About this file
This document is a Contract Amendment (#4) issued by the Arizona Department of Child Safety's Office of Procurement, located in Phoenix, Arizona. The amendment is associated with Contract #CTR071450 and is designed to modify specific sections of the original contract's scope of work.
The amendment focuses on two primary modifications to the contractor's requirements: first, establishing drug testing protocols that include a clear pre-employment drug test and quarterly random drug testing aligned with the ADCS Drug Testing Policy; and second, updating background check and screening procedures for all paid and unpaid adults and subcontractors. The new requirements mandate obtaining a Level 1 fingerprint card, Central Registry Clearance, and potential Interstate Central Registry background check prior to hiring. Additionally, each adult must complete a Background Check Authorization form and be entered into the Department's electronic database before being hired. The amendment will take effect upon execution in the Arizona Procurement Portal or by April 1, 2025, whichever occurs earlier.
