Section G- Site Map (Part 3.5.)
Attach the required SWPPP site maps or plans to this SWPPP Form. The site maps or plans must clearly indicate all the required information in Part 3.5. of the General Permit. This means SWPPP site maps must be of sufficient size, scale, and legibility. This cannot be accomplished on standard USGS topographic maps like those required to be attached to the NOi Form. For most construction projects, in order to accommodate all required inclusions, a series of site maps is necessary and recommended as this information cannot be legibly and completely shown on one map. One map could show the undeveloped site and its current features. An additional map or maps can be developed to show the developed site or, for more complicated sites, show the major phases of development. All information on the attached SWPPP site maps must correlate with the information and identifying names provided on this SWPPP Form, such as identifying "Outfall 001" by that name on the site map. ·
Section H - Identification and Summary of Potential Pollutant Sources (Part 3.6.)

6. Outdoor storage activities (building materials, fertilizers, chemicals, etc.).
Npt Applicable
7. Significant dust or particulate generation.
Control measure: Sprinkling Implementation: Apply at a rate of 3 gallons per acre so that the soil is wet but not saturated or muddy and so that no dust is being generated.
Maintenance requirements: Dust control requires constant attention: it is not a one-time or once-in- awhile activity. Dust control sprinkling may have to be done several times a day during hot, dry weather.
8. Vehicle and equipment maintenance and fueling.
On-site fueling areas will have a spill kit present, visible, and maintained. Employees and subcontractors will receive training regarding equipment/vehicle fueling and maintenance practices, including proper fueling procedures, proper use of pumps, emergency shutoff valves.
9. Routine maintenance activities involving fuels, oils, solvents, pesticides, herbicides, fertilizers, detergents, etc.
Designate vehicle/equipment service will be marked and the areas will be located away from drainage facilities and watercourses to prevent stormwater run-on and runoff. Designated washout areas will be shown on the SWPPP plan sheets .
... u. On-site waste management practices (waste piles, liquid wastes. dumpsters, etc.).
• Designated trash and bulk waste-collection areas on-site (location of areas are shown and updated on plan sheets. • Litter and debris from the construction will be cleaned up on a daily basis. Waste-collection areas will be located away from streets, gutters, watercourses, and storm drains. Secondary containment around waste collection areas will be implemented if needed to further minimize the likelihood of contaminated discharges. Portable restrooms will have tie downs or stake downs in areas of high winds or susceptibility to vandalism.
11. Concrete truck and equipment washing.
Equipment/vehicle washing and allowable non-stormwater will be controlled using procedures and practices include using off-site facilities; washing in designated, contained areas only; eliminating discharges to the storm drain by infiltrating the wash water or routing to the sanitary sewer. The locations of equipment/vehicle washing areas, storm drains, tributary drainage areas, and surface waterbodies on or near the site are shown and updated on the SWPPP plan sheets
12. Dedicated asphalt and concrete batch plants.
Concrete trucks or equipment will not be washed out into storm drains, streets, gutters, uncontained areas, or any watercourses. Washout areas will be established prior to commencing the activity and their locations advertise with signs or delineators. Materials will be disposed of properly {for example, allow the water to evaporate and to recycle the hardened concrete).• Employees and subcontractors will receive training regarding designated washout areas, including proper handling and storage practices.
13. Non-industrial waste sources such as worker trash and portable toilets.
Portable restrooms will be located away from streets, gutters, watercourses, and storm drains (location of areas are shown i updated on plan sheets). Secondary containment around waste collection areas will be implemented if needed to iher minimize the likelihood of contaminated discharges.

2. Describe all BMPs which will be used at the construction project based on Part 3. 7, and Part 2.1, of the General Permit. This description can generally address a type ofBMP ifthat BMP is to be used more than- -once, but the specific BMP and their respective locations must still be provided on the Construction Phasi Sheets provided in Section E of this SWPPP Form, and on the SWPPP site maps and/or plans provided in Section G of this SWPPP Form. All potential pollutant sources in Section H must be addressed with corresponding BMPs in this item regardless of whether the BMP description is dependent on a reference source listed in Section 1.1.
Control measure: Silt Fence Implementation: The silt fence shall be functional before other land disturbing activities take place. Silt fence should be functional throughout the duration of construction and for a long enough duration following construction to allow disturbed areas to stabilize following revegetation and seeding.
Maintenance requirements: Silt fences should be inspected periodically for damage (such as tearing by wind, animals, or equipment) and for the amount of sediment that has accumulated. Remove the sediment when it reaches one-half the height of the silt fence. In situations where access is available, machinery can be used. Otherwise, the silt should be removed manually. The following are key elements to remember: The sediment deposits should be removed when heavy rain or high water is anticipated. The sediment deposits should be placed in an area where there is little danger of erosion.
The silt fence should not be removed until adequate vegetative growth ensures no further erosion of the slopes. Generally, the fabric is cut at ground level, the wire and posts are removed, then the sediment is spread, seeded, and protected (mulched) immediately.
Control measure: Fiber Roll Implementation: The Fiber Rolls shall be functional before other land disturbing activities take place. Fiber Rolls should be functional throughout the duration of construction and for a long enough duration following construction to allow disturbed areas to stabilize following revegetation and seeding.
Maintenance requirements: Remove sediment before it has accumulated to one-half of the above~ground height of the straw wattles. It is the Contractor's responsibility to upgrade erosion control measures as necessary if the sediment controls are ineffective and turbid water is observed discharging from the site.

Final stabilization means:
•All ground disturbing activities at the site have been completed, AND
• Vegetation has been established that provides a uniform perennial vegetative cover with a density of 70% of the natural vegetative cover prior to construction.
•In arid (0-10" annual precipitation) or semiarid (10-20" annual precipitation) areas, final stabilization is considered to be achieved when the area is seeded or planted and vegetation is expected to provide 70% cover within 3 years; AND, erosion controls are in place that provide soil protection for at least 3 years without active maintenance. Document arid/ semi-arid conditions in SWPPP
• Effective non-vegetative stabilization is in-place such as rip rap, gabions, geotextiles, rock, aggregate etc.
li. Minimize Ground Disturbance (Part 3.7.8.). Describe measures to be taken to minimize ground disturbance and preserve pre-existing stabilization measures of earth material as much as possible. Describe how these BMPs will address the specific criteria stated in Part 2.1.1. of the General Permit as applicable and relevant.
Control measure: heavy equipment and small repeated forces Implementation: During construction, soil compaction should be minimized. This includes compaction by heavy equipment and small repeated forces such as persistent foot traffic. Limiting on-site stockpiling of materials and parking also reduces soil compaction. Staging areas should be chosen carefully, located preferably in an area that is already designed to be a hard surface such as a driveway, patio, or plaza, and well away from important trees or sensitive areas. In some cases, it is possible to use existing roads or parking areas adjacent to the site for materials storage.
i. Ground Water Dewatering (Part 3. 7.9.). Describe BMPs and locations used for ground water dewatering at the site. Describe how these BMPs will address the specific criteria stated in Part 2.1.3. of the General Permit as applicable and relevant. For discharges of ground water due to dewatering practices which will not discharge to state surface waters, be sure to address any design, installation, and maintenance protocol as applicable and pertinent to the BMPs.

J. Operational Controls (Part 3. 7.10.). If not addressed elsewhere, address BMPs which will be used on a day to-day basis on the site to reduce or eliminate the contribution of potential pollutants in storm water runoff such as good housekeeping activities to maintain a clean and orderly site, and removal of accumulated sediment on or off the site as much as practicable. This is the section to specifically describe and locate ah BMPs implemented at the site for the potential pollutant sources listed in Sections H7, H8, H9, and H15 of this SWPPP Form. Describe how these BMPs will address the specific criteria stated in Parts 2.1.4. and
2.1.5. of the General Permit as applicable and relevant. Be sure to address any design, installation, and maintenance protocol as applicable and pertinent to the BMPs.
Corrective actions will be taken to:
• Repair, modify, or replace any stormwater control BMP used at the site;
•Replace/modify BMPs that were initially installed incorrectly or never installed;
• Replace BMPs that are ineffective in preventing/minimizing sediment discharges in accordance with applicable water quality standards;
• Clean up and properly dispose of spills, releases, or other deposits, or
• Remedy a permit violation
k. Spill Prevention and Response Procedures (Part 3. 7.11. ). Describe what procedures will be used to prevent and respond to spills, leaks, and other releases. This is the section to specifically describe all BMPs implemented at the site for the potential pollutant sources listed in Sections H15 of this SWPPP Form.
Describe how these BMPs will address the specific criteria stated in Parts 2.1.4. and 2.1. 5. of the General Permit as applicable and relevant.
Prior to earth-disturbing activities or pollutant-generating activities, the following construction personnel will receive trair to understand the requirements of the EPA/State NPDES stormwater permit and their specific responsibilities:
• Personnel responsible for installation, maintenance, or repair of stormwater control BMPs;
• Personnel responsible for the application and storage of treatment chemicals (if applicable);
• Personnel responsible for conducting inspections;
• Personnel responsible for taking corrective actions

5.1. General Definitions and Abbreviations
The purpose of this Preamble is to provide the construction project "owner or operator" (permittee) who submits a Notice of Intent Package for a "storm water discharge associated with construction activity" under the "General Permit for Storm Water Discharges Associated with Construction Activity"· (permit) with a summary of the requirements of this permit.
The basic principle of the permit is to identify pollutant sources associated with construction activities and install and maintain Best Management Practices {BMPs) reduce the potential discharge of pollutants from the construction project The degree of pollution control necessary will vary depending on the site and the associated construction activity, but the BMPs and pollution control measures must collectively prevent exceedances of applicable water quality standards in state surface waters.
The primary potential pollutant from construction activities is sediment. Adequate erosion and sediment control BMPs must be used to minimize or prevent sediment discharges from the construction activity to state surface waters.
Other potential pollutants likely to be a problem sites are fuels, lubricating oils, construction materials, various wastes, fertilizers, or pesticides. Proper management of these materials is essential to ensure potential pollutants are not discharged from the construction activity to state surface waters.
In order to achieve compliance with the conditions of this permit, the permittee is required to address the effluent limitations in Part 2 by developing a Storm Water Pollution Prevention Pian (SWPPP) as described in Part 3. The SWPPP must clearly address the effluent limitations and the selected Best Management Practices to be used to manage pollutant sources and ensure appropriate protection of state surface waters.
The permit requires that the site reach "final stabilization" before permit coverage may be terminated. In Montana's semi-arid climate, the time necessary to achieve this "final stabilization" often requires permit coverage well beyond…
