PAST PERFORMANCE QUESTIONNAIRE
CONTRACTOR PERFORMANCE CUSTOMER QUESTIONNAIRE
(THIS DOCUMENT IS CONSIDERED TO BE SOURCE SELECTION SENSITIVE INFORMATION)
Past performance references shall be as a prime Contractor or first-tier subcontractor on previous or current projects/tasks that are similar in size, scope, and complexity to the work identified in the Statement of Work (SOW). Past performance does not necessarily have to be Government-related; but it is important to demonstrate and provide evidence of past performance for work similar to that described in the Screening for Information request (SIR). The Offeror is required to clearly demonstrate management actions implemented to overcome problems and the effects of those actions, in terms of improvements achieved or problems solved.
At least two Past Performance Questionnaire’s must be submitted by the prime and at least one from each major subcontractor but the total of Past Performance Questionnaire’s submitted to the FAA shall not exceed a total of ten submissions combined. Contracts listed may include those entered into by the federal government, agencies of state and local governments, and commercial customers.
INSTRUCTIONS TO OFFERORS SENDING QUESTIONNAIRE FORMS:
Part I shall be completed by the Offeror (the firm requesting the reference) prior to issuing the questionnaires. The Offeror shall send the questionnaires for relevant projects that are in progress or completed in the past seven (7) years to the individuals who will provide the reference. For Government contracts, send to the Contracting Officer (CO) or CO’s Representative/Technical Representative. For commercial references, send to personnel with duties similar to those for Government contracts. It is the responsibility of the Offeror to follow-up and encourage references to submit the questionnaire. The completed questionnaire shall be submitted via e-mail directly from the person providing the reference. In addition, copies of Part I of all requested questionnaire forms by the Offeror to the Reference source must be included with the Offeror’s proposal due 3:00 PM EDT, on October 22, 2016.
INSTRUCTIONS TO PERSON PROVIDING REFERENCE:
In order to assess Offeror past performance under the NAVTAC II competition, the FAA contracting office is obtaining past performance information with respect to each Offeror. As a reference for work performed by the firm identified in Part I of this questionnaire, your candid response will assist in the evaluation process. Please complete Part II of the questionnaire as thoroughly as possible and submit to iann.washington@faa.gov no later than 3:00 PM EDT, on October 22, 2016. The subject line in the submission e-mail shall clearly indicate: NAVTAC II Past Performance Survey Submission for (Offeror name cited in Part I) and the message shall originate from the reference’s government or commercial e-mail system.
In addition to this questionnaire, you may receive a follow-up phone call to confirm or clarify information. Your input into this evaluation is greatly appreciated. If you have questions regarding this questionnaire or require assistance, please submit inquiries to: 9-AWA-AAQ-NAVTACII@faa.gov.
