About this file

This file is a required forms and certifications document for a contract opportunity with Menard County, Texas for Roof Assessment, Design, Construction Documents, Procurement Assistance, and Construction Administration Services. The document serves as the vendor information and offer statement section of the bid submission package, requiring contractors to provide company legal name, business address, tax identification number, SAM.gov UEID number, and contact information for both the company contact person and authorized representative. Vendors must acknowledge receipt and understanding of all solicitation documents, addenda, and amendments, and certify compliance with applicable contractual requirements and terms and conditions.

The certification section requires contractors to verify compliance with numerous federal, state, and local regulations applicable to Texas governmental contracts. These include debarment and suspension certifications under federal standards (2 CFR Part 180 and 200.214), conflict of interest disclosures, and compliance with Texas-specific requirements including House Bill 89 (Israel boycott prohibition), firearm trade association discrimination prohibitions, energy company boycott prohibitions, child support obligation verification, and critical infrastructure foreign ownership restrictions. For contracts valued at $100,000 or more with companies employing ten or more full-time employees, additional certifications regarding Israel boycott, firearm discrimination, and energy company boycott are required. Contractors must also obtain debarment and suspension certifications from any subcontractors or lower-tier participants and maintain copies for county review. All certifications constitute material representations of fact, and false certifications may result in contract termination, pursuit of legal remedies, and referral to appropriate federal and state authorities.
