About this file

This document is an Exhibit detailing an Unusual/Significant Incident Report Protocol for the Arizona Department of Child Safety (ADJC), specifically outlining service providers' responsibilities for reporting incidents involving juveniles under ADJC jurisdiction. The protocol applies to service providers working with ADJC juveniles, including dual wards who are both adjudicated delinquent and dependent, and establishes clear notification procedures for the department's Community Services Office.

The document defines a comprehensive set of reportable incidents, ranging from serious events like death or injury to less critical but still significant occurrences such as substance use, law enforcement contact, or transportation-related issues. Service providers are required to make verbal reports within two hours of an incident during both regular business hours (8:00 AM - 5:00 PM, Monday-Friday) and after-hours (5:01 PM - 7:59 AM weekdays, and all weekend/holiday hours), followed by a detailed written report within 24 hours. The protocol specifies precise communication channels, including specific phone numbers for the Community Services Office and After-Hours Administrator, and requires comprehensive documentation including juvenile details, incident description, staff information, and any follow-up actions.
