About this file
This is a Statement of Work (Exhibit B) for a contract with the Colorado Department of Public Health and Environment (CDPHE) to expand produce prescription programs across rural Colorado. The project aims to establish sustainable operational infrastructure for scaling produce prescription programs through funding for dedicated staff and regional expansion efforts, with a specific focus on diet-related chronic diseases including prediabetes, diabetes, cancer, and cardiovascular disease. The contractor must identify and partner with a minimum of four local food banks in rural Colorado counties, execute signed agreements with each entity, and hire full-time staff to manage fund distribution. Primary deliverables include signed food bank agreements, needs assessments for each contracted food bank, lists of purchased equipment and hired personnel, quarterly progress reports, and a final program impact report, with specific completion dates to be determined during contract execution. The contractor is responsible for conducting needs assessments covering infrastructure, equipment, personnel, and technical assistance requirements; coordinating acquisition of cold storage and food preparation equipment; verifying collaboration between food banks and community-based organizations; and ensuring nutrition education delivery to program participants on food storage, preparation, and safe handling practices.
The contract specifies that no grant funds will be used for direct food procurement, with capital budgets strictly allocated toward cold storage, food preparation, delivery equipment, and infrastructure. CDPHE will review and approve the list of identified local food banks prior to implementation and will provide the contractor with quarterly progress report templates no later than 30 business days from contract execution. The contractor must submit all deliverables via email to the designated CDPHE position, with CDPHE providing feedback within a specified number of business days. Monitoring will be conducted throughout the contract period through review of progress reports, fiscal documentation, and programmatic documentation, with a final contractor performance evaluation at contract conclusion. Non-compliance issues will trigger a written notification and collaborative resolution process between the contractor and the State, with potential exercise of contractual remedies if the contractor demonstrates inaction or disregard for compliance resolution plans.

STATEMENT OF WORK
To Original Contract Number: XXX
These provisions are to be read and interpreted in conjunction with the provisions of the contract specified above.
I. Entity Name: xxx ENTITY NAME xxx
II. Project Description:
This project serves to expand produce prescription programs across rural Colorado by awarding contracts to qualified entities that can support local food banks and reduce the statewide burden of chronic disease. The project focuses specifically on diet-related conditions, including prediabetes, diabetes, cancer, and cardiovascular disease. Chronic diseases are one of the leading causes of death in the United States and are caused by a few key risk behaviors, including poor nutrition. In Colorado, chronic diseases account for more than 55% of all annual deaths and rank among the top ten leading causes of death. Today, 66.8% of Colorado adults live with at least one chronic condition, and nearly 40% manage multiple comorbidities.
The goal of this health project is to establish the sustainable operational infrastructure required to scale produce prescription program interventions across rural Colorado. To achieve this goal, funding will be strategically allocated to hire dedicated staff to manage and expand these regional produce prescription programs. Furthermore, funding will provide the vital resources needed to strengthen clinical partnerships, scale patient outreach, and enhance the capacity of community-based organizations to distribute fresh food. To maximize long-term resource efficiency, the capital budget is strictly allocated toward cold storage and food preparation and delivery equipment and infrastructure; no grant funds will be used for direct food procurement.
