About this file

This is a Price Schedule exhibit (Exhibit D) for a formal Request for Proposal issued by the State of Minnesota for Pressure Washers, De-Icing Systems, and Vegetation Sprayers. The solicitation seeks qualified vendors to establish one or more indefinite quantity contracts to supply equipment and related services, including accessory installation, repair services, and rental options. The resulting contracts will be available for use by State agencies and Cooperative Purchasing Venture (CPV) members on an as-needed basis with no guaranteed minimum purchase quantities. The State reserves the right to award multiple contractors and to review alternative contracts such as NASPO agreements to determine if participating in those contracts better serves the State's interests. Prospective responders must submit questions in writing to paul.thomas@state.mn.us no later than October 7, 2026, at 3:00 PM Central Time, with responses provided via addendum. Responses to the solicitation must be submitted electronically through the Supplier Portal by the specified deadline, with non-electronic and email submissions rejected.

The price schedule exhibit establishes the pricing structure and terms under which vendors must submit cost proposals for the equipment categories specified in Exhibit C of the Sample Contract. Pricing terms, any volume discounts, set-asides for small businesses or other categories, incumbent vendor considerations, and other financial conditions are detailed within this exhibit. The State's use of this contract structure allows flexibility to leverage competitive pricing while maintaining the option to utilize alternative cooperative purchasing agreements if determined advantageous to state operations and budget management.

Instructions

Overview: STEP 1: Review this file's second tab titled: "Purchase- % Discount Off List". Responder instructions are below for each section found on this tab.
Section No.Responder Instructions
1.1.1Enter Equipment Manufacturer Name.
1.1.2Enter the name of the catalog or price list included with your response. Use this same name on the catalog or price list you submit separately. If you are submitting multiple catalogs or pricelists, create a new, separate tab for each one by copying the “Purchase – % Discount Off List” tab before filling it out. For clarity, you must have one unique ‘Purchase – % Discount Off List’ tab corresponding to each catalog or pricelist included in your response.
1.1.3Enter an identification date for each Catalog/Pricelist included in your response. Use this same date on the Catalog/Pricelist(s) you submit separately.
1.2The discount you enter in this section will be applied to all items listed in the Catalog/Pricelist you identify under Section 1.1.2. If your Catalog/Pricelist uses different discount percentages (%) for different items within the same pricelist/catalog, you must list each distinct discount in this section and what items it applies to (e.g., base equipment, parts, accessories, etc).

Next, complete the following step ONLY if offering multiple discounts in the same Catalog/Pricelist: Go through your entire Catalog/Pricelist and include, next to each line item:

1) MSRP/List Price

2) Discount (%)

3) Net (Contract) Price

Tip: If your catalog or pricelist includes multiple discount structures, it may be easier to separate the products into individual catalog(s) or pricelist(s) based on the applicable discount percentage. This approach helps you avoid having to list both the applicable discount and the resulting net price next to each item within the catalog or pricelist.

Reminder #1: If multiple discounts apply within the SAME pricelist/catalog, the responder must ensure that every item listed within the catalog or pricelist clearly reflects both the applicable discount and the resulting net price. Alternatively, if your catalog or pricelist uses a single uniform discount percentage across all items in the submitted pricelist or catalog, this step is not required.

Reminder #2: If you are submitting multiple catalogs/pricelists in your response, you must complete a separate PURCHASE – % DISCOUNT OFF LIST tab for each catalog/pricelist provided. You may do this by copying the template tab onto additional tabs within this workbook before entering the required information.

Ensure that all catalogs/pricelists are included in your submission. Failure to provide applicable catalog/pricelist(s) may result in rejection of your response.
Example of Applying the Percentage Discount to Catalog/Pricelist: Equipment Item 1 is priced in the provided corresponding Catalog/Pricelist at $300.00. Responder offers a percentage discount in Section 1.2 of 3.5%. Calculation to find the State Contract Price would be $300.00 - (300 x 0.035) = $289.50. Offered State Contract Price is $289.50.
1.3Enter the Standard Equipment Warranty Information.
1.4.1Enter the name of the City where the equipment will ship from (starting point). If multiple, identify as such or add all.
1.4.2Enter the State (if applicable) where the equipment will ship from (starting point). If multiple, identify as such or add all.
1.4.3Enter the Zip Code (if applicable) where the equipment will ship from (starting point). If multiple, identify as such or add all.
1.4.4Enter the Price Per Loaded Mile offered. See Exhibit D, Section 6.1 in the Sample Contract before entering a response. If not charging for delivery/shipping/transportation; enter "$0.00". DO NOT LEAVE BLANK.
1.4.5If there is a minimum delivery charge when using price per loaded mile; you may enter it here. If no minimum, leave blank or enter "$0.00".
Part and Accessory Installation may be requested on an as‑needed basis. If offering, but at no charge, enter $0.00. If you do not offer this service, you may leave blank. The Percentage % Discount entered into Section 1.1.2 does not apply to this offered rate.
1.5.1Enter Labor Rate for Part and Accessory Installation (if offered, $___/hour).
Inspection, diagnostics and repair services may be requested on an as‑needed basis. Labor rate only (excludes repair parts). If you do not offer this service, you may leave blank. The Percentage % Discount entered into Section 1.1.2 does not apply to this offered rate.
1.6.1Enter Labor Rate for Inspection, Diagnostics, and Repairs ($__/hour).
Overview: STEP 2: Review this file's third tab titled: "RENTAL". Use this tab to enter any equipment you are offering on a rental basis (if applicable). If you are not offering rental equipment, leave this tab blank. For clarity, the rates you list on this tab will be considered your final proposed rental rates for the equipment entered. For avoidance of doubt, the discount off percentage from the PUCHASE- % DISCOUNT OFF LIST tab (Secton 1.1.2) does not apply to any rates entered here.
