About this file

This document is an Exhibit from the Arizona Department of Child Safety (ADJC) that outlines a service provider incident reporting protocol. The document specifically details the responsibilities and procedures for service providers working with ADJC juveniles when unusual or significant incidents occur. The protocol applies to ADJC juveniles and dual wards (juveniles who are both adjudicated delinquent and dependent) under the jurisdiction of the Arizona Department of Child Safety and with an assigned ADJC Parole Officer.

The protocol establishes comprehensive reporting requirements for service providers, mandating verbal notifications within 2 hours of an incident during both regular business hours (8:00 AM - 5:00 PM, Monday-Friday) and after hours (5:01 PM - 7:59 AM weekdays, all weekend and holiday hours). Service providers must submit written reports within 24 hours, detailing specific incident information such as juvenile identification, incident description, environmental factors, and follow-up actions. The protocol covers 17 types of reportable incidents, including but not limited to death/serious injury, use of restraint, sexual assault, law enforcement contact, AWOL, substance use, and other events with safety, programmatic, or contractual implications.
