About this file

This is an income verification calculation worksheet developed by the Arizona Department of Economic Security (DES) for use in the Rapid Rehousing (RRH) program serving youth and families. The document establishes standardized procedures for determining client eligibility based on income requirements and serves as the official form for documenting income verification in client files.

The worksheet requires income certification to be completed after one year of financial assistance, with household income maintained at or below 50% of the Area Median Income (AMI) for the applicable jurisdiction. Eligibility is determined using HUD income limits in effect at the time of verification rather than limits corresponding to the grant year. Income calculations are derived from third-party documentation or client self-certification and must be computed using the HUD CPD Income Calculator, with results printed and filed with the client documentation. The form includes a Declaration of No Income section requiring clients to attest under penalty of perjury that they have received no income from any source, including employment, benefits, assets, or assistance, and must provide written verification from the income source for any bills or expenses paid on their behalf. Total annual income, including income from assets, is calculated and compared against 50% of AMI thresholds to determine continued program eligibility.
