About this file
The document is an Exhibit M Program Performance Report for the Arizona Department of Economic Security (DES) Emergency Solutions Grant (ESG) Program. It provides a quarterly and annual reporting template for homeless service providers operating in the Maricopa (MAR), Tucson Pima Collaborative Homeless (TPCH), and Balance of State (BOS) Continuum of Care (CoC) regions. The report is designed to track progress, identify barriers, and assess solutions for homeless service programs across different activities such as Emergency Shelter, Street Outreach, Rapid Re-Housing, and Homeless Prevention.
The performance report requires providers to submit detailed metrics including persons served, bed utilization, income exit rates, destination outcomes, data quality, length of participation, and grant expenditure percentages. The template includes performance goals for each service category, with specific targets such as 60-90% for various performance indicators. Providers must complete the report quarterly and annually, using data extracted from their HMIS (Homeless Management Information System) or comparable databases, with the primary goal of helping the DES State Homeless Coordination Office monitor and support program effectiveness across different homeless service interventions.
