This Statement of Work (SOW) template outlines the requirements for the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program administered by the Colorado Department of Early Childhood (CDEC) in partnership with the Health Resources and Services Administration (HRSA) and the Administration for Children and Families (ACF). The program requires contractors to deliver Nurse-Family Partnership (NFP) services to a minimum caseload of families throughout Colorado during the performance period of October 1, 2026, through September 30, 2027. Key deliverables include recruitment and retention of MIECHV-eligible families with at-risk characteristics, provision of voluntary home visits aligned with NFP model fidelity, implementation of Continuous Quality Improvement (CQI) initiatives, collection and reporting of performance data through specified platforms including the FLO Database and monthly CDEC report templates, and submission of MIECHV performance benchmarks biannually and quarterly as required by the State Intermediary (Invest in Kids), CDEC, and HRSA. Contractors must maintain qualified staffing, ensure all Nurse Home Visitors hold current RN licenses and CPR/BLS certifications, complete mandatory NFP training, participate in weekly reflective supervision and joint visit observations, and comply with all federal and state requirements including HIPAA compliance, record retention for three years, and submission of informed participant consent forms from all clients.

Contractor reimbursement is provided on a monthly basis through the OnBase invoice submission platform, with invoices due 15 days after the end of each month and final invoices permitted up to 45 days following the performance period. Contractors must adhere to 2 CFR 200.334 record retention requirements and maintain comprehensive fiscal, administrative, and programmatic policies and procedures. MIECHV funds are restricted and cannot be used for direct medical, dental, or legal services, employee incentives, entertainment, gifts, or employee leave payouts. Eligible families for enrollment include pregnant women and birthing people under age 21, families with histories of child abuse or neglect, families with substance abuse histories or needs, families with tobacco users in the home, families with children demonstrating low student achievement or developmental delays, and families with current or former Armed Forces members.
