About this file
The document is a Personal Allowance Ledger form issued by the Arizona Department of Child Safety for use in congregate care facilities. It is designed to track and document personal allowances and purchases made for children in out-of-home care, with a specific focus on maintaining a detailed record of financial transactions involving youth placed in licensed agencies and operating facilities.
The ledger provides a structured template for recording each financial transaction, including the date, description of distribution or purchase, payment method, amount, and requiring signatures from both the child (if 12 years or older) and staff member. The form mandates that all personal allowances, emergency money, special needs money, graduation money, school supplies, and fees must be documented, with receipts to be kept with the log for one year. The purpose is to ensure transparency, accountability, and proper financial management of funds allocated to children in congregate care settings.
