About this file

The document is a Disqualification Disclosure form in compliance with California Public Contract Code §10162, specifically designed for prospective bidders in the On-Call Geotechnical Engineering Services contract opportunity with the City of California. The form requires bidders to disclose whether the company, its officers, or employees with proprietary interest have ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.

The disclosure form is a standard compliance document that allows bidders to transparently report any past issues that might impact their eligibility for the geotechnical engineering services contract. Bidders must check either "Yes" or "No" to the disqualification question, with the option to attach explanatory documents if they have affirmative responses. This form is part of the City's due diligence process to ensure contractor reliability and adherence to legal and safety standards for the proposed on-call geotechnical engineering services project.
