About this file
This is an Exhibit A data collection, reporting, and evaluation requirements document for the Tuolumne County Probation Department's Proposition 47 Cohort 5 initiative, which establishes minimum standards for all subcontracted Community-Based Organizations (CBOs) receiving grant funds. The document outlines comprehensive participant-level data collection requirements, including enrollment and demographic information, service delivery details, and participant outcomes such as housing and employment status at entry and exit. CBOs must collect and maintain data for all individuals meeting the Proposition 47 target population requirements, specifically those involved in the criminal justice system with histories of mental health and/or substance use disorders. Contractors are required to submit participant-level data and service summaries to the Lead Agency no less than quarterly to support Quarterly Progress Reports, Local Evaluation Plan reporting, and reimbursement documentation on a reimbursement basis.
The document establishes that the Tuolumne County Probation Department, as the Lead Agency, shall be responsible for collecting and reporting recidivism data using official criminal justice records obtained from local law enforcement or probation sources. CBOs must maintain secure data systems and comply with all applicable confidentiality laws, including HIPAA and 42 CFR Part 2, while retaining all financial and programmatic records in accordance with BSCC Grant Administration Guide requirements and applicable state and federal law. Contractors must participate in data review meetings and cooperate with evaluation activities, and failure to comply with data collection and reporting requirements may result in withholding of reimbursement, corrective action, contract modification, or termination.
