About this file
This is a Proposer's Disclosure Statement form issued by the Board of Regents (BOR), a public agency in Georgia, for use in the state and local contract opportunity selection process. The document requires all proposers to submit a completed disclosure statement along with their proposal to ensure transparency and mitigate conflicts of interest. Proposers must respond to three specific disclosure questions: first, describing any business transactions between their firm and BOR, the using agency, or the project end-user that occurred within the prior two years; second, disclosing any gifts, hospitality, or benefits provided by the firm to these entities within the prior one-year period; and third, identifying any actual or potential conflicts of interest based on a definition that encompasses any actions or decisions that could result in private monetary or financial benefit or detriment to the firm, its relatives, or associated businesses, as viewed from the perspective of a reasonable person with knowledge of relevant facts.
The disclosure statement must be dated and signed by an authorized signatory of the proposer and submitted as a deliverable with the proposal submittal. The form serves as a compliance mechanism to fulfill Georgia state laws, rules, and executive orders governing public procurement, with the stated objective of avoiding the appearance of impropriety and conflicts of interest during both the selection process and subsequent contract negotiation and performance phases.

Exhibit G Proposer’s Disclosure Statement
All proposers should be aware that the project you are submitting a proposal on is a public project, and the Owner (BOR) is a public agency. Pursuant to the laws, rules and Executive Orders of the State of Georgia, BOR shall make every effort to avoid even the appearance of a conflict of interest or any impropriety in both the selection process for this project and the negotiation and performance of any resulting contract. As part of any submittal you intend to make for this project, you must include a Disclosure Statement with your submittal which answers the following specific questions:
1. Describe any business transactions occurring within the prior two years between your firm and BOR, the Using Agency, or the ultimate end-user of the proposed project.
