About this file
This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for the Enterprise Monitoring project (P-57907). The form serves as a mandatory questionnaire that prospective bidders must complete under penalty of perjury, requiring disclosure of whether the bidder, any officer of the bidder, or any employee with proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate either "Yes" or "No" in response to this inquiry, with instructions to explain circumstances and attach supporting documentation if the answer is affirmative.
The disclosure form contains no pricing terms, set-asides, incumbent information, or funding details, as it is purely a compliance and qualification document. This form is a prerequisite requirement for bidders on the City of Pasadena's Enterprise Monitoring solution project, which seeks a unified monitoring and observability platform to strengthen alert management and monitoring processes across the City's hybrid and multi-cloud infrastructure. All prospective vendors must complete and submit this form as part of their bid package to demonstrate their eligibility and compliance with California contracting regulations.
