The document outlines compliance requirements under Texas Family Code Section 231.006, which stipulates that entities can be deemed ineligible for state payments if they have unresolved child support obligations. Respondents must either complete the ownership information section or check a box indicating the schedule is not applicable. A federal privacy act notice is included, explaining that social security numbers will be used to identify potential child support issues and will be kept confidential.
