About this file

This document is Exhibit C, an Unusual/Significant Incident Report Protocol from the Arizona Department of Child Safety (ADJC), specifically from the Office of Procurement located in Phoenix, Arizona. The protocol outlines reporting requirements for service providers working with juveniles under ADJC jurisdiction, including those receiving services funded by ADJC, Department of Child Safety (DCS), Tribal/Regional Behavioral Health Authorities (T/RBHA), inter-state, or private pay programs.

The document details comprehensive reporting procedures for service providers when unusual or significant incidents occur involving ADJC juveniles. Providers are required to make verbal reports within two hours of an incident during business hours (8:00 AM - 5:00 PM Monday-Friday) to the assigned Parole Officer and Community Services Office, or during after-hours (5:01 PM - 7:59 AM weekdays, all weekend and holiday hours) to the ADJC After Hours Administrator. Written incident reports must be submitted within 24 hours via fax or email, and must include specific details such as juvenile information, incident description, environmental factors, triggers, resolution, and any follow-up actions. The protocol defines 17 types of reportable incidents, ranging from serious injuries and restraint use to law enforcement contacts, substance use, and other potentially newsworthy or sensitive events.
