SAFETY, FIRE PROTECTION AND HEALTH SPECIFICATION INDUSTRIAL SAFETY REQUIREMENTS
TYPE WORK: Logistics and Consulting Services for Depot Maintenance and Depot Supply Finance, Material and Production Systems and Processes (309 MXW)

SECTION I - GENERAL REQUIREMENTS
A. Safety Program Requirements.
In context with AFMC Acquisition Reform initiatives, contract language will now focus on ensuring safety objectives are met through contractor risk management rather than the traditional method of providing specific government guidance for risk elimination. With this new “performance-based” language, the government will provide the contractor with areas of concern (Protection of government personnel and property) and the contractor will be free to meet those objectives in a manner that is consistent with corporate standards. The contractor will implement a safety program plan that ensures protection of Government personnel and property. The program will consist of, as a minimum:
1. Mishap reporting, as defined in paragraph B1 below.
2. A Safety Plan and corresponding site specific checklist that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and will be used during the performance of the work described in the contract. The Safety Plan and checklist will be approved by 309 MXW/QPE (Safety Office) prior to commencement of any work described in this contract.
3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.
4. Competent personnel to provide surveillance of the Safety Plan.
5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.
6. All contractor personnel shall be trained and qualified to perform their duties safely.
7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.
B. Mishap Notification
1. The contractor shall notify 309 MXW Safety (Phone (801) 586-4176) who in turn will notify 75 ABW/SEG ((801) 777-3333) or 309 Wing Operations Center ( (801) 777-3238), or the Hill AFB Command Post (777-3007) after normal duty hours, and the designated Government Representative, i.e., the ACO, PCO, or QAE within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to 309 MXW Safety. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.
Mishap notifications shall contain, as a minimum, the following information:
(a) Contract, Contract Number, Name and Title of Person(s) Reporting
(b) Date, Time and exact location of accident/incident
(c) Brief Narrative of accident/incident (Events leading to accident/incident)
(d) Cause of accident/incident, if known
(e) Estimated cost of accident/incident (material and labor to repair/replace)
(f) Nomenclature of equipment and personnel involved in accident/incident
(g) Corrective actions (taken or proposed)
(h) Other pertinent information
2. Ensure government contract lease agreements determine convening authority prior to executing the contract/lease agreements, and that all contracts ensure the following, IAW AFI 91-204, paragraph 2.4.6:
The contractor will support and comply with the safety investigation and report requirements of AFI 91-204
A record of all mishaps involving Air Force resources will be entered into the Air Force reporting system
A record of mishaps involving other DoD resources will be forwarded to the involved agencies
When a mishap involves a contract managed by DCMA, DCMA safety personnel will review the final safety report and send their response to HQ AFSC
3. Ensure contract/lease agreements for aerospace vehicles state: The Air Force is responsible for the investigation of mishaps involving aerospace vehicles. The Air Force is authorized to investigate mishaps involving non-accepted Air Force aerospace vehicles (Non-delivered vehicles for which the government has accepted responsibility; DD Form 250. Material Inspection and Receiving Report, HAS NOT been executed).
4. If requested by authorized government personnel (ACO, PCO, QAE, safety representative, Fire Department, and Security Forces), the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.
C. General Safety Requirements
If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative.
Follow all applicable federal, USAF, state, and local safety directives outlined in Section II below.
Ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels.
Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the 309 MXW Safety Office.
SECTION II – SPECIFIC REQUIREMENTS
The contractor’s prepared Safety Plan shall:
· Demonstrate a management commitment to employee safety and health
· Identify applicable rules and regulations
· Identify the roles and responsibilities of Management, Supervisors, Employees and Safety Coordinator
· Identify work to be performed and location of expected operations
· Provide a description of safety program, safety monitoring responsibilities, organizational structure, and contact information for on-site personnel
· Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures
· Identify employee safety and health training requirements and the documentation process
· Include emergency response plans and procedures that relate to protection of government personnel and property
· Include a workplace inspection frequency, to include the identity of the individual responsible for conducting the inspection
· Include hazard reporting procedures and identify individual(s) responsible for the correcting identified hazards
· Identify first aid and injury procedures
· Identify procedures for accident reporting and investigation
· Identify the process for tracking controlled hazards in contractors work area
· Provide a facility description to include the electrical wiring and fire protection systems (i.e., fire suppression, fire detection, fire extinguishers, fire walls, etc.), required equipment and any other items necessary to fulfill the elements referenced below for the protection of government personnel and property. The contractor’s safety plan shall meet or exceed the referenced standards below.
· The authorized Government Representative (309 MXW Safety Office) shall approve the contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification /Reporting Procedures, which are outlined in Section I, paragraph B of this Appendix C.
