About this file
This document is an Unusual/Significant Incident Report Protocol issued by the Arizona Department of Child Safety (ADJC), specifically outlining reporting procedures for service providers working with juveniles under ADJC jurisdiction. The protocol applies to service providers managing juvenile cases, including both delinquent and dependent youth with assigned ADJC Parole Officers, and establishes standardized communication requirements for reporting critical incidents.
The protocol details specific reporting mechanisms for incidents ranging from serious injuries and sexual assault to law enforcement interactions, substance use, and transportation-related events. Service providers are mandated to make verbal reports within two hours of an incident during regular business hours (8:00 AM - 5:00 PM, Monday-Friday) to the assigned Parole Officer and Community Services Office, or to the ADJC After Hours Administrator during non-standard times. Written reports must be submitted within 24 hours via fax or email, including comprehensive details such as juvenile identification, incident description, staff information, and any follow-up actions required. The protocol provides specific contact numbers, reporting scripts, and defines 17 distinct incident categories that necessitate immediate notification.
