About this file

This document is a Contractor/Consultant Satisfaction Form and evaluation program established by the City of Toledo, Ohio to assess the performance of contractors and consultants providing services to the municipality. The program aims to evaluate performance, increase communication, and foster positive relationships while ensuring contractors and consultants meet the City's expectations for timely, cost-effective, and quality service delivery to citizens. The evaluation form is included in all bid packets and requests for proposals, and is discussed at pre-bid and pre-construction meetings. Completed evaluations must be submitted by the employee directly responsible for contract oversight within 30 days of project completion, followed by a post-contract meeting with the contractor or consultant to review the results if necessary.

The satisfaction program evaluates contractors and consultants across four primary categories: Organization and Management (12 criteria, 140 maximum points), Equipment (2 criteria, 20 maximum points), Work Performance (9 criteria, 160 maximum points), and Subcontractor Management (5 criteria, 50 maximum points), for a total possible score of 370 points. Evaluation criteria include management availability and competence, timely documentation submission, wage compliance, work quality, schedule adherence, budget compliance, safety and cleanliness standards, traffic control maintenance, equipment reliability, cooperation with other contractors and agencies, and Disadvantaged Business Enterprise utilization. Individual ratings range from 1-10 points with weighted factors applied to certain criteria; ratings below 10 require supporting documentation such as inspector reports. Evaluation files are maintained by each City Division and placed on a public database accessible to all divisions for use in future bid award decisions. The program became effective May 24, 2002.
